            "abstractNote": "OBJECTIVES: To understand public perceptions and opinions of three options for prescribing medicine: individualized genetic testing, race-based prescription, and traditional prescription.METHODS: Focus groups in urban, suburban, and rural communities over-sampled for minority groups conducted from February through April, 2001 in Georgia.RESULTS: Group members (N = 102) identified individualized genetic testing as providing the best quality of care (60% of talk turns; 75% in postdiscussion anonymous survey), but stipulated the need for protection from the invasion of privacy, discrimination, and prohibitive cost. Most individuals chose genetic testing because it provided individualized attention, and African-Americans indicated they would choose genetic testing even if the costs were high. Overall, individuals were suspicious of race-based prescription. Analyses for degree of suspicion revealed a main effect for race and an interaction effect for race and gender.CONCLUSIONS: If issues of cost, discrimination, and privacy are addressed, lay individuals prefer genetic testing as the basis for prescription of medicines that exhibit racially patterned response variation.",

            "abstractNote": "Uncertainty is an important part of the illness experience. Mishel elaborated a theory of uncertainty in acute illness and later expanded the framework to account for uncertainty in chronic illness. Researchers subsequently have investigated the causes and outcomes associated with the uncertainty in illness experience across a variety of medical conditions. The current study applies and extends Mishel's model within the context of HIV illness-related uncertainty. In this qualitative study, focus group methods were used to examine the nature of illness uncertainty experienced by persons living with HIV or AIDS. Findings confirm Mishel's contention that the causes of uncertainty extend beyond those of medical diagnosis, treatment, and recovery to personal and social aspects of daily life. Identified sources of uncertainty may have important mental health and quality of life implications.",

            "abstractNote": "This study reports the revival experiences of persons who once were reconciled to their death from HIV/AIDS but who, as a result of dramatic treatment responses, now believe they may survive (popularly known as the Lazarus Syndrome). A purposive sample of men and women living with HIV infection or AIDS were interviewed in six focus groups. As part of a larger study of uncertainty in HIV illness, participants described their uncertainty accompanying renewed health and a return to the joys and problems of continued life. While new discoveries about the disease and exciting antiretroviral therapies hold the promise of improved survival, ambiguity about the durability of treatment response and ultimate survival contribute to the level of uncertainty with which a patient must cope. The experience of uncertainty in the narratives about revival involved renegotiation. Participants described physical renewal as an unexpected new stressor forcing them to renegotiate: (a) feelings of hope and future orientation, (b) social roles and identities, (c) interpersonal relations, and (d) the quality of their lives. Implications for prevention, practice, research and theory are presented and suggestions for education and assistance are offered.",

            "abstractNote": "Communication is a means of managing uncertainty. In a state of uncertainty: (a) information seeking can reduce uncertainty by allowing for better discrimination between or among alternatives; (b) information seeking can increase uncertainty by increasing the number of alternatives, or by blurring the distinction between or among alternatives; and (c) information avoidance can maintain uncertainty. Individuals living with HIV or AIDS, like many other chronically‐ill or terminally‐ill individuals, must manage high levels of uncertainty about their illness. Participants in a focus group study of persons with HIV or AIDS reported effective uncertainty management, including managing uncertainty that was challenging, managing uncertainty that was essential for maintaining hope, learning to live with chronic uncertainty, and managing information problems. New information can serve uncertainty management even if it fails to reduce the number or ambiguity of alternatives, because new information can invite a reappraisal of uncertainty. A theory of uncertainty management based on these findings is offered.",

            "abstractNote": "Despite the fact that many individuals express a desire for more information and involvement in the health care process, it remains to be seen if they have adopted a more participative approach by becoming involved in decisions made about their health. Research indicates that, in actual practice, individuals are differentially willing or able to be active patients. AIDS patient activists are 1 group of individuals who have become more involved in their health care decision making. This study tests the reliability and validity of a measure of patient activism-the Patient Self-Advocacy Scale (PSAS)--designed to assess the dimensions of (a) increased illness and treatment education, (b) increased assertiveness in health care interactions, and (c) increased potential for nonadherence. Tests administered to 2 samples of participants (174 adults from an HIV-AIDS population and 21 8 adults from a general population) demonstrated that the PSAS was a reliable and valid measure of patient involvement in health care decision making.",

            "abstractNote": "In a study of AIDS activism and communication patterns between people with HIV or AIDS and health care personnel, parallel persuasive processes are described between social or political activism and personal self‐advocacy. The analysis of public and private discourse leads to 3 interrelated conclusions about AIDS activist behaviours at the collective and individual levels: (a) greater patient education about the illness and treatment options is encouraged, (b) a more assertive stance toward health care is promoted, and mindful nonadherence is considered. Activists perceived that their self‐advocacy behaviors, in turn, impact the physician‐patient interaction. In communicative interactions, education allows patients to challenge the expertise of the physician, assertiveness allows them to confront paternalistic or authoritarian interactional styles, and mindful nonadherence allows them to reject treatment recommendations and offer reasons for doing so. Participants reported that physicians had mixed reactions to their self‐advocacy attempts.",

            "abstractNote": "It is nearly impossible to watch television without encountering advertisements for pharmaceuticals that purport to ease a plethora of medical concerns. From restless leg syndrome to heartburn, there are new drugs available for a wide variety of health-related conditions. One of the more controversial remedies on this list is the Gardasil vaccine. Gardasil was designed by the drug company Merck to protect against 4 strains of the sexually transmitted disease human papillomavirus. According to the Gardasil website, one of the human papillomavirus strains that Gardasil protects against causes 70% of cervical cancers. Marketing Gardasil as a miracle product and an anticancer vaccine, Merck in 2006 launched a massive public relations campaign to encourage all young women between 9 and 26 years of age to receive Gardasil.",

Media \"reconstruction\" of the device as the \"bionic ear\" was typically encouraged by implant pioneers. Unexpectedly, however, a \"counter-rhetoric\" based on a very different understanding of deafness emerged. With it, deaf people are slowly succeeding in gaining influence over the further deployment of the technology. The analysis suggests modifications to existing theoretical models of technological change in medicine.",

            "abstractNote": "There is growing concern in the medical community about potential genetic determinism in the patient population. Limited information about the public understanding of genetic factors in disease formation is available. To access public perceptions of potentially deterministic phrasing of genetic risk factors, we sought to establish interpretations of the phrase, \"a gene for heart disease.\" Focus groups in urban, suburban, and rural communities were conducted from July through October, 2001 in Georgia. A total of 108 participants were recruited. Participants were recruited to balance sex and racial representation. We used three outcome measures for participants understandings of the phrase: (1) participants' statements of the meaning of the phrase; (2) the level of determinism assigned to genetic factors by participants; and (3) participant reports of the health consequences of having \"a gene for heart disease.\" Participants did not report a single interpretation of the phrase. There were dominant participant interpretations under each outcome measure: (1) \"a gene for heart disease\" was interpreted as meaning genetic and environmental factors both played roles in disease formation; (2) genetic predisposition was perceived as heightened, not absolute, risk; (3) the perceived health impact was a greater risk of becoming sick. Minority interpretations were found under each measure. Overall, naming \"a gene for heart disease\" does not appear to have a deterministic impact on a plurality of participants' perceptions of risks associated with genetic factors. Genetic fatalism in patient populations may be confined to a sizable minority. Important considerations for provider intervention and patient education are indicated.",

            "abstractNote": "Critics of genetic discourse are concerned that deterministic and discriminatory views of genetics are increasingly becoming adopted. These views argue that current genetic discourse becomes a source of power whereby powerful institutions harm people with so-called “bad” genes. This essay argues that current analyses of the power of genetics discourse are grounded in an improper reading that disempowers patients. Deploying Michel Foucault's concept “care of the self,” this essay claims that genetics discourse is better understood as a way that patients take on power through rhetoric rather than a force that has power over patients. Through a close reading of the “My Family Health Portrait” program, this paper argues that patients experience a process of “subjection” wherein they become agents of and objects of genetics discourse both. This alternative mode of analyzing the power of genetics discourse has implications for our collective understanding of the operations of the care of the self and the uses of genetic information that we propose.",

            "abstractNote": "This paper discusses how the American public accounts for the concerns that they have about genetic research and the benefits that they foresee. We develop a general framework for discussing public claims about genetic technology based on Stephen Toulmin's model of warrants in argumentation. After a review of the results from public opinion polls about genetic research, we present a focus group study of public understandings of genetics. We outline the warrants, or publicly accepted “good reasons”, that this group offers for accepting some aspects of genetic technology and for rejecting other aspects.             "abstractNote": "Family health history (FHH) can identify families at increased risk for disease. Purpose. To learn mothers’ (1) perceptions of the benefits of FHH and (2) willingness to complete a FHH tool, My Family Health Portrait (MFHP). Methods. Qualitative in-depth interviews were conducted with mothers recruited through Cincinnati Children’s Hospital. Deductive and inductive codes were developed. Results. A total of 25 mothers were interviewed. Perceived benefits included keeping the pediatrician informed (n = 12; 48%) and preventive screenings recommended based on FHH (n = 10; 40%). Participants had positive impressions of MFHP and felt that it was user-friendly (n = 17; 68%). Lack of FHH knowledge was the most common challenge to completing MFHP, but most respondents stated that they would be able to complete MFHP prior to their child’s medical appointment (n = 23; 92%). Conclusion. Mothers are interested in and may be motivated to complete a parent-generated FHH prior to a pediatric appointment. Future research should focus on FHH implementation in practice.",
