postoperative complications, not all healthcare institutions have universally adopted this practice. A multicenter, retrospective, descriptive study was conducted to determine the effect of stoma site marking on stomal and peristomal complications. The 1-year study included 748 patients (408 [54.5%] male, mean age 56.60 ± 16.73 years) from eight stomatherapy units in Turkey. Patient data, including age, gender, diagnosis, type of surgery, history of preoperative stoma site marking, person performing the marking, and postoperative complications, were obtained from patient records, abstracted, and analyzed. Cancer was the reason for the operation in 545 (72.9%) of the cases. In 287 patients (38.4%), the stoma and wound care nurse and/or surgeon marked the stoma area; this occurred 1 day before or on the day of surgery according to Wound Ostomy Continence Nurses Society and American Society of Colon and Rectal Surgeons recommendations. Stomal/ peristomal complications developed in 248 (33.2%) persons; the most frequently observed complications in patients were parastomal skin problems (136, 48.7%), mucocutaneous separation (52, 18.6%), and retraction (31, 11.1%). The rate of complications was higher among patients whose stoma site was not marked than among those whose stoma site was marked (22.9% and 46%, respectively; P <0.001). The results of this study confirm the stoma area should be marked preoperatively in all planned surgical interventions in order to reduce the risk of postoperative complications. Equipping staff to be better able to engage this client group is important, and web-based, self-directed learning is a potentially cost-effective way to train staff. This study examined the implementation of a web-based training programme called Readiness Enhancement Management Strategies (REMS) in three types of service. Completion rates were 94.4% in community health services; 92.3% in prison offender health services; and 46.5% in probation services. Staff found the content of REMS acceptable and useful. This study demonstrated that staff in NHS and criminal justice settings can complete REMS, but staff in probation services are challenged by time pressures and limited computer access. Staff at probation sites were less familiar with PD issues compared with the NHS staff. A web-based staff training programme called Readiness Enhancement Management Strategies (REMS) was developed to promote the engagement of people with personality difficulties in treatment. This 'proof of concept' study examined the REMS implementation process, its acceptability and the feasibility of using service data for future evaluation. Staff in six services working with people diagnosed with personality disorder or undiagnosed people with personality difficulties were eligible to participate: two community health services, two prison offender health services and two probation services. Of 92 eligible staff, 74 were available to undertake REMS. These staff completed knowledge and acceptability surveys and rated service user engagement with treatment. The proportion of treatment sessions attended by service users was collected for a 30-week period. REMS completion rates were community - 94.4%, prison - 92.3% and probation - 46.5%. Three quarters of participants rated REMS as 7 out of 10 or higher. All teams were able to provide service data for the study period. REMS can be implemented by National Health Services (NHS) staff in community and prison settings, but technology issues need to be resolved in probation services. interventions can enhance health and, in general, older people are motivated to take such preventive action. The National Service Framework for Older People sets the promotion of healthy living as a standard for UK National Health Service Trusts. However, the delivery of health promotion is 'haphazard'; patients aged over 65 years rarely report receiving health promotion, and reports from nurses suggest they are unsure how to deliver effective health promotion.\nMETHOD: A theory-based survey of all nurses working in a department specializing in the care of people aged over 65 years, 41% of questionnaires were returned.\nRESULTS: The majority of nurses identified examples of health promotion and 88% judged health promotion to be effective and worthwhile. Three quarters of the sample viewed health promotion as part of their role and most of the respondents were confident in their ability to provide health promotion. However, the respondents also reported that health promotion was not appropriate for all their patients and considered it an increasingly difficult task as people got older. Organizational barriers to the routine provision of health promotion were identified.\nCONCLUSION: Nurses working on wards for people aged over 65 years are mostly positive about integrating health promotion into their everyday work. However, for health promotion to be routinely implemented, all nursing staff need to feel confident in undertaking the task and believe it is worthwhile. Health promotion needs to be awarded greater importance by hospital management to ensure that it does not conflict with other work priorities. Until this happens the provision of health promotion in hospitals will remain sporadic and lack conviction.\nRELEVANCE TO CLINICAL PRACTICE: With an increasing older population nurses need to be confident and proficient at implementing health promotion to patients aged over 65 years.
