The objective of this study is to appraise the evidence describing process or effectiveness of public-private partnerships (PPPs) that aim to promote population health, and analyse how characteristics such as independence or competing interests influence the results of their evaluation.\nMethods: We carried out a systematic search of Medline and Web of Science to identify scientific reports evaluating the process or effectiveness of PPPs that aim to promote population health. Two reviewers applied inclusion criteria, extracted and evaluated study quality. We classified PPPs according to the health problem tackled, the independence of the evaluation, and the potential for competition between business interests of the private partner and health promotion activity undertaken. We classified the conclusions of the evaluation as positive (supportive/tentatively supportive) or negative (semi-critical/critical).\nResults: We identified 36 studies evaluating 25 PPPs. Evaluations that were favourable to the use of PPPs in health promotion were more frequently classed as “not independent” and of poor quality. On the other hand, negative evaluations were more common when the PPP involved a private partner with a high potential for competition between the health promotion activity undertaken and their financial interests. PPPs that sought to prevent noncommunicable diseases were more frequently negatively evaluated compared to PPPs tackling infectious disease or other types of health problem. Almost all of the evaluations evaluated process, with only 2 papers reporting quantitative health related outcomes.\nConclusions: There is still a lack of sound evidence supporting the effectiveness of public-private partnerships in health promotion, and the evidence base is skewed by non-independent evaluations. Drawing on a case study of an Austrian diversity-sensitive health promotion project related to obesity prevention, we show how the articulation of these trends—governance by buzzwords and projectification—often leads to not unproblematic and often paradoxical outcomes. Buzzwords such as “diversity” become especially important in an innovation-driven environment encouraging a promissory rhetoric. At the same time, the project form shapes and restricts how buzzwords (as typically vague terms that need to be fleshed out) are articulated and translated into a specific project design. In our case study of an obesity prevention program, the need to translate diversity into a “doable” project encouraged the identification of seemingly clearly delineated target groups and thus promoted a rather narrow understanding of diversity, which stands in tension with much more fluid and context-sensitive ways of performing “diversity.” We show how actors grapple with these paradoxes. The South exists only where group expression can be found, and universal international organizations represent \" cardinal space \" par excellence. Third World states have tried to use international bodies to build their own political space and to try to loosen the outside constraints weighing on their development. They have used a threefold strategy : reappropriation and reinterpretation of Western norms for their own ends, non-alignment, and collective bargaining. After a semblance of success in the early 1970s, the South seems to have lost its bargaining power and its capacity to shape itself into a political force. The forums in which it holds the majority adopt resolutions which can have no practical consequences, while there is an increase in the bilateralization of public aid and the influence of the international financial institutions with which each country negotiates individually. The Third World does however have its own political culture, a way of perceiving itself as a group of developing countries and of conceiving international relations. The analysis of the interest groups' expectations and demands towards the government's statements and the negotiation of free trade agreements shows that some associations and business groups had to be convinced of the pertinence of a new policy. U.S. policy towards bilateral free trade agreements was deployed at a time when the business community elites were resistant to the idea of including a new policy stance which departed from the traditional American government commitment towards multilateralism. Some associations, specifically those representing the large business community, only leaned towards supporting the bilateral free trade policy after they understood that this approach could contribute in creating favorable conditions that would lead to the adoption of an ambitious and ground-breaking Uruguay Round agenda, and most importantly, an agenda in line with American commercial goals.
