            "abstractNote": "Background We aimed to report the outcomes associated with leech therapy and to identify risk factors associated with reconstructive failure. Methods We retrospectively reviewed cases of flap reconstruction or replanted appendages that required leech therapy over an 8-year period at the Duke University Medical Center. Using logistic regression, we assessed the association of risk factors with reconstructive failure. Results The study cohort included 87 patients which correspond to 2.1% of 4,115 cases done during the study period. The most common flap recipient site was the lower extremity (n = 33, 37.9%) followed by the upper extremity (n = 30, 34.5%), head and neck (n = 13, 14.9%), and trunk (n = 11, 12.6%). Flap types were pedicled in 44 (50.5%) cases and free in 24 (27.5%) cases. Fifteen (17.2%) were digital replantation, and four (4.5%) were replanted appendages. The average duration of therapy was 4.6 days (range: 1-11). The overall leech therapy success rate was 60.9% (53/87) and accounted for cases without flap loss (n = 45, 51.7%) and with partial flap loss in which the original reconstructive goal was achieved without further reconstructive procedures (n = 8, 9.2%). Postoperative blood transfusion was administered in 32 (36.7%) cases, and infectious complications occurred in 7 cases in spite of the administration of prophylactic antibiotics (8%). Conclusions This study represents the largest single-institution series evaluating the outcomes after leech therapy. Our data support the use of leeches as an adjunct for the management of venous congestion after reconstructive surgery.             "abstractNote": "Leech therapy or hirudotherapy is a well-recognized option for treatment of venous congestion in plastic and reconstructive surgery. It is clearly documented in our ancient Sanskrit text like 'Sushruta Samhita'. Recently, hirudotherapy has become an important modality in salvaging venous congested flaps. This prospective study of 1-year period was conducted at L.N. Medical College and J.K. Hospital, Bhopal, Central India. Leech therapy used in a total of nine patients in this study. Out of nine patients, five were males and four were females, with a mean age of 44 years (range 20 and 74 years). Free flaps were employed in two patients and pedicles flap was used in seven. Venous congestion in most of the patient appeared before 24 h. A number of leeches used ranged from 121 to 180 leeches per patient, with a mean of 150.8. The number of days of leeching was from 4 to 8 days, with a mean of 5.8 days. Blood transfusion required was from 1 to 5 units with a mean of 2.5 units. No local or systemic complications were observed in our study. The overall flap survival rate was 77.7% (7/9) with flap loss in two patients. Venous congestion is the most common cause of flap failure. Application of leech therapy is a helpful tool in salvaging the tissue transfer, not otherwise salvageable by surgical means.             "abstractNote": "Pain and associated neuropathic conditions (e.g. hyperalgesia, allodynia) are highly prevalent, yet the basic mechanisms underlying how nociceptive pathways are modified are poorly understood. The goal of this dissertation is to contribute to the foundational understanding of nociception by exploring the physiological and behavioral role of both the Transient Receptor Vanilloid (TRP) channels and the endovanilloid (eVN) signaling molecules that are known to contribute to the detection and modulation of pain signals, using the model system of the leech. Physiological and behavioral experiments confirmed the presence of both a peripheral and central TRPV-like channel within the polymodal nociceptive neurons that was characterized by sensitivity to capsaicin and noxious heat. Capsaicin was found to elicit a nocifensive response in a concentration-dependent manner; specifically, reflexive withdrawal of the posterior sucker and locomotion away from the noxious stimuli. Further experiments also revealed, for the first time in any annelid, the presence of a TRPA-like channel that was characterized by sensitivity to mustard oil and moderate cold. A TRPA antagonist blocked the responses to both mustard oil and cold, while sensitizing the responses to heat, indicating a potential interaction between the TRPV and TRPA channels within the polymodal nociceptive neurons. Mustard oil was also found to elicit similar nocifensive responses in a concentration dependent manner. Experiments exploring the role of eVNs found behavioral evidence of opposing effects in which eVNs induced attenuation of nociceptive responses and a sensitization of non-nociceptive responses in uninjured animals that was TRPV-dependent. Subsequent experiments induced an injury to the leech's posterior sucker that resulted in a persistent sensitization to non-nociceptive stimuli (allodynia) and nociceptive stimuli (hyperalgesia). When eVN treatment was given to the injured animals three days after the induction of injury, there was an attenuation of the injury-induced hyperalgesia, but no effect was observed on the allodynia. Taken together, these results demonstrate the conservation of the pain pathway and the therapeutic potential and pitfalls of eVN treatment after injury.             "abstractNote": "The behaviors generated by neural circuits are constrained by the connectivity pattern among the neurons involved. Determining this connectivity pattern for circuits involving more than a handful of neurons becomes infeasible for physiological approaches that measure how the membrane potential of one neuron is affected by currents elicited in another. On the other hand, determining connectivity by anatomically discovering synapses is difficult due to the complicated and intertwining arbors that neurons possess. One approach to unravel this knotty problem is to image serial thin sections of neural tissue with an electron microscope. In this thesis, I describe the first application of large scale serial electron microscopy to the ganglion of the medicinal leech, Hirudo verbana. The leech ganglion is an ideal preparation for this experiment as it is compact enough in size that it is feasible to collect images spanning an entire ganglion. Here, I discuss results we collected from two image volumes of leech tissue. One spans a small region of adult ganglion neuropil, while the other includes an entire ganglion belonging to a juvenile leech. In these volumes, I reconstruct three-dimensional representations of neuronal arbors and locate the synapses between them. In Chapter 2, I show that we can differentiate neurons on the basis of where synaptic input and output sites are distributed throughout their arbors. In Chapter 3, I show that we can locate the synapses between pairs of neurons previously known to be connected, and that we can discover new synapses anatomically that are then recovered physiologically. Together, these results demonstrate the potential that this 'connectomics' approach has when applied to the already physiologically accessible leech ganglion.             "abstractNote": "Purpose: Two studies published recently in this journal sought to expand on previous attempts to explain and predict young children’s recovery from stuttering. This letter addresses the influence of treatment on such research. Conclusions: Recent publications from Kefalianos et al. (2017) and Leech, Bernstein Ratner, Brown, and Weber (2017) added to previous information that gender and language ability (or language growth) may be related to children’s recovery from stuttering. The conclusions from both studies are difficult to interpret, however, because neither incorporated two factors known to influence children’s recovery: a family history of recovery and, especially, the type and timing of treatment. Consideration of these two articles therefore raises multiple empirical, theoretical, and clinical issues that deserve to be fully addressed if our discipline is seeking to understand and to maximize recovery for young children who stutter.             "abstractNote": "How did parental care behaviors evolve? Is parental care in invertebrates distinct from parental care in vertebrates? These two questions provide the framework for my dissertation. Parental care is a complex set of behaviors that increases the fitness of a parent and its offspring. Parental behavior is found throughout the animal kingdom and appears to be an ancient behavior evolving over millions of years. Even with a long evolutionary history of parental care behaviors in invertebrates, research is lacking in this largest group of organisms. My dissertation will examine the evolutionary history of parental care in leeches (Annelida: Hirudinida) including a review of the literature to update the patterns of parental care in the family Glossiphoniidae and explore the evolution of these behaviors. In the process of studying the life history and parental care behaviors of leeches, I noticed discrepancies between the published descriptions of morphology, life-history, and behaviors for the populations I was supposed to encounter in central and northern Arizona, and the individuals that comprised my research populations. To resolve this issue, I performed morphological and molecular analyses of the COI region of these questionable individuals from Montezuma Well, resulting in new descriptions of the two Helobdella spp (Helobdella blinni and Helobdella rexvanderfordi). On leeches confirmed to represent Helobdella modesta, I also performed a laboratory reciprocal transplant experiment among three Arizona populations to investigate how their aquatic environment might lead to local adaptions in life-history traits including adult survival, breeding, and fecundity.             "abstractNote": "In the present manuscript we analyze the behavior and interactions among leeches in the same observation tank. Colored beads were glued onto their skin, so that their behavior could be followed and quantified. When 2 or 3 leeches were present in the observation tank, they searched around for a maximum of two hours and their motion and behavior were independent from those of their conspecifics. When the number of leeches in the tank was increased to 10, leeches were attracted to each other and exhibited episodes of highly correlated behavior. Solitary leeches injected with serotonin or dopamine increased the portion of time spent pseudo-swimming and crawling, respectively. The behavior of 3-5 leeches injected with serotonin was not statistically independent and leeches were attracted to their conspecifics and exhibited episodes of correlated behavior. Therefore, serotonin not only induces pseudo-swimming in leeches but also promotes social interactions, characterized by a mutual attraction and by episodes of correlated /collective behavior.             "abstractNote": "Using double immunofluorescence experiments, we described the expression of the leech Hox genes, Lox1 and Lox2 by central neurons that stained for either serotonin or the leech-specific neuronal marker, Laz1-1. The goal is to determine whether the segmental boundaries of Lox1 and Lox2 expression in identified neurons coincide with segmental and regional differences in the differentiation of these cells. A number of neurons described here have been previously identified. The anteromedial serotonergic neurons are restricted to rostral ganglion 1 (R1) to midbody ganglion 3 (M3), but only express Lox1 in M2 and M3. The posteromedial serotonergic neurons which are situated in all segments as bilateral pairs early in development, but later become unpaired starting at M3, expressed Lox1 only in M2 and M3, and Lox2 in M8 to M21, in all paired and unpaired stages. The Retzius neurons, which stain for serotonin, express Lox2 in M7 to M21 where they exhibit different morphologies from their segmental homologs of the sex ganglia in M5 and M6. The Laz1-1 immunoreactive (Laz1-1+) heart accessory-like neurons express Lox1 in M4 and Lox2 in M7 to M17, but not in their segmental homologs of the heart accessory (HA) neurons located exclusively in M5 and M6. Also, Laz1-1+ neurons, which we named Lz3 expressed Lox1 in M4 to M8 where they are unpaired, but express Lox2 in M9 to M16 where they are bilaterally paired. Other Laz1-1 cells show more restricted and isolated Lox1 and Lox2 expression patterns.             "abstractNote": "Sensory coding has long been discussed in terms of a dichotomy between spike timing and rate coding. However, recent studies found that in primate mechanoperception and other sensory systems, spike rates and timing of cell populations complement each other. They simultaneously carry information about different stimulus properties in a multiplexed way. Here, we present evidence for multiplexed encoding of tactile skin stimulation in the tiny population of leech mechanoreceptors, consisting of only 10 cells of two types with overlapping receptive fields. Each mechanoreceptor neuron of the leech varies spike count and response latency to both touch intensity and location, leading to ambiguous responses to different stimuli. Nevertheless, three different stimulus estimation techniques consistently reveal that the neuronal population allows reliable decoding of both stimulus properties. For the two mechanoreceptor types, the transient responses of T (touch) cells and the sustained responses of P (pressure) cells, the relative timing of the first spikes of two mechanoreceptors encodes stimulus location, whereas summed spike counts represent touch intensity. Differences between the cell types become evident in responses to combined stimulus properties. The best estimation performance for stimulus location is obtained from the relative first spike timing of the faster and temporally more precise T cells. Simultaneously, the sustained responses of P cells indicate touch intensity by summed spike counts and stimulus duration by the duration of spike responses. The striking similarities of these results with previous findings on primate mechanosensory afferents suggest multiplexed population coding as a general principle of somatosensation.             "abstractNote": "Although habituation is ubiquitous in the animal kingdom, its underlying mechanisms remain poorly understood. In this study, we began to explore the molecular cascades underlying short-term habituation in the leech Hirudomedicinalis. In H. medicinalis, a training paradigm, consisting of low-frequency repetitive electrical stimulation of the skin, produces a gradual increase in the latency to swim that spontaneously recovers within 20–30 min. As first step in determining the molecular pathways in short-term habituation, we examined the role of Ca2+. Both Ca2+ influx through voltage-gated channels and Ca2+ release from intracellular stores were found to contribute to short-term habituation. The analysis of the downstream targets of elevated cytosolic Ca2+ revealed that the activation of the phosholipase A2 was required for the induction of short-term habituation. Finally, we reported that the recruitment of arachidonic acid metabolites, generated by the 5-lipoxygenase pathway, was also necessary for the induction of swim induction habituation. These results provide the framework for a comprehensive characterization of the molecular underpinnings of habituation. This outcome will allow us to compare the mechanisms of habituation with those underlying other forms of nonassociative learning in the leech, such as sensitization and dishabituation, and, more in general, with those governing habituation in different vertebrate and invertebrate model systems.             "abstractNote": "The Ionized calcium‐Binding Adapter molecule 1 (Iba1), also known as Allograft Inflammatory Factor 1 (AIF‐1), is a 17 kDa cytokine‐inducible protein, produced by activated macrophages during chronic transplant rejection and inflammatory reactions in Vertebrates. In mammalian central nervous system (CNS), Iba1 is a sensitive marker associated with activated macrophages/microglia and is upregulated following neuronal death or brain lesions. The medicinal leech Hirudo medicinalis is able to regenerate its CNS after injury, leading to a complete functional repair. Similar to Vertebrates, leech neuroinflammatory processes are linked to microglia activation and recruitment at the lesion site. We identified a gene, named Hmiba1, coding a 17.8 kDa protein showing high similarity with Vertebrate AIF‐1. The present work constitutes the first report on an Iba1 protein in the nervous system of an invertebrate. Immunochemistry and gene expression analyses showed that HmIba1, like its mammalian counterpart, is modulated in leech CNS by mechanical injury or chemical stimuli (ATP). We presently demonstrate that most of leech microglial cells migrating and accumulating at the lesion site specifically expressed the activation marker HmIba1. While the functional role of Iba1, whatever species, is still unclear in reactive microglia, this molecule appeared as a good selective marker of activated cells in leech and presents an interesting tool to investigate the functions of these cells during nerve repair events.             "abstractNote": "Reports an error in 'Mental health apps for adolescents and young adults: A systematic review of randomised controlled trials' by Teghan Leech, Diana Dorstyn, Amanda Taylor and Wenjing Li (Children and Youth Services Review, 2021[Aug], Vol 127[106073]). The authors regret that select data in Table 1 were misplaced (incorrect table row) for two studies. The corrected details are presented in an erratum. Objective: Smartphone applications (‘apps’) have the potential to improve the scalability of mental health interventions for young people, however, the effectiveness of stand-alone apps in mental health management remains unclear. This systematic review, with meta-analysis, provides an up-to-date summary of the available high-quality evidence. Methods: Eleven randomized controlled trials, involving a pooled sample of 1706 adolescents and young adults (age range 10 to 35 years), were identified from the Cochrane Library, Embase, Google Scholar, PsycINFO and PubMed databases. The reporting quality of studies was evaluated using the Cochrane Risk of Bias Tool 2.0 (RoB 2.0). Hedges’ g effect sizes with 95% confidence intervals, p values and heterogeneity statistics were additionally calculated using a random effects model. Results: Study reporting quality was sound, with no trials characterized as ‘high’ risk. App interventions produced significant symptom improvement across multiple outcomes, compared to wait-list or attention control conditions (depression gw = 0.52 [CI: 0.18–0.84], p = .01, k = 8; stress gw = 0.30 [CI: 0.06–0.53], p = .02, k = 2). Longer-term benefits could not be established (k = 4), although individual studies reported positive trends up to 6 months post. Age was not identified as a significant moderator. Conclusions: Smartphone apps hold promise as a stand-alone self-management tool in mental health service delivery. Further controlled trials with follow-up data are needed to confirm these findings as well as determine treatment engagement and effectiveness across diverse groups of participants.
