            "abstractNote": "The Quality of Well-being Scale (QWB) is a general measure of health-related quality of life that has been used in a wide variety of population and clinical studies. One of the major disadvantages of the QWB is that it requires a trained interviewer. Recently, a self-administered version of the QWB was developed. The purpose of this study was to compare the self-administered QWB with the established interviewer-administered form. The respondents were 218 English speaking adults who attended primary care clinics. Each respondent was evaluated twice with an interval of one month in-between. At each session respondents were randomly assigned to complete either the interviewer-administered or self-administered QWB, resulting in a 2 x 2 factorial design. Data from the study demonstrated that the self-administered QWB yields scores equivalent to the interviewer- administered form. Further, QWB scores remain stable over the course of a one month interval. The results sug- gest that an inexpensive self-administered QWB may produce data comparable to the more difficult and expensive interviewer-administered version. Disadvantages of self-administered forms are also discussed.",

            "abstractNote": "This is a review of the Health Utilities Index (HUI®) multi-attribute health-status classification systems, and single- and multi-attribute utility scoring systems. HUI refers to both HUI Mark 2 (HUI2) and HUI Mark 3 (HUI3) instruments. The classification systems provide compact but comprehensive frameworks within which to describe health status. The multi-attribute utility functions provide all the information required to calculate single-summary scores of health-related quality of life (HRQL) for each health state defined by the classification systems. The use of HUI in clinical studies for a wide variety of conditions in a large number of countries is illustrated. HUI provides comprehensive, reliable, responsive and valid measures of health status and HRQL for subjects in clinical studies. Utility scores of overall HRQL for patients are also used in cost-utility and cost-effectiveness analyses. Population norm data are available from numerous large general population surveys. The widespread use of HUI facilitates the interpretation of results and permits comparisons of disease and treatment outcomes, and comparisons of long-term sequelae at the local, national and international levels.",

            "abstractNote": "Described the development of the Spanish Quality of Life Questionnaire, the CCV, a new 39-item measure of health-related perceived quality of life (QOL). 245 Spaniards (aged 18–55 yrs) rated 96 questions according to the level of influence in their conception of QOL. Items that were rated as \"quite\" or \"very\" influential by more than 80% of the Ss constituted the final 39-item questionnaire. Two additional samples, including a total of 2,810 adults (aged 18–65 yrs) then analyzed the psychometric properties of the instrument. Four factors were found: (1) social support, (2) general satisfaction, (3) physical/psychological well-being, and (4) absence of work over-load/free time. Results indicate that the CCV is a measure of perceived QOL that presents a satisfactory validity and reliability, as well as a reasonable degree of sensitivity to change.             "abstractNote": "A 36-item short-form (SF-36) was constructed to survey health status in the Medical Outcomes Study. The SF-36 was designed for use in clinical practice and research, health policy evaluations, and general population surveys. The SF-36 includes one multi-item scale that assesses eight health concepts: 1) limitations in physical activities because of health problems; 2) limitations in social activities because of physical or emotional problems; 3) limitations in usual role activities because of physical health problems; 4) bodily pain; 5) general mental health (psychological distress and well-being); 6) limitations in usual role activities because of emotional problems; 7) vitality (energy and fatigue); and 8) general health perceptions. The survey was constructed for self-administration by persons 14 years of age and older, and for administration by a trained interviewer in person or by telephone. The history of the development of the SF-36, the origin of specific items, and the logic underlying their selection are summarized. The content and features of the SF-36 are compared with the 20-item Medical Outcomes Study short-form.",

            "abstractNote": "The final development of the Sickness Impact Profile (SIP), a behaviorally based measure of health status, is presented. A large field trial on a random sample of prepaid group practice enrollees and smaller trials on samples of patients with hyperthyroidism, rheumatoid arthritis and hip replacements were undertaken to assess reliability and validity of the SIP and provide data for category and item analyses. Test-retest reliability (r = 0.92) and internal consistency (r - 0.94) were high. Convergent and discriminant validity was evaluated using the multitrait--multimethod technique. Clinical validity was assessed by determining the relationship between clinical measures of disease and the SIP scores. The relationship between the SIP and criterion measures were moderate to high and in the direction hypothesized. A technique for describing and assessing similarities and differences among groups was developed using profile and pattern analysis. The final SIP contains 136 items in 12 categories. Overall, category, and dimension scores may be calculated.",

            "abstractNote": "This paper is a preliminary report on the development of a new instrument, the Impact of Weight on Quality of Life (IWQOL) questionnaire, that assesses the effects of weight on various areas of life. We conducted two studies utilizing subjects in treatment for obesity at Duke University Diet and Fitness Center. The first study describes item development, assesses reliability, and compares pre- and post-treatment scores on the IWQOL. In the second study we examined the effects of body mass index (BMI), gender, and age on subjects' perceptions of impact of weight on quality of life. Results indicate adequate psychometric properties with test-retest reliabilities averaging .75 for single items, and .89 for scales. Scale internal consistency averaged .87. Post-treatment scores differed significantly from pre-treatment scores on all scales, indicating that treatment produced positive changes in impact of weight on quality of life. The results of the second study indicate that the impact of weight generally worsened as the patients' size increased. However for women there was no association between BMI and impact of weight on Self-Esteem and Sexual Life. Even at the lowest BMI tertile studied, women reported that weight had a substantial impact in these areas. There were also significant gender differences, with women showing greater impact of weight on Self-Esteem and Sexual Life compared with men. The impact of age was a bit surprising, with some areas showing positive changes and others showing no change.",

            "abstractNote": "Background: The Moorehead-Ardelt Quality of Life Questionnaire was originally developed as a disease-specific instrument to measure postoperative outcomes of self-perceived quality of life (QoL) in obese patients. 5 key areas were examined: self-esteem, physical well-being, social relationships, work, and sexuality. Each of these questions offered 5 possible answers, which were given + or - points according to a scoring key. The questionnaire was used independently or incorporated into the Bariatric Analysis and Reporting System (BAROS). The instrument is simple, unbiased, user-friendly and can be completed in <1 minute. It has been found useful, reliable and reproducible in numerous clinical trials in different countries. Further research and feedback from some of its users prompted refinements, now included in the Moorehead-Ardelt Quality of Life Questionnaire II (M-A QoLQII). This study tested the validity of the improved instrument. Methods: The wording of the questions was changed, to make them less suggestive and allow for the use of the survey before and after medical intervention, and with control groups. A 6th question, analyzing eating behavior, was added. The ±1 point given to the evaluation of self-esteem was split with this new question, thus maintaining the consistency of the scores. The drawings were simplified. Finally, the scoring key was changed to a 10-point Likert scale, to improve response-differentiation. To validate the M-A QoLQII, we examined its concordance with other health and well-being indicators, specifically the MOS 36-Item Short-Form Health Survey (SF-36), the Beck Depression Inventory-II (BDI-II) and the Stunkard and Messick Eating Inventory. The study population included 110 morbidly obese patients (20 males, 90 females, mean BMI=50), participants of gastric bypass support groups. Reliability of the M-A QoLQII was determined using Cronbach's alpha coefficient. Construct validity was measured by conducting a series of Spearman rank correlations. Results: A Cronbach's alpha coefficient of 0.84 indicated satisfactory internal consistency. The M-A QoLQII was found to be significantly correlated (P <0.01) to 7 of the 8 SF-36 scales: Physical Role (r=0.357), Bodily Pain (r=-0.486), General Health (r=0.413), Vitality (r=0.588), Social Functioning (r=0.517), Emotional Role (r=0.480), and Mental Health (r=0.489). The questionnaire also significantly correlated (P <0.01) to the Beck Depression Inventory-II (r=0.317), as well as to the 'Disinhibition' (r=-0.307) and 'Hunger' (r=-0.254) factors of the Stunkard and Messick Eating Inventory. Conclusions: The M-A QoLQII correlates well with other widely used health and well-being indicators such as the SF-36, Beck Depression Inventory II and the Stunkard and Messick Eating Inventory. The study established the validity and reliability of this improved disease-specific instrument for QoL measurement in the obese population.",

            "abstractNote": "BACKGROUND: Quality of life (QoL) is considered to be the true measure for the effectiveness of a surgical procedure, but there are only a few validated instruments available for bariatric surgery. Therefore, a new diseasespecific 30-item instrument was created, which was called Bariatric Quality of Life (BQL) questionnaire.\nMETHODS: To validate the BQL, we studied 133 patients after 4 different types of bariatric surgery. Initially, mean body mass index (BMI) was 47.2 +/-7.6 kg/m2 and mean age was 38.8 +/-11.0 years. At baseline, and 1, 6, and 12 months after surgery, patients filled in the BQL, the SF-12 (Short Form of SF-36 Health Survey), the GIQLI (Gastrointestinal Quality of Life Index), and the BAROS (Bariatric Analysis and Reporting Outcome System).\nRESULTS: Internal consistency of the BQL was found to be good, with Cronbach's alpha ranging between 0.71 and 0.86. Factor analyses suggested that the BQL included a highly consistent set of QoL items and a second part on co-morbidities and gastrointestinal symptoms. At the 12 months follow-up, the BQL was closely correlated to SF 12 (Pearson's r = 0.86), GIQLI (0.68), BAROS (0.71), and excess weight loss (0.55). Standardized effect sizes over time were larger for the BQL (1.39 and 1.58) than for the other instruments.\nCONCLUSIONS: The BQL questionnaire is a validated instrument ready for clinical use.",

            "abstractNote": "The short form of impact of weight on quality of life (IWQOL)-Lite is a 31-item, self-report, obesity-specific measure of health-related quality of life (HRQOL) that consists of a total score and scores on each of five scales--physical function, self-esteem, sexual life, public distress, and work--and that exhibits strong psychometric properties. This study was undertaken in order to assess test-retest reliability and discriminant validity in a heterogeneous sample of individuals not in treatment. Individuals were recruited from the community to complete questionnaires that included the IWQOL-Lite, SF-36, Rosenberg self-esteem (RSE) scale, Marlowe-Crowne social desirability scale, global ratings of quality of life, and sexual functioning and public distress ratings. Persons currently enrolled in weight loss programs or with a body mass index (BMI) of less than 18.5 were dropped from the analyses, leaving 341 females and 153 males for analysis, with an average BMI of 27.4. For test-retest reliability, 112 participants completed the IWQOL-Lite again. ANOVA revealed significant main effects for BMI for all IWQOL-Lite scales and total score. Females showed greater impairment than males on all scales except public distress. Internal consistency ranged from 0.816 to 0.944 for IWQOL-Lite scales and was 0.958 for total score. Test-retest reliability ranged from 0.814 to 0.877 for scales and was 0.937 for total score. Internal consistency and test-retest results for overweight/obese subjects were similar to those obtained for the total sample. There was strong evidence for convergent and discriminant validity of the IWQOL-Lite in overweight/obese subjects. As in previous studies conducted on treatment-seeking obese persons, the IWQOL-Lite appears to be a reliable and valid measure of obesity-specific quality of life in overweight/obese persons not seeking treatment.",

            "abstractNote": "BACKGROUND: Being overweight or obese has substantial effects on individuals' perceptions of their health and quality of life (QoL). Generic measures often miss important QoL information, and existing obesity-specific instruments have shortcomings with respect to cross-cultural input, application to differing levels of severity, conceptual clarity, and patient burden.\nOBJECTIVE: This study aimed to develop culturally sensitive measures of QoL and symptoms in overweight or obese persons, both those trying to lose weight and those not trying to lose weight.\nMETHODS: Currently accepted criteria and guidelines for questionnaire development were streamlined and augmented to include cross-cultural input from 5 European countries and the United States. The preliminary pool of items was created based on qualitative interviews conducted in the United States, followed by an early check of item translatability and preharmonization across all languages. Ten additional qualitative in-country interviews were then conducted to produce further culture-specific items. This was followed by 2 forward and 1 backward translation, plus cognitive debriefing interviews in each country. Finally, an expanded international harmonization meeting was held to ensure inclusion of appropriate new items and their acceptability across all 6 cultures.\nRESULTS: The procedures described resulted in development of the 41-item Obesity and Weight Loss Quality of Life questionnaire and the 20-item Weight-Related Symptom Measure. These questionnaires are subject to further psychometric validation.\nCONCLUSIONS: Augmentation of the initial item-generation and international harmonization steps of questionnaire development through integration of cross-cultural input allowed greater validity of cross-cultural content while meeting time, budget, and resource constraints.",

            "abstractNote": "OBJECTIVE: To develop a reliable and valid measure of distress, related to extreme obesity.\nDESIGN: Items related to distress over obesity were selected from the literature, clinical experience and from input provided by a gastroplasty patient support group. The items were assessed in a longitudinal study, with the body mass index (BMI) and psychological assessment occurring 2-6 months prior to, and 12 months following, gastroplasty surgery.\nSUBJECTS: 81 females and eight males (mean age 35.9 y) who had been accepted for gastroplasty surgery. All but two of the patients had BMIs > 40 (Mean = 48.11, s.d. = 6.84).\nMEASUREMENTS: BMIs were calculated using weight and height. Psychological characteristics were assessed using the Mental Health Inventory (MHI), the Sickness Impact Profile (SIP), and the Eating Inventory (EI). Demographic information was collected with a questionnaire.\nRESULTS: Attempts to factor analyse the 95 item questionnaire were unsuccessful. Alternatively, a shorter, 20 item questionnaire was developed. The questionnaire shows good test-retest reliability (r = 0.867), good internal consistency (coefficient alpha = 0.719), good face and construct validity, and is sensitive to pre-post surgical change.\nCONCLUSIONS: The Obesity Adjustment Survey (OAS) may be useful as a brief measure of distress in obese individuals. This measure can be used to index the psychological impact of gastroplasty surgery on psychological functioning, and can be used in future research as a disease-specific measure to predict success of surgery.",

            "abstractNote": "The development and validation of a self-reported measure of obesity-related quality of life, the Obesity Related Well-Being (ORWELL 97), were undertaken to examine the intensity and the subjective relevance of physical and psychosocial distress. The questionnaire was validated in a sample of 147 obese patients (99 females, 48 males). The Eating Disorder Examination 12.0D interview, a structured diagnostic interview for DSM-III-R (DSM-IV criteria for binge eating disorder), Beck Depression Inventory, Binge Eating Scale, and the State-Trait Anxiety Inventory 1 and 2 scales were also applied. Internal consistency and test-retest reliability were satisfactory. Factor analysis allowed the identification of two subscales: ORWELL 97-1 related to psychological status and social adjustment, and ORWELL 97-2 related to physical symptoms impairment. Obese female patients showed a lower quality of life, and the severity of obesity appeared to interfere with physical functioning rather than psychological status and social adjustment. The ORWELL 97 questionnaire appears to be a simple and reliable measure of obesity-related quality of life, which can be used in current clinical practice.",

            "abstractNote": "STUDY OBJECTIVE: To determine the aspects of quality of life that are affected by weight in the general population, to develop a specific questionnaire (OSQOL) that can measure with reliability and validity the impact of overweight and obesity on well being, and to compare the results with those obtained using a well known generic tool (the \"SF 36\" scale).\nDESIGN: Cross sectional survey with matched control group.\nSETTING: Community.\nPATIENTS: A permanent survey base of approximately 10,000 representative ordinary households were screened for weight and height. Five hundred subjects were randomly drawn from the sub-sample of adult people with a body mass index equal or greater than 27. A control sample of 500 subjects matched for sex, age, and employment status was drawn from the non-obese population.\nMAIN RESULTS: A short specific Quality Of Life scale, the OSQOL, was produced that comprises 11 items belonging to four independent dimensions. Using this scale and the SF36, it was found that: (1) moderately obese subjects (27 < or = BMI < 30) did not significantly differ from the control group except for physical capacity; (2) in the group of obese subjects with a BMI > 30, quality of life seemed to be impaired for five of nine dimensions of the SF36 compared with the control population, all related to physical consequences of obesity. This population essentially perceived itself in terms of poor general health. (3) No significant difference was observed between the samples for the psychological and social dimensions of the SF36.\nCONCLUSION: The quality of life of patients with severe obesity is impaired, but it mainly affects the physical consequences of the disease. The psychological and social repercussions that could have been expected to affect this population were not demonstrated. The hypothesis of a process of adaptation of the person and their social environment cannot therefore be excluded. Methodologically, comparison of the specific OSQOL with the generic SF3 shows clearly that the two kinds of scales correspond to different objectives and should be considered as complementary.",

            "abstractNote": "The objective of this study was to assess the reliability, validity and responsiveness of a new health-related quality-of-life (HRQOL) measure containing global and obesity-specific domains and an obesity-specific health state preference (HSP) assessment. A total of 417 obese and 'normal' weight individuals completed these assessments. Internal consistency and test-retest reliability were demonstrated, with Cronbach's alpha, intraclass correlation coefficient and kappa values well above the acceptable level for most scales. Construct validity hypotheses were confirmed by examining scale correlations. The normal weight individuals reported statistically significantly better functioning and well-being on the majority of the HRQOL scales and HSP than obese individuals. Guyatt's statistic of responsiveness was moderate to high for all the scales and items in the weight-loss and weight-gain groups; however, many of the scales and items in the weight-stable group also displayed responsiveness. The results of this study support the reliability and validity of these assessments. However, further testing is needed to evaluate the responsiveness of both assessments in a weight-stable group.",
