(T2DM) is a modern day epidemic. Chronic course of diabetes is detrimental to the cognitive functions.\nAim\nTo decipher the pattern of cognitive impairment in relation to the duration of diabetes.\nStudy design\nCross-sectional.\nMaterial and methods\nT2DM patients (Group I: ⩽5 years duration of diabetes, n = 11; Group II: >5 years duration of diabetes, n = 17) without clinical evidence of central nervous system damage and non-diabetic controls (n = 18) were studied clinically and P300 event-related potentials (ERPs) recorded using three stimuli oddball paradigm. Subjects were examined with Folstein mini-mental state examination (MMSE) for cognitive function and those showing scores more than 26 (maximum score = 30) were enrolled for the study. Patients with known diabetic complications were excluded.\nResults\nP300 latencies in diabetic group did not relate linearly to the duration of diabetes. Diabetic subgroups with ⩽5 years and >5 years duration of diabetes showed striking differences, patients with over 5 years of disease duration had much prolonged P300 latencies in contrast to patients with 5 years or less disease duration who showed trends similar to that of control group. Differences in P300 amplitudes between groups were non-significant. Hypertensive diabetics showed much prolongation in P300 latencies compared to normotensive diabetics.\nConclusions\nP300 ERPs revealed cognitive dysfunction which was not detected by neuro-psychometric test (MMSE). Patients with T2DM have decreased cognitive function which is more prominent when the disease duration exceeds 5 years. Astwood's laboratory, when my directional sails were set and obesity was the compass heading. The next section of this memoir traces the wanderings of an itinerant academic who moved from Boston to Los Angeles and finally to Baton Rouge. The influence of Sir William Osler's idea that there is a time for education, a time for scholarship, a time for teaching, and time to retire has always been a guide to allocating time ever since I was an intern at Johns Hopkins Hospital. It was in Baton Rouge that the final phase began: I agreed to become the first full-time executive director of the Pennington Biomedical Research Center, a decision that changed my life. The potential for GM foods to improve the efficiency of food production, increase customer satisfaction, and provide potential health benefits has contributed to the rapid incorporation of GM foods into the American diet. However, GM foods and GMOs are also a topic of ethical debate. The use of GM foods and GM technology is surrounded by ethical concerns and situational judgment, and should ideally adhere to the ethical standards placed upon food and nutrition professionals, such as: beneficence, nonmaleficence, justice and autonomy. The future of GM foods involves many aspects and trends, including enhanced nutritional value in foods, strict labeling laws, and potential beneficial economic conditions in developing nations. This paper briefly reviews the origin and background of GM foods, while delving thoroughly into 3 areas: (1) GMO labeling, (2) ethical concerns, and (3) health and industry applications. This paper also examines the relationship between the various applications of GM foods and their corresponding ethical issues. Ethical concerns were evaluated in the context of the code of ethics developed by the Academy of Nutrition and Dietetics (AND) that govern the work of food and nutrition professionals. Some authors have adopted a non-surgical closure method in chosen cases and, since 1993, endoclips have been used to close perforation in the stomach. The endoscopic practice of endoclips has been commonly used in the gut for hemostasis. Currently, the use of endoscopic techniques is increasing for the closure of endoscopic submucosal dissection or endoscopic mucosal resection. Endoscopic perforations that improved with endoscopic closure in the literature prior to 2008 have been previously described. In the present article, we present a descriptive review of cases with iatrogenic perforation in the gut treated with endoclips between 2008 and 2014.\n\nMethods\n\nComprehensive literature screening and a systematic review using PubMed and Medline was done for all reports published between January 2008 and December 2014 using the endoclip technique in the closure of iatrogenic perforations.\n\nResults\n\nA total of 47 studies published between 2008 and 2014 using endoclips for the closure of iatrogenic perforations of the gut (nine esophagus, 11 stomach, 15 duodenum, 12 colon and rectum) were found. All studies were explained briefly and summarized in a table.\n\nConclusions\n\nThere is strong evidence to show the efficacy of endoclips in the management of iatrogenic perforations, especially when recognized early.
