This study by the Center for Studying Health System Change (HSC) documents strategies that a range of physician practices use to coordinate care for their patients. While there was no single recipe for coordination given the variety of patient, physician, practice and market factors, some cross-cutting lessons were identified, such as the value of a commitment to interpersonal continuity of care as a foundation for coordination. Respondents also identified the importance of system support for the standardization of office processes to foster care coordination. While larger practices may have more resources to invest, many of the innovations described could be scaled to smaller practices. Some coordination strategies resulted in improved efficiency over time for practices, but by and large, physician practices currently pursue these efforts at their own expense. In addition to sharing information on effective strategies among practices, the findings also provide policy makers with a snapshot of the current care coordination landscape and implications for initiatives to improve coordination. Efforts to provide technical support to practices to improve coordination, for example, through medical-home initiatives, need to consider the baseline more typical practices may be starting from and tailor their support to practices ranging widely in size, resources and presence of standardized care processes. If aligned with payment incentives, some of these strategies have the potential to increase quality and satisfaction among patients and providers by helping to move the health care delivery system NPs are preparing for this future by developing doctoral-level training programs in comprehensive care, whereas PAs are adding training opportunities in specific specialties. Yet, neither discipline has expanded its training capacity to the degree that will be required, and, like physicians, neither will have a supply of practitioners that will match future demand. However, the majority of states across the country restrict the NP ability to provide primary care. As of 2015, only 21 states offer NP’s full practice authority.Data Sources: A comprehensive literature search was conducted using data bases by PubMed, and MeSH, EBM Search, CINAHL, and Google Scholar. Thirty articles were chosen for synthesis.Results: Several barriers impede the ability of NPs to practice autonomously including lack of formal business or marketing education, reduced reimbursement rates, and lack of recognition from public and other health professionals about their preparation. The most common barrier is restrictive legislation halting NP practice at the level of their full knowledge and preparation. No articles were found identifying specific barriers to implementing FPA legislation.Implications: There is a gap in the literature identifying specific barriers to implementing successful legislation to remedy this problem. Content analysis was performed using grounded theory and ATLAS.ti 5.0.\n\nResults\nWe found that paper-based leaders prioritized the following: sufficient workstations and printers, a physician information technology (IT) champion at the practice, workflow education to ensure a successful transition to a paperless medical practice, and a high existing comfort level of practitioners and support staff with IT. In contrast, EHR-based leaders prioritized: improved technical training and ongoing technical support, sufficient protection of patient privacy, and open recognition of physician resistance, especially for those who were loyal to a legacy EHR. Unlike paper-based practices, EHR-based leadership believed that comfort level with IT and adjustments to workflow changes would not be difficult challenges to overcome.\n\nConclusions\nLeadership at paper- and EHR-based practices in 1 academic network has different priorities for implementing a new EHR.
