A standard method of estimation has been to sum the direct costs of expenditure on prevention and treatment, and the indirect costs of productive labour time lost. This paper discusses the many problems in using such data to reflect the burden to society or the potential benefits from control. Studies have generally focussed on febrile illness, overestimating the burden of uncomplicated malaria, but underestimating the costs of severe illness, other debilitating manifestations, and mortality. Many use weak data to calculate indirect costs, which fail to account for seasonal variations, the difference between the average and marginal product of labour, and the ways households and firms ‘cope’ in response to illness episodes. Perhaps most importantly, the costs of coping mechanisms in response to the risk of disease are excluded, although they may significantly affect productive strategies and economic growth. In 2011–2012, a number of malaria cases reported in Evrotas, Laconia, in Southern Greece, where a large number of immigrants from malaria-endemic countries live and work. A total of 84 malaria cases, both in immigrants (38 cases) and in Greeks (46 cases), were detected. A number of malaria control measures were deployed in the area since 2011. Despite a decrease of the number of malaria cases in 2012, elimination could not be achieved, and thus antimalarial MDA was considered as an additional measure to prevent the reestablishment of the disease. During 2013 and 2014, a combination of two drugs was administered under direct observation to all immigrants in the epicenter of the 2011 Evrotas outbreak. The antimalarial MDA program was started on July-August and was successfully completed by 1094 immigrants (87.3% coverage). No serious adverse events were recorded except one case of primaquine-induced hemolysis due to false G6PD test result. In 2013 and 2014, no malaria cases were recorded in Evrotas, Laconia. Owing to the drug's neuropsychiatric contraindications and its high rate of inducing neuropsychiatric symptoms, which are contraindications to the drug's continued use, the routine prescribing of mefloquine in military settings may be problematic. Due to these considerations and to recent concerns of chronic and potentially permanent psychiatric and neurological sequelae arising from drug toxicity, military prescribing of mefloquine has recently decreased. In settings where mefloquine remains available, policies governing prescribing should reflect risk-benefit decision-making informed by the drug's perceived benefits and by consideration both of the risks identified in the drug's labeling and of specific military risks associated with its use.
