"abstractNote": "Neisseria meningitîdis est une bactérie à Gram négatif, à la fois commensale et pathogène de l'espèce humaine. Au cours de ces travaux, nous nous sommes intéressés à l'interaction entre cette bactérie et son hôte. La colonisation des cellules par N, meningitîdis est un processus complexe qui intègre : l'adhésion aux cellules de l'hôte dépendante de la présence de Pili de type IV (Pt4), la multiplication et la survie des bactéries, la formation de microcolonies et leur maintien sur les cellules puis l'envahissement progressif de la surface cellulaire. Cette étape de colonisation est centrale dans le cycle commensal et dans la pathogenèse de cette bactérie. Elle a donc été largement étudiée par le passé. Cependant deux aspects avaient été négligés dans la plupart de ces études. (1) La colonisation des cellules humaines se fait dans un contexte de flux de liquides, mucus ou sang, qui génère des forces hydrodynamiques qui s'opposent à la colonisation. (2) L'interaction entre une bactérie et une cellule eucaryote implique une participation de chacune des deux cellules. Laicolonisation des cellules humaines par N. meningitîdis induit plusieurs changements physiologiques dont une réorganisation complexe du cytosquelette d'actine aboutissant à la formation de projections membranaires à la surface des cellules hôte. C'est donc en intégrant ces deux aspects que nous avons abordé la question de la colonisation de l'hôte par N. meningitidis. Ces travaux de thèse ont permis de montrer que la réorganisation du cytosquelette d'actine induite par la bactérie participait activement à la colonisation. La formation des projections membranaires permet à la plupart des bactéries au sein de la microcolonie, d'établir des liaisons robustes avec la membrane plasmique. Ces liaisons sont les seules qui permettent à N. meningitidis de résister aux forces hydrodynamiques présentes dans son environnement naturel. De plus, nous avons montré que les Pt4 de la bactérie sont le vecteur moléculaire de l'induction de la réorganisation du cytosquelette. Nous avons identifié deux protéines dans ces pili qui assurent la fonction inductrice. Ces protéines, les pilines mineures PilV et PiLX, sont nécessaires à l'induction de la réorganisation'du cytosquelette d'actine, et par conséquent, à la colonisation des cellules humaines par N. meningitidis. Dans ces deux piline^ mineures, une région particulière caractérisée par un pont disulfure est essentielle à cette fonction. Cette région est exposée à l'extérieur de la bactérie et pourrait agir comme ligand d'un récepteur cellulaire. Ces travaux ont associés une nouvelle fonction aux Pt4 de la bactérie. De plus, nous avons découvert une fonction inédite' à cette réponse cellulaire dans la colonisation de N. meningitidis. Le détournement des voies de signalisation de la cellule hôte permet à la bactérie de résister aux conditions hydrodynamiques de son environnement naturel. "abstractNote": "Neisseria meningitidis (ou méningocoque) est une bactérie responsable de septicémies et de méningites. Le réservoir du méningocoque est le rhinopharynx de l'homme où coexistent de nombreuses espèces bactériennes au sein d'un biofilm. Des études récentes attribuent un rôle à certains systèmes toxine-antitoxine dans la formation des biofilms polymicrobiens et dans les phénomènes de compétition entre bactéries. L'analyse des génomes de méningocoques a révélé l'existence de trois loci contenant quatre gènes appelés mafB suivis de quatre petits gènes que nous avons nommés mafl, dont les fonctions sont inconnues. Des études chez E. Coli et chez le méningocoque nous ont permis de démontrer que la surexpression des protéines MafB1, MafB-related et MafB3 est toxique pour ces bactéries. Nous avons ensuite démontré que les gènes maflB1, maflB-related et maflB3 protégeaient spécifiquement de la toxicité des protéines MafB1, MafB-related et MafB3 respectivement. D'autre part, nous avons mis en évidence la présence de la toxine MafB1 dans le surnageant de culture d'une souche de méningocoque surexprimant cette protéine ce qui démontre que la toxine est sécrétée. Enfin, nous avons caractérisé l'activité enzymatique de MafB1. Il s'agit d'une endoribonucléase capable de cliver l'ARN au niveau d'une séquence contenant des uridines. Nous avons donc caractérisé expérimentalement une nouvelle famille de toxines sécrétées chez le méningocoque. Comme dans le cas des autres systèmes toxine-antitoxine sécrétés, Maf pourrait jouer un rôle dans les phénomènes de compétition entre bactéries, notamment au sein du biofilm rhinopharyngé.",

Ce travail a tout d'abord consisté à une présentation de l'épidémiologie nationale des méningites bactériennes de l'enfant, grâce aux données colligées par l'Observatoire National des Méningites Bactériennes dont il n'existe aucun équivalent dans le monde. Plusieurs constatations: l'éradication presque complète d'Haemophilus influenzae b depuis l'introduction de la vaccination systématique, la prédominance du méningocoque dans les méningites bactériennes de l'enfant et l'effet modéré de la mise en place de la vaccination anti pneumococcique systématique lié au phénomène de remplacement sérotypique et à l'augmentation de l'incidence des méningites à pneumocoque dans la population non ciblée par la vaccination. En seconde partie, il s'agit de la présentation des résultats d'une étude rétrospective menée au CHU de Montpellier entre 2004 et 2009 sur les méningites purulentes de l'enfant. Les tendances épidémiologiques locales se sont avérées en grande partie proches des tendances nationales. On constate cependant une différence dans la répartition des germes responsables: il y a autant de méningites à pneumocoque qu'à méningocoque au CHU de Montpellier. Les modifications récentes du calendrier vaccinal vont encore influencer l'épidémiologie des méningites bactériennes de l'enfant. La poursuite de la surveillance paraît donc indispensable.",

"abstractNote": "Introduction : La méningite C est la deuxième méningite à méningocoque en France. Face à l’une des incidences la plus élevée d’Europe, le Haut Conseil de Santé Publique a émis de nouvelles recommandations en juin 2009. L’objectif était donc d’étudier et de déterminer les circonstances de prescriptions du vaccin et son intégration dans le calendrier vaccinal. Il s’agissait également de dresser un état des lieux des éventuels freins et motivations à cette vaccination afin d’établir deux profils « vaccinateur » et « peu vaccinateur ». Méthode : Une étude déclarative descriptive a été réalisée à l’aide d’un questionnaire adressé à 400 médecins généralistes de la Somme en 2011. Résultats : 40 % des médecins interrogés ont participé à cette étude. Les médecins déclaraient connaître les nouvelles recommandations à 89,1% et 48,7% prescrivaient le vaccin systématiquement. Ils affirmaient vacciner les nourrissons à l’âge de 18 mois en moyenne, et 41,9 % d’entre eux prescrivaient le vaccin entre 80 et 100 % chez les 12-24 mois constituant le groupe vaccinateur. Discussion et conclusion : L’intégration de ce vaccin dans le calendrier vaccinal reste difficile. Malgré l’intérêt porté par les médecins généralistes pour ce vaccin, il existe des freins dont le principal est la réticence des parents. La pratique des formations médicales continues, de bonnes connaissances et l’efficacité du vaccin restent les principaux facteurs influençant la vaccination. Cette étude confirme l’implication des médecins généralistes dans leur rôle de premier maillon de chaine de prévention. Des campagnes de santé publique pourraient contribuer à les aider à améliorer cette couverture vaccinale. Introduction : Meningitis C is the second meningococcal meningitis in France. Faced with one of the highest incidence in Europe, the French high council for public health has developed news recommendations in June 2009. Therefore the objective was to study and to determine the circumstances of this vaccine prescription and its integration in the vaccine calendar. It was also to analyze the barriers and motivations for this vaccination in order to establish two physician profiles “vaccinators” and “non vaccinators”. Method : A declarative descriptive study was made using a questionnaire addressed to 400 general practitioner in the Somme region in 2011. Results : 40% of targeted physicians took part in this study. 89.1 % of general practitioners declared to know the recommendations, and 48.1% systematically prescribed the vaccine. They declared to vaccinate infants at an average age of 18 months and 41.9% of them prescribed the vaccine between 80 and 100% in the group of the 12-24 months represented the vaccinator group. Discussion and conclusion : The integration of this vaccine in the vaccine calendar remains difficult. In spite of the general practitioner’s interest for this vaccine, the main barrier is parents’ reticence. The practice of continued medical education, good knowledge and the vaccine’s effectiveness are the main drivers of vaccination. This study confirms the general practitioners’ commitment in their role of a first link in the prevention chain. Public health campaigns could contribute to help them improve protection through vaccination.",
