            "abstractNote": "Context  \n                  The quality of consumer health information on the World Wide Web is \nan important issue for medicine, but to date no systematic and comprehensive \nsynthesis of the methods and evidence has been performed.Objectives  \n                  To establish a methodological framework on how quality on the Web is \nevaluated in practice, to determine the heterogeneity of the results and conclusions, \nand to compare the methodological rigor of these studies, to determine to \nwhat extent the conclusions depend on the methodology used, and to suggest \nfuture directions for research.Data Sources  \n                  We searched MEDLINE and PREMEDLINE (1966 through September 2001), Science \nCitation Index (1997 through September 2001), Social Sciences Citation Index \n(1997 through September 2001), Arts and Humanities Citation Index (1997 through \nSeptember 2001), LISA (1969 through July 2001), CINAHL (1982 through July \n2001), PsychINFO (1988 through September 2001), EMBASE (1988 through June \n2001), and SIGLE (1980 through June 2001). We also conducted hand searches, \ngeneral Internet searches, and a personal bibliographic database search.Study Selection  \n                  We included published and unpublished empirical studies in any language \nin which investigators searched the Web systematically for specific health \ninformation, evaluated the quality of Web sites or pages, and reported quantitative \nresults. We screened 7830 citations and retrieved 170 potentially eligible \nfull articles. A total of 79 distinct studies met the inclusion criteria, \nevaluating 5941 health Web sites and 1329 Web pages, and reporting 408 evaluation \nresults for 86 different quality criteria.Data Extraction  \n                  Two reviewers independently extracted study characteristics, medical \ndomains, search strategies used, methods and criteria of quality assessment, \nresults (percentage of sites or pages rated as inadequate pertaining to a \nquality criterion), and quality and rigor of study methods and reporting.Data Synthesis  \n                  Most frequently used quality criteria used include accuracy, completeness, \nreadability, design, disclosures, and references provided. Fifty-five studies \n(70%) concluded that quality is a problem on the Web, 17 (22%) remained neutral, \nand 7 studies (9%) came to a positive conclusion. Positive studies scored \nsignificantly lower in search (P = .02) and evaluation \n(P = .04) methods.Conclusions  \n                  Due to differences in study methods and rigor, quality criteria, study \npopulation, and topic chosen, study results and conclusions on health-related \nWeb sites vary widely. Operational definitions of quality criteria are needed.",

            "abstractNote": "Objective  \n    To examine adolescents' use of and attitudes toward accessing health information through the Internet.Design  \n    Cross-sectional, school-based survey.Participants  \n    A socioeconomically and ethnically diverse sample of 412 suburban New York 10th graders (mean [SD] age, 15.8 [0.68] years).Main Outcome Measures  \n    Accessing the Internet for health information.Results  \n    Half (49%) of the sampled adolescents had used the Internet to get health information. Topics most often explored through the Internet included sexually transmitted diseases; diet, fitness, and exercise; and sexual behaviors. Adolescents found Internet information to be of high value (using a composite gauging worth, trustworthiness, use, and relevance), with no significant differences related to sex, ethnicity, or mother's education. When considering 11 separate health topics, girls found it more valuable to have information on birth control, diet and nutrition, exercise, physical abuse, sexual abuse, and dating violence. Only for alternative medicine were there differences by ethnicity, and there were no differences based on mother's education for the value of having specific health information available through the Internet.Conclusion  \n    For adolescents, the Internet is an accessed and valued information source on a range of sensitive health issues.",

Without accurate estimates \nof use and effects, it is difficult to focus policy discussions or design \nappropriate policy activities.Objectives  \n                  To measure the extent of Internet use for health care among a representative \nsample of the US population, to examine the prevalence of e-mail use for health \ncare, and to examine the effects that Internet and e-mail use has on users' \nknowledge about health care matters and their use of the health care system.Design, Setting, and Participants  \n                  Survey conducted in December 2001 and January 2002 among a sample drawn \nfrom a research panel of more than 60 000 US households developed and \nmaintained by Knowledge Networks. Responses were analyzed from 4764 individuals \naged 21 years or older who were self-reported Internet users.Main Outcome Measures  \n                  Self-reported rates in the past year of Internet and e-mail use to obtain \ninformation related to health, contact health care professionals, and obtain \nprescriptions; perceived effects of Internet and e-mail use on health care \nuse.Results  \n                  Approximately 40% of respondents with Internet access reported using \nthe Internet to look for advice or information about health or health care \nin 2001. Six percent reported using e-mail to contact a physician or other \nhealth care professional. Both public- and private-sector stakeholders, particularly government agencies and private corporations, will need to collaboratively reduce the gap between the health information \"haves\" and \"have-nots.\" This will include supporting health information technology access in homes and public places, developing applications for the growing diversity of users, funding research on access-related issues, ensuring the quality of health information and support, enhancing literacy in health and technology, training health information intermediaries, and integrating the concept of universal access to health information and support into health planning processes.",

            "abstractNote": "Increasingly, consumers engage in health information seeking via the Internet. Taking a communication perspective, this review argues why public health professionals should be concerned about the topic, considers potential benefits, synthesizes quality concerns, identifies criteria for evaluating online health information and critiques the literature. More than 70 000 websites disseminate health information; in excess of 50 million people seek health information online, with likely consequences for the health care system. The Internet offers widespread access to health information, and the advantages of interactivity, information tailoring and anonymity. However, access is inequitable and use is hindered further by navigational challenges due to numerous design features (e.g. disorganization, technical language and lack of permanence). Increasingly, critics question the quality of online health information; limited research indicates that much is inaccurate. Meager information-evaluation skills add to consumers' vulnerability, and reinforce the need for quality standards and widespread criteria for evaluating health information. Extant literature can be characterized as speculative, comprised of basic `how to' presentations, with little empirical research. Future research needs to address the Internet as part of the larger health communication system and take advantage of incorporating extant communication concepts. Not only should research focus on the `net-gap' and information quality, it also should address the inherently communicative and transactional quality of Internet use. Both interpersonal and mass communication concepts open avenues for investigation and understanding the influence of the Internet on health beliefs and behaviors, health care, medical outcomes, and the health care system.",

            "abstractNote": "Background  \n    The context in which patients consume health information has changed dramatically with diffusion of the Internet, advances in telemedicine, and changes in media health coverage. The objective of this study was to provide nationally representative estimates for health-related uses of the Internet, level of trust in health information sources, and preferences for cancer information sources.Methods  \n    Data from the Health Information National Trends Survey were used. A total of 6369 persons 18 years or older were studied. The main outcome measures were online health activities, levels of trust, and source preference.Results  \n    Analyses indicated that 63.0% (95% confidence interval [CI], 61.7%-64.3%) of the US adult population in 2003 reported ever going online, with 63.7% (95% CI, 61.7%-65.8%) of the online population having looked for health information for themselves or others at least once in the previous 12 months. Despite newly available communication channels, physicians remained the most highly trusted information source to patients, with 62.4% (95% CI, 60.8%-64.0%) of adults expressing a lot of trust in their physicians. When asked where they preferred going for specific health information, 49.5% (95% CI, 48.1%-50.8%) reported wanting to go to their physicians first. When asked where they actually went, 48.6% (95% CI, 46.1%-51.0%) reported going online first, with only 10.9% (95% CI, 9.5%-12.3%) going to their physicians first.Conclusion  \n    The Health Information National Trends Survey data portray a tectonic shift in the ways in which patients consume health and medical information, with more patients looking for information online before talking with their physicians.",

Concerns include whether patients' access to large volumes of information will improve their health; whether the variable quality of the information will have a deleterious effect; the effect on health disparities; and whether the physician-patient relationship will be improved as patients become more equal partners, or be damaged if physicians have difficulty adjusting to a new role.Methods  \n    Telephone survey of nationally representative sample of the American public, with oversample of people in poor health.Results  \n    Of the 3209 respondents, 31% had looked for health information on the Internet in the past 12 months, 16% had found health information relevant to themselves and 8% had taken information from the Internet to their physician. Looking for information on the Internet showed a strong digital divide; however, once information had been looked for, socioeconomic factors did not predict other outcomes. Most (71%) people who took information to the physician wanted the physician's opinion, rather than a specific intervention. The effect of taking information to the physician on the physician-patient relationship was likely to be positive as long as the physician had adequate communication skills, and did not appear challenged by the patient bringing in information.Conclusions  \n    For health information on the Internet to achieve its potential as a force for equity and patient well-being, actions are required to overcome the digital divide; assist the public in developing searching and appraisal skills; and ensure physicians have adequate communication skills.",
