Systems biology - holistic, global and integrative in approach - has given rise to systems medicine, a systems approach to health and disease. Systems medicine promises to (1) provide deep insights into disease mechanisms, (2) make blood a diagnostic window for viewing health and disease for the individual, (3) stratify complex diseases into their distinct subtypes for a impedance match against proper drugs, (4) provide new approaches to drug target discovery and (5) generate metrics for assessing wellness. P4 medicine, the clinical face of systems medicine, has two major objectives: to quantify wellness and to demystify disease. Patients and consumers will be a major driver in the realization of P4 medicine through their participation in medically oriented social networks directed at improving their own healthcare. P4 medicine has striking implications for society - including the ability to turn around the ever-escalating costs of healthcare. The challenge in bringing P4 medicine to patients and consumers is twofold: first, inventing the strategies and technologies that will enable P4 medicine and second, dealing with the impact of P4 medicine on society - including key ethical, social, legal, regulatory, and economic issues. Managing the societal problems will pose the most significant challenges. To evaluate and implement interventions, information is needed regarding their reach as well as regarding the characteristics of program users and non-users.\nOBJECTIVE: In this study, individual, lifestyle, and health indicators were investigated in relation to initial, and sustained participation in an Internet-delivered physical activity and healthy nutrition program in the workplace setting. In addition, determinants of program website use were studied.\nMETHODS: Determinants of participation were investigated in a longitudinal study among employees from six workplaces participating in a two-year cluster randomized controlled trial. The employees were invited by email to participate. At baseline, all participants visited a website to fill out the questionnaire on lifestyle, work, and health factors. Subsequently, a physical health check was offered, followed by face-to-face advice. Throughout the study period, all participants had access to a website with information on lifestyle and health, and to fully automated personalized feedback on the questionnaire results. Only participants in the intervention received monthly email messages to promote website visits during the first year and had access to additional Web-based tools (self-monitors, a food frequency questionnaire assessing saturated fat intake, and the possibility to ask questions) to support behavior change. Website use was monitored by website statistics measuring access. Logistic regression analyses were conducted to identify characteristics of employees who participated in the program and used the website.\nRESULTS: Complete baseline data were available for 924 employees (intervention: n=456, reference: n=468). Lifestyle and health factors were not associated with initial participation. Employees aged 30 years and older were more likely to start using the program and to sustain their participation. Workers with a low intention to increase their physical activity level were less likely to participate (Odds Ratio (OR)=0.60, 95% Confidence interval (95%CI), 0.43-0.85) but more likely to sustain participation throughout the study period (ORs ranging from 1.40 to 2.06). Furthermore, it was found that smokers were less likely to sustain their participation in the first and second year (OR=0.54, 95%CI 0.35-0.82) and to visit the website (OR=0.72, 95%CI 0.54-0.96). Website use was highest in the periods immediately after the baseline (73%) and follow-up questionnaires (71% and 87%). Employees in the intervention were more likely to visit the website in the period they received monthly emails (OR=5.88, 95%CI 3.75-9.20) but less likely to visit the website in the subsequent period (OR=0.62, 95%CI 0.45-0.85).\nCONCLUSIONS: Modest initial participation and high attrition in program use were found. Workers with a low intention to change their behavior were less likely to participate, but once enrolled they were more likely to sustain their participation. Lifestyle and health indicators were not related to initial participation, but those with an unhealthy lifestyle were less likely to sustain. This might influence program effectiveness. Regular email messages prompted website use, but the use of important Web-based tools was modest. The project attempted to intervene with the daily routines of staff members, who were believed to be bodily inactive while performing their daily tasks. In seeking to add movement and exercise to the daily routines of staff members, ‘Body & Bureau’ was understood to promote a new pattern of behaviour in the office that would help avoiding life style conditions such as overweight, hypertension, pre-diabetes and increased levels of cholesterol, generally considered to expose office workers to increased risks of poor health. The article addresses the ways in which the bodies of staff members were objectified by the project and the ways in which it influenced their everyday lives, and how it became a catalyst for new modalities of social relations within the office. Subsequent to individual health tests being performed, a number of staff members were classified as being overweight, which led them to form what they ironically dubbed ‘The Group ofMourners’ in order to cope with their sense of sadness. About one month later, however, the group decided to become ‘The Group of Enthusiasts’, thereby taking on the tasks of changing the way they perceived of themselves and developing into an exercise support group. While the project had set out to promote movement as integral to the work routines, which was set against an undesirable notion of exercise for its own sake, the staff members ended up by pursuing the latter rather than the former. While the authors acknowledge the importance of tracing how bodies become objects of political power in the Foucauldian sense, they underscore that the nitty-gritty details of how projects play out in local social worlds such as the Development Department will eventually shape the trajectory of the intervention. Taking their cue from anthropologists Nancy Schepher-Hughes and Margaret Lock, the authors argue that a good starting point for the analysis of how a bodily objectifying project like that of ‘Body & Bureau’ intervenes in the social reality of an office is to pay equal attention to the political body, the individual body, and the social body. Similarly, these three perspectives on the body are used to explain how the state sponsored intervention politicizes the body of office workers, which leads to the testing and measuring of individual bodies, which finally becomes a collectivizing moment for staff members, because they eventually chose to socialize around their common condition of overweight, thereby producing new ways of relation in the office. The foundation necessary to develop quantified knowledge about society is the population. If quantified knowledge changes society, the question arises on how individuals become to be represented as population. The population has to be extracted from individuals in a process that we call “populationisation.” This encompasses the development of the individual into a segment of a population through the compilation of individual data into population data and its analysis. To describe the process of populationisation, we follow a statistical risk assessment tool, the German Diabetes Risk Score, from its inception in a cohort study to its entry into the public sphere of German society. The population is extracted from individuals that function as research subjects through an identification number. Preprocessed information is entered into a dynamic database that enables its rearrangement according to main aggregates. Through populationisation, a signified is enacted that can be sliced up and that is equal in all its parts. Now predictions can be computed and fed back to society as tools of moral judgment. on prescribing patterns have not been systematically evaluated.\nOBJECTIVE: This study investigates how pharmaceutical sales representative visits influenced physicians' company-specific drug preferences and prevalence of steroid prescribing.\nMETHODS: Observational cohort study in Funen County, Denmark, including 165 general practices visited 832 times by pharmaceutical representatives and 54 080 patients treated with asthma drugs. Visits were conducted from 2001 to 2003. Our main outcome measures were (i) company-specific drug preferences measured as the proportion of dispensings of the promoted drug among all dispensings of fixed combinations of inhaled corticosteroid and long-acting beta2-agonists and (ii) the proportion of patients receiving repeated beta2-agonist dispensings who were treated with inhaled steroids.\nRESULTS: The first visit had a statistically significant effect on the GPs' drug preference in favour of the marketed drug [odds ratio (OR), 2.39; 95% confidence interval (CI), 1.72-3.32]. The effect on drug preference increased further after the second visit (OR, 1.51; 95% CI, 1.19-1.93), while there was no significant change after the third visit (OR, 1.06; 95% CI, 0.94-1.20). Pharmaceutical sales representative visits did not influence the overall treatment pattern with inhaled steroids (OR, 1.01; 95% CI, 0.97-1.06).\nCONCLUSIONS: Pharmaceutical sales representative visits markedly increased the market share of the promoted drug, but only the two first visits had significant impact.
