Common, mild disorders often remit in young adulthood, but more severe disorders can become chronic and need long-term medical and psychological management. Doctors are uniquely placed to opportunistically assess and manage alcohol use disorders, but in practice diagnosis and treatment are often delayed. Brief behavioural intervention is effective in primary care for hazardous drinkers and individuals with mild disorders. Brief interventions could also encourage early entry to treatment for people with more-severe illness who are underdiagnosed and undertreated. Sustained abstinence is the optimum outcome for severe disorder. The stigma that discourages treatment seeking needs to be reduced, and pragmatic approaches adopted for patients who initially reject abstinence as a goal. P.", "lastName": "Massicotte" } ], "abstractNote": "Health transition of youth from a child-centered care model to the adult model has been recognized to be of critical importance due to the increasing numbers of children now surviving chronic conditions. A formalized transition process is required adequately assess the AYA's readiness for transition and to move towards adult care. Indefinite warfarin therapy poses challenges as warfarin is a narrow therapeutic index drug that requires frequent monitoring and attentiveness to warfarin interactions and affects.\nObjective\nThe objective of this study was to evaluate transition to adult care for AYAs requiring indefinite warfarin therapy within a structured self-management program.\nOutcome measures\nResults were compared between Phase 1 (enrollment to patient self-management) and Phase 2 (independent warfarin management) 6 months following confirmation of transition to adult care. There was no statistical difference between outcome measures except INR testing frequency, and no adverse events.\nConclusions\nThis transition process resulted in successful transition as measured by TTR and other clinical end-points from pediatric to adult care. These young people had experienced traumatic events such as abuse, violence, addictions, mental health issues, and many had been excluded from school. The young people (aged between 13 and 17) were users of multiple services (statutory and nongovernmental services including: child welfare, juvenile justice, remedial education, and mental health services). Qualitative interviews (n = 109) explored young people's experiences in their families, communities, education, and their perspectives on support provided by services and their own support networks. Dans ce contexte, les\ngouvernements adoptent des programmes basés sur l’évidence (Evidence-based\nProgram) en espérant qu’une fois mis en œuvre, ils donneront les mêmes résultats que\nceux obtenus avec le modèle du programme. Par conséquent, les évaluations\nd’implantation de ces programmes ont pour objet de mesurer la fidélité d’implantation\ncomparativement au modèle initial du programme afin d’expliquer les résultats obtenus.\nCette perspective ignore l’importance du contexte et des particularités des populations\ncibles lors de l’implantation et considère toute déviance au modèle initial comme un\néchec de mise en œuvre ou plus encore du programme. Ceci étant dit, d’autres\nperspectives commencent à se tailler une place concurrente et parfois opposée de la\nperspective basée sur la fidélité. En évaluation de programme, trois grandes écoles\nmènent le débat autour de la question de la fidélité et de l’adaptation des programmes\npublics.\nLa discussion dans le séminaire sera alimentée par les quatre textes proposés pour la\nlecture. Les différents types, approches, etc. autant de la fidélité que de l’adaptation\nseront discutés.
