incidence of mental health problems based on the DSM-IV among adolescents in custody in Tasmania, using the Adolescent Psychopathology Scale (APS) .Methods: The APS was administered to 50 adolescents admitted consecutively to a youth detention centre in Tasmania, Australia, using a structured interview format.Results: In this sample 46% scored positively for a mood disorder, 36% for posttraumatic stress disorder (PTSD), and 32% for an anxiety disorder excluding PTSD.Conclusions: Tasmania is no exception to the rule that adolescents in custody have a high proportion of mental health problems. This study confirmed findings of previous studies that detained adolescents had a roughly equivalent level of mental health problems as young people referred to mental health services and five times more than adolescents in the community. Therefore, all detained adolescents should be assessed for mental health problems as their risk is equivalent to that of young people referred to mental health services in the community. Youth detention centres are able to provide significant opportunities for individual therapy and group programmes for young people with mental health problems. problems, prevalence of suicidal ideation and behaviour, and health-related quality of life of 13-17-year-olds on remand with that of 13-17-year-olds in the general community.\nMETHOD: Self reported questionnaires completed by 13-17-year-olds who were remanded in South Australia in 2008/9 (N = 159), 13-17-year-olds who participated in the Child and Adolescent Component of the National Survey of Mental Health and Well-being in Australia (N = 1283), and 13-17-year-olds who participated in the Western Australian Aboriginal Child Health Survey (N = 1100). Mental health problems were identified using the Youth Self-Report and the Strength and Difficulties Questionnaire. Health-related quality of life was assessed using the Child Health Questionnaire.\nRESULTS: A total of 50.0% (95% CI, 42.3-58.3) of adolescents on remand versus 18.9% of adolescents in the community scored above the recommended cut-off score on the Youth Self-Report. Among Indigenous adolescents, 55.8% (95% CI, 41.3-69.5) on remand versus 32.1% in the community scored above the recommended cut-off score on the Strength and Difficulties Questionnaire. Among those on remand, 19.1% (95% CI, 13.2-26.2) reported making a suicide attempt during the previous 12 months compared to 4.3% in the community. Health-related quality of life was significantly worse among adolescents on remand than adolescents in the community. These differences remained after adjusting for differences in the demographic characteristics of the two groups of adolescents.\nCONCLUSIONS: Compared with adolescents in the community, both Indigenous and non-Indigenous adolescents on remand experience a wide range of problems, including poorer mental and physical health, a higher prevalence of suicidal ideation and behaviour, greater family adversity, and poorer school attendance. This broad range of problems needs to be effectively addressed to enable adolescents on remand to become active and productive members of their communities.", "publicationTitle": "The Australian and Non‐Aboriginal adolescents aged 12–18 years. Anxiety, depression, hopelessness, locus of control, self‐efficacy beliefs and coping or problem beliefs were examined. Results indicate that Aboriginal adolescents experience a greater sense of anxiety about custody than Non‐Aboriginal adolescents. This level of anxiety experienced appears related to anxiety beliefs regarding their experience of custody and a significantly high level of negative beliefs about custody. Aboriginal adolescents were also found to have poorer coping skills linked to lower self‐efficacy and more external locus of control than Non‐Aboriginal adolescents. Over the study period, all Indigenous young people admitted into custody were referred for screening with the WASC-Y, a culturally validated fivescaled instrument developed to identify Indigenous young people at risk for a range of mental health or substance abuse problems. The WASC-Y also incorporates a separate scale for cultural resilience. High levels of mental health and substance abuse problems were reported, with 94.6% of males and 100% of females surveyed screening above the designated clinical cut-off on at least one scale. Based on moderate/medium levels of risk, 27.7% screened positive for depression, 34.0% for suicide ideation or intent, 89.4% for substance use, 36.2% for impulsivity, and 68.1% for symptoms of anxiety. Females screened higher than males across all five subscales, though differences reached statistical significance only for suicidal ideation or intent. Cultural resilience as a protective factor for mental health problems (excluding substance abuse) approached clinical significance. Given their pervasively high levels of psychological distress, it is recommended that on admission into custody all Indigenous youth are referred for a mental health assessment and substance abuse counselling.
