            "abstractNote": "OBJECTIVE: The goal of this work is to evaluate if there is an increase in the risk of thromboembolic events (TEEs) due to concomitant exposure to dexamethasone and apixaban or rivaroxaban. Direct oral anticoagulants (DOACs), as well as corticosteroid dexamethasone, are commonly used to treat individuals hospitalised with COVID-19. Dexamethasone induces cytochrome P450-3A4 enzyme that also metabolises DOACs apixaban and rivaroxaban. This raises a concern about possible interaction between dexamethasone and DOACs that may reduce the efficacy of the DOACs and result in an increased risk of TEE.\nDESIGN: We used nested case-control study design.\nSETTING: This study was conducted in the National COVID Cohort Collaborative (N3C), the largest electronic health records repository for COVID-19 in the USA.\nPARTICIPANTS: Study participants were adults over 18 years who were exposed to a DOAC for 10 or more consecutive days. Exposure to dexamethasone was at least 5 or more consecutive days.\nPRIMARY AND SECONDARY OUTCOME MEASURES: Our primary exposure variable was concomitant exposure to dexamethasone for 5 or more days after exposure to either rivaroxaban or apixaban for 5 or more consecutive days. We used McNemar's Χ2 test and adjusted logistic regression to evaluate association between concomitant use of dexamethasone with either apixaban or rivaroxaban.\nRESULTS: McNemar's Χ2 test did not find a discernible association of TEE in patients concomitantly exposed to dexamethasone and a DOAC (χ2=0.5, df=1, p=0.48). In addition, a conditional logistic regression model did not find an increase in the risk of TEE (adjusted OR 1.15, 95% CI 0.32 to 4.18).\nCONCLUSION: This nested case-control study did not find evidence of an association between concomitant exposure to dexamethasone and a DOAC with an increase in risk of TEE. Due to small sample size, an association cannot be completely ruled out.",

            "abstractNote": "BACKGROUND: Untreated pain is associated with short-term and long-term consequences, including post-traumatic stress disorder and insomnia. Side effects of some analgesic medications include dysphoria, hallucinations and delirium. Therefore, both untreated pain and analgesic medications may be risk factors for delirium. Delirium is associated with longer length of stay or cognitive impairment. Our systematic review and meta-analysis will examine the relationship between pain or analgesic medications with delirium occurrence, duration and severity among critically ill adults.\nMETHODS AND ANALYSIS: MEDLINE, EMBASE, CINAHL, the Cochrane Central Register of controlled trials and a review of recent conference abstracts will be searched without restriction from inception to 15 May 2023. Study inclusion criteria are: (1) age≥18 years admitted to intensive care; (2) report a measure of pain, analgesic medications and delirium; (3) study design-randomised controlled trial, quasiexperimental designs and observational cohort and case-control studies excluding case reports. Study exclusion criteria are: (1) alcohol withdrawal delirium or delirium tremens; or (2) general anaesthetic emergence delirium; or (3) lab or animal studies. Risk of bias will be assessed with the Risk of Bias V.2 and risk of bias in non-randomised studies tools. There is no language restriction. Occurrence estimates will be transformed using the Freeman-Tukey double arcsine. Point estimates will be pooled using Hartung-Knapp Sidik-Jonkman random effects meta-analysis to estimate a pooled risk ratio. Statistical heterogeneity will be estimated with the I2 statistic. Risk of small study effects will be assessed using funnel plots and Egger test. Studies will be analysed for time-varying and unmeasured confounding using E values.\nETHICS AND DISSEMINATION: Ethical approval is not required as this is an analysis of published aggregated data. We will share our findings at conferences and in peer-reviewed journals.\nPROSPERO REGISTRATION NUMBER: The finalised protocol was submitted to the International Prospective Register of Systematic Reviews (PROSPERO ID: CRD42022367715).",

            "abstractNote": "Interpretability and predictive performance are important aspects of a machine learning model. Typically, there is a trade-off between interpretability and predictive performance. This trade-off results in a choice between accurate but opaque models such as multilayer perceptron (MLP) and less accurate but more transparent models such as logistic regression (LR). In the healthcare domain, model interpretability is especially important because the real-life goals (e.g. patient well-being) are hard to model (and thus optimize) formally. Traditional methods such as LR \\& MLP use aggregate features and are therefore not able to effectively model temporal dimension that is inherent in Electronic Health Records (EHR) data. Recently, Recurrent Neural Network (RNN) approaches have been successful in modelling healthcare data because they are able to effectively take the temporal dimension into account. However, the RNN model is notoriously hard to interpret. We have looked at three recently proposed RNN-based models for medical event prediction that claim to be interpretable (Dipole, GRNN-HA and RETAIN). The interpretability of these models is tied to the implementation of a neural attention mechanism. Having considered how well the models are able to relate the input to the output in understandable terms, we devised an ordering of the interpretability of these models. Then, we compared performance in predicting 30 re-hospitalisation on an EHR dataset with 37,287 medical histories using admission and diagnosis data. The interpretability/performance trade-off within the three `interpretable' models was partly observed. Although the performance of the RNN-based models was quite similar, the difference in interpretability is more substantial. Therefore we believe that the interpretable RNN-based models are the better overall option to use for predicting events in the healthcare domain.",

            "abstractNote": "Integrating large language models (LLMs) like GPT-4 into medical ethics is a novel concept, and understanding the effectiveness of these models in aiding ethicists with decision-making can have significant implications for the healthcare sector. Thus, the objective of this study was to evaluate the performance of GPT-4 in responding to complex medical ethical vignettes and to gauge its utility and limitations for aiding medical ethicists. Using a mixed-methods, cross-sectional survey approach, a panel of six ethicists assessed LLM-generated responses to eight ethical vignettes.The main outcomes measured were relevance, reasoning, depth, technical and non-technical clarity, as well as acceptability of GPT-4’s responses. The readability of the responses was also assessed. Of the six metrics evaluating the effectiveness of GPT-4’s responses, the overall mean score was 4.1/5. GPT-4 was rated highest in providing technical (4.7/5) and non-technical clarity (4.4/5), whereas the lowest rated metrics were depth (3.8/5) and acceptability (3.8/5). There was poor-to-moderate inter-rater reliability characterised by an intraclass coefficient of 0.54 (95% CI: 0.30 to 0.71). Based on panellist feedback, GPT-4 was able to identify and articulate key ethical issues but struggled to appreciate the nuanced aspects of ethical dilemmas and misapplied certain moral principles.This study reveals limitations in the ability of GPT-4 to appreciate the depth and nuanced acceptability of real-world ethical dilemmas, particularly those that require a thorough understanding of relational complexities and context-specific values. Ongoing evaluation of LLM capabilities within medical ethics remains paramount, and further refinement is needed before it can be used effectively in clinical settings.All data relevant to the study are included in the article or uploaded as supplementary information.",

            "abstractNote": "Background\nFalls and their consequences are particularly common in older adults in hospitals and long‐term care (LTC) facilities.\n\nAim\nTo avoid falls and their consequences, and provide nurses with an overview of all relevant research literature on fall prevention, and a practice guideline on fall prevention in older adults was developed.\n\nMethods\nThe development process included a systematic literature review to identify systematic reviews and primary studies on the topic of fall prevention, an assessment of the study quality, the preparation of meta‐analyses to summarize the results, and the application of the GRADE (Grading of Recommendations Assessment, Development and Evaluation) approach to grade the scientific literature. The guideline panel and an external multidisciplinary team graded the recommendations using the Delphi method. In addition, the panel and team formulated expert opinions.\n\nResults\nA total of 79 randomized controlled trials on fall prevention were identified, which formed the basis of the recommendations. Strongly recommended measures for both settings included multifactorial interventions, professionally supported body exercise interventions, and education and counselling interventions. The panel and team did not recommend the use of a specific assessment tool for fall risk assessment, low‐floor beds in hospitals, or body exercise interventions in frail residents.\n\nLinking Evidence to Action\nDuring the development of this guideline, particular attention was paid to collecting evidence‐based knowledge relevant to practice. By applying the recommendations, the outlined nursing care is justified, enabling healthcare personnel to achieve the overriding goal of providing optimal care to persons at risk of falling. Evidence for several fall prevention interventions was graded as very low. Sound intervention studies are necessary to strengthen the confidence in the evidence for low‐floor beds, alarm sensor systems, medication review, and staff education in hospitals.",

            "abstractNote": "Introduction:\nThis study: (1) provides annual population estimates of fatal and nonfatal injury incidence rates for older adults for 2004–2017; (2) determines if trends differ by whether the injury was fatal or nonfatal, a fall or non-fall injury, and, for nonfatal injuries, minor or serious; and (3) investigates whether trends vary by age, sex, and race.\n\nMethods:\nThis study used National Vital Statistics System and National Health Interview Survey data covering the population of adults aged ≥65 for 2004–2017. Fatal injury incidence rates were estimated using negative binomial models; nonfatal injury incidence rates were estimated using Poisson models. All models compared overall risk and trend differences by year, age, sex, and race, and interactions between year and age, sex, and race. All analyses were conducted in 2019.\n\nResults:\nFatal injury incidence was stable over time, but this apparent stability masked a 35% increase in fatal falls and a 17% decrease in fatal non-fall injuries. Increases in fall-related deaths were concentrated among those aged ≥85 years, men, and white older adults. The trend in fatal falls accelerated over time for those aged ≥85 years and white older adults. By contrast, there was a large increase in nonfatal injury incidence, occurring across all injury types. Nonfatal injury risk grew with age, and was higher for women and white older adults, but trends did not vary by age, sex, or race.\n\nConclusions:\nLarge increases in fatal and nonfatal injuries underscore the urgency of national implementation of fall prevention programs, and expanding fall prevention efforts to more general injury prevention.",

            "abstractNote": "Interpretability and predictive performance are important aspects of a machine learning model. Typically, there is a trade-off between interpretability and predictive performance. This trade-off results in a choice between accurate but opaque models such as multilayer perceptron (MLP) and less accurate but more transparent models such as logistic regression (LR). In the healthcare domain, model interpretability is especially important because the real-life goals (e.g. patient well-being) are hard to model (and thus optimize) formally. Traditional methods such as LR \\& MLP use aggregate features and are therefore not able to effectively model temporal dimension that is inherent in Electronic Health Records (EHR) data. Recently, Recurrent Neural Network (RNN) approaches have been successful in modelling healthcare data because they are able to effectively take the temporal dimension into account. However, the RNN model is notoriously hard to interpret. We have looked at three recently proposed RNN-based models for medical event prediction that claim to be interpretable (Dipole, GRNN-HA and RETAIN). The interpretability of these models is tied to the implementation of a neural attention mechanism. Having considered how well the models are able to relate the input to the output in understandable terms, we devised an ordering of the interpretability of these models. Then, we compared performance in predicting 30 re-hospitalisation on an EHR dataset with 37,287 medical histories using admission and diagnosis data. The interpretability/performance trade-off within the three `interpretable' models was partly observed. Although the performance of the RNN-based models was quite similar, the difference in interpretability is more substantial. Therefore we believe that the interpretable RNN-based models are the better overall option to use for predicting events in the healthcare domain.",

            "abstractNote": "This paper explores the possibility of using multiplicative gates to build two recurrent neural network structures. These two structures are called Deep Simple Gated Unit (DSGU) and Simple Gated Unit (SGU), which are structures for learning long-term dependencies. Compared to traditional Long Short-Term Memory (LSTM) and Gated Recurrent Unit (GRU), both structures require fewer parameters and less computation time in sequence classification tasks. Unlike GRU and LSTM, which require more than one gate to control information flow in the network, SGU and DSGU only use one multiplicative gate to control the flow of information. We show that this difference can accelerate the learning speed in tasks that require long dependency information. We also show that DSGU is more numerically stable than SGU. In addition, we also propose a standard way of representing the inner structure of RNN called RNN Conventional Graph (RCG), which helps to analyze the relationship between input units and hidden units of RNN.",

            "abstractNote": "Learning to store information over extended time intervals by recurrent backpropagation takes a very long time, mostly because of insufficient, decaying error backflow. We briefly review Hochreiter's (1991) analysis of this problem, then address it by introducing a novel, efficient, gradient based method called long short-term memory (LSTM). Truncating the gradient where this does not do harm, LSTM can learn to bridge minimal time lags in excess of 1000 discrete-time steps by enforcing constant error flow through constant error carousels within special units. Multiplicative gate units learn to open and close access to the constant error flow. LSTM is local in space and time; its computational complexity per time step and weight is O. 1. Our experiments with artificial data involve local, distributed, real-valued, and noisy pattern representations. In comparisons with real-time recurrent learning, back propagation through time, recurrent cascade correlation, Elman nets, and neural sequence chunking, LSTM leads to many more successful runs, and learns much faster. LSTM also solves complex, artificial long-time-lag tasks that have never been solved by previous recurrent network algorithms.",
