Two hundred women who developed CVD were matched 1:1 by age, smoking, and postmenopausal hormone therapy (HT) to controls who remained free of CVD. We measured testosterone, estradiol, and sex hormone binding globulin (SHBG) levels and calculated free androgen index (FAI), free estradiol index, and the FAI/free estradiol index ratio. Results were stratified by HT use. Among HT nonusers, cases had significantly higher androgen profiles (higher median FAI and lower SHBG levels) than controls. After adjustment for age, smoking, use of aspirin, vitamin E, and alcohol, family history of myocardial infarction, and physical activity, nonusers in the lowest SHBG quartile had an OR of 2.25 (95% CI, 1.03 to 4.91) for CVD, and there were significant trends across FAI quartiles (P for trend=0.03). Additional adjustment for body mass index, hypertension, diabetes, and elevated cholesterol eliminated associations with SHBG and FAI. Among women using HT, no significant differences in hormones or SHBG were observed among women who developed CVD and controls.\nConclusions— Among HT nonusers, lower SHBG and higher FAI levels were noted among postmenopausal women who developed CVD events, but this was not independent of body mass index and other cardiovascular risk factors. Severe malnourishment is found to be higher in Mumbai than in Jawhar. A connection is postulated between characteristics of urban informal labour markets, specially for women's labour, and the high observed incidence of malnutrition among children. The paper discusses policy mixes for minimising the incidence of malnutrition in this population. A policy that combines income improvement with greater public health facilities and legislation is most likely to succeed in Mumbai. The Physicians' Health Study (PHS), a randomized trial of beta-carotene and aspirin in the primary prevention of cancer and cardiovascular disease in men, provided an opportunity to examine the association between height and total malignant neoplasms (excluding non-melanoma skin cancer), as well as site-specific cancers including prostate, colorectal, and lung cancer. The PHS is comprised of 22,071 US male physicians in the United States, a population homogeneous for adult socioeconomic status, aged 40 to 84 years in 1982. Participants were classified into five height categories at study entry. After an average follow-up of over 12 years, there were 2,566 cases of incident total malignant neoplasms, including 1,047 prostate, 341 colorectal, and 170 lung cancer cases. Height was associated positively with both total malignant neoplasms and prostate cancer. Compared with men in the shortest category(≤ 67 inches), relative risks and 95 percent confidence intervals (CI)for total malignant neoplasms for men whose height (in inches) was 68-69,70-71, 72, and 73+ were, respectively: 1.13 (CI = 0.99-1.28), 1.15 (CI =1.02-1.30), 1.29 (CI = 1.12-1.49), and 1.21 (CI = 1.05-1.39), P trend 0.001,adjusted for age, randomized treatment assignments, body mass index (wt/ht2), cigarette smoking, alcohol use, and exercise frequency. For prostatecancer, the corresponding RR values were 1.23 (CI = 1.00-1.51), 1.26 (CI =1.04-1.54), 1.59 (CI = 1.27-1.98), and 1.26 (CI = 1.00-1.59), P trend 0.005.For colorectal cancer, in some but not all height categories compared with the shortest, there were elevated RRs without a significant linear trend: RR= 1.51 (CI = 1.06-2.14), 1.14 (CI = 0.80-1.62), 1.19 (CI = 0.79-1.80), and1.53 (CI = 1.04-2.25), P trend 0.23. In contrast, there was no evidence of an association of height with lung cancer. Based on previous indirect evidence, we hypothesized that this senescent decline occurs because growth plate stem-like cells, located in the resting zone, have a finite proliferative capacity that is gradually depleted. Consistent with this hypothesis, we found that the proliferation rate in rabbit resting zone chondrocytes (assessed by continuous 5-bromo-2′-deoxy-uridine labeling) decreases with age, as does the number of resting zone chondrocytes per area of growth plate.\nGlucocorticoid excess slows growth plate senescence. To explain this effect, we hypothesized that glucocorticoid inhibits resting zone chondrocyte proliferation, thus conserving their proliferative capacity. Consistent with this hypothesis, we found that dexamethasone treatment decreased the proliferation rate of rabbit resting zone chondrocytes and slowed the numerical depletion of these cells. Estrogen is known to accelerate growth plate senescence. However, we found that estradiol cypionate treatment slowed resting zone chondrocyte proliferation.\nOur findings support the hypotheses that growth plate senescence is caused by qualitative and quantitative depletion of stem-like cells in the resting zone and that growth-inhibiting conditions, such as glucocorticoid excess, slow senescence by slowing resting zone chondrocyte proliferation and slowing the numerical depletion of these cells, thereby conserving the proliferative capacity of the growth plate. We searched for sequence variants that affect height by scanning the genomes of 25,174 Icelanders, 2,876 Dutch, 1,770 European Americans and 1,148 African Americans. We then combined these results with previously published results from the Diabetes Genetics Initiative on 3,024 Scandinavians and tested a selected subset of SNPs in 5,517 Danes. We identified 27 regions of the genome with one or more sequence variants showing significant association with height. The estimated effects per allele of these variants ranged between 0.3 and 0.6 cm and, taken together, they explain around 3.7% of the population variation in height. The genes neighboring the identified loci cluster in biological processes related to skeletal development and mitosis. autoregulatory effect on further pituitary growth hormone release. Similar evidence was sought in normal human volunteers. Growth hormone levels were raised either by introducing a provocative stimulus, exercise or arginine, or by infusing human growth hormone at a constant rate. When circulating levels of growth hormone were raised by a measured exercise stimulus or by an arginine infusion, a second stimulus introduced in tandem failed to cause a further rise, and circulating growth hormone levels dropped to baseline. This was not due to pituitary growth hormone exhaustion, since combining the stimuli produced a far greater growth hormone release than either stimulus alone. Normal release in response to arginine or exercise could be demonstrated if a 30-min pause was allowed between tests. Similarly, when growth hormone levels were raised by injections or with a constant growth hormone infusion, no response to exercise or to arginine was seen. Thus, in man, as in animals, it appears that circulating levels of growth hormone do exert an autoregulatory influence on the secretory activity of either the hypothalamus or the pituitary.
