Managed care, driven by the economic concerns of governments' and large employers' health and welfare benefit costs, has transformed care. In so doing, it sheds light on significant issues and leaves in the shadows other important issues. This article examines six ways that managed care has transformed caring: (a) it transforms patients from those who suffer into quasi-consumers with limited choice; (b) it transforms the medical care facility into a factory; (c) it transforms the patient into a population member; (d) it shifts cost risks from a third party to the patient and the provider; (e) it limits the reach of caring; and (f) it transforms providers into bureaucrats. We discuss two contradictions that arise through these changes, substitutability versus relationships and biotechnological knowledge versus humanistic knowledge. La controverse analysée dans cet article entend contribuer à améliorer les connaissances concernant les implications politiques de la rationalisation quantitative des pratiques médicales qui traverse la médecine depuis le milieu du XXe siècle. Elle oppose, depuis une vingtaine d'années, la communauté de la cancérologie pédiatrique française, qui s'inscrit dans une approche organisée et normalisatrice de la médecine, à un service qui défend une approche individuelle et empirique. L'étude de cette controverse démontre que les attitudes des patients, peu aidés en cela par certains des principaux protagonistes — dont les médias et les médecins eux-mêmes — , sont très ambivalentes vis-à-vis de cette rationalisation. Mais ce cas souligne surtout que les principaux changements concernent pour le moment les relations entre médecins dans la mesure où la rationalisation change le fondement de la légitimité des
