There has been only limited study of how individuals with schizophrenia experience mental illness stigma. This study evaluated 74 stable outpatients with schizophrenia (aged 18-65 yrs) receiving community care. Study participants were interviewed with the Consumer Experiences of Stigma Questionnaire (CESQ), the Positive and Negative Syndrome Scale, and several social functioning measures. On the CESQ, all but 1 respondent indicated having at least 1 stigma experience. The most frequently reported CESQ items were respondents' worry about being viewed unfavorably because of their psychiatric illness (70%) and avoidance of telling others about it (58%). Many respondents also indicated having heard offensive statements (55%) and media accounts (43%) about persons with psychiatric disorders. Socioeconomic variables, but not symptoms or social functioning measures, were related to the extent of stigma and discrimination experiences. These results document the extent to which persons with mental illness experience negative reactions from others. Strategies are needed to enhance how persons with schizophrenia cope with stigma. Method: 210 patients with schizophrenia or major depression were questioned about anticipated and concrete stigmatization experiences, using a questionnaire especially developed for this study. Results: Most of the patients expected negative reactions from the environment. Three quarters were convinced that their job application would be rejected when it became known that they are mentally ill. Almost two thirds felt apprehensive that others would avoid them due to their illness. There is hardly any difference between schizophrenia and depressive patients' assessment of stigmatization of mentally ill people. Concrete stigmatization experiences were most frequently reported in the domain of interpersonal interaction. Second comes the distorted picture of mentally ill people that is depicted in the media and experienced as hurtful by the patients. The obstacles to access social roles (partnership, work, etc.) perceived by the patients come third. Participants least frequently mentioned structural discrimination, i.e. disadvantages regarding psychiatric treatment or rehabilitation measures. Contrary to anticipated stigmatization, there are differences between the two diagnostic groups when it comes to concrete stigmatization experiences. Schizophrenia patients more frequently report that others would avoid contact with them and that the access to social roles was especially complicated for them. They also seemed to be more exposed to structural discrimination than depressive patients. Background
Schizophrenia is regarded as one of the most stigmatized mental illnesses. Relatively few studies have investigated actual stigma experiences among people with schizophrenia and the factors which may contribute to it.Aims
This cross-sectional study assesses the extent of stigma experienced by patients with schizophrenia and attempts to establish its clinical and sociodemographic predictors.Methods
A total of 153 subjects with schizophrenia (62 outpatients and 91 inpatients) were evaluated with the use of the Stigma section of the Consumer Experiences of Stigma Questionnaire (CESQ) and several instruments measuring their subjective quality of life, social functioning and severity of psychiatric symptoms.Results
Stigmatization experiences were common among respondents who most frequently reported having concealed their illness (86%), witnessed others saying offensive things about the mentally ill (69%), worried about being viewed unfavorably (63%) and been treated as less competent (59%). Higher levels of stigma were related to lower subjective quality of life and younger age of illness onset. No significant associations were found between stigma and symptoms or level of social functioning.Conclusion
Our findings point at the reduced life satisfaction as a key aspect of the subjective experience of the stigma of schizophrenia.

It is known to cause loss of self-esteem which considered as the most tragic consequences. The aim of the study was to explore the association between perceived stigma with the level of self-esteem, clinical and socio-demb- graphic variables among patients with schizophrenia. Design: A cross-sectional study. Material and methods: The study sample was stable patients with schizophrenia attending regular psychiatric clinic follow-up. They were selected through convenience sampling. The perceived stigma and self- esteem were assessed with the Devaluation and Discrimination Scale (DDS) and Shamsunnisah Self-Estcem Scale (SSE) respectively. Results: The final sample consisted of 227 patients with mean duration of illness of 11.6 (+ 8.1) years. The mean stigma score was 2.38 (+ 0.39). Majority of the patients (58.1%) scored lower than the mid point on the DDS, which reflected low level of perceived stigma; 131 (57.7%) had moderate level of self-esteem and the percentage of low self-esteem was the lowest (13.2%). There was significant difference in mean stigma between the three levels of self-esteem (p < 0.001); high, moderate and low. However, there was no significant association between clinical and socio-demographic variables with the level of perceived stigma. Conclusions: The perceived stigma among stable patients with schizophrenia was low. There was significant association between perceived stigma with the level of self-esteem.
