Previous reports suggest an association between TBI and chronic pain syndromes (eg, headache) thought to be more common in patients with mild TBI and in those who have sustained brain injury from violent rather than unintentional trauma. Comorbid psychiatric disorders such as posttraumatic stress disorder (PTSD) may also mediate chronic pain symptoms.\nOBJECTIVES: To determine the prevalence of chronic pain as an underdiagnosed consequence of TBI and to review the interaction between chronic pain and severity of TBI as well as the characteristics of pain after TBI among civilians and combatants.\nEVIDENCE ACQUISITION: The Ovid/MEDLINE database was searched for articles published between 1951 and February 2008 using any combination of the terms brain injury, pain, headache, blast injury, and combat (combat disorders, war, military medicine, wounds and injuries, military personnel, veterans). The PubMed and MD Consult databases were searched in a similar fashion. The Cochrane Collaboration, National Institutes of Health Clinical Trials Database, Meta-Register of Current Controlled Trials, and CRISP databases were searched using the keyword brain injury. All articles in peer-reviewed journals reporting original data on pain syndromes in adult patients with TBI with regard to pain prevalence, pain category, risk factors, pathogenesis, and clinical course were selected, and manual searches were performed of their reference lists. The data were pooled and prevalence rates calculated.\nEVIDENCE SYNTHESIS: Twenty-three studies (15 cross-sectional, 5 prospective, and 3 retrospective) including 4206 patients were identified. Twelve studies assessed headache pain in 1670 patients. Of these, 966 complained of chronic headache, yielding a prevalence of 57.8% (95% confidence interval [CI], 55.5%-60.2%). Among civilians, the prevalence of chronic pain was greater in patients with mild TBI (75.3% [95% CI, 72.7%-77.9%]) compared with moderate or severe TBI (32.1% [95% CI, 29.3%-34.9%]). Twenty studies including 3289 civilian patients with TBI yielded a chronic pain prevalence of 51.5% (95% CI, 49.8%-53.2%). Three studies assessed TBI among 917 veterans and yielded a pain prevalence of 43.1% (95% CI, 39.9%-46.3%). PTSD may mediate chronic pain, but brain injury appears to have an independent correlation with chronic pain.\nCONCLUSIONS: Chronic pain is a common complication of TBI. Physiologically, migraine headache is a systemic event, affecting multiple neurophysiological systems. Treatment often calls for a multidisciplinary approach. Research supports the efficacy of both general biofeedback and, to a lesser extent, neurofeedback in the treatment of headache, including migraine. Abnormal electrophysiological patterns, detectable with quantitative EEG, are frequently found in patients with migraine, especially after closed head injury. Research has also shown the frequent presence of trigger point activity in several areas of the musculature of the head and neck in headache patients, including those with migraine. Finally, the role of stress has been reported in the onset and exacerbation of headache pain. The authors provide a case study showing the application of quantitative EEG, surface electromyography (SEMG), and psychophysiological stress profiling in the assessment of a 56-year-old female with closed head injury and migraine headache. The treatment included myofascial massage with trigger point release, SEMG training to balance asymmetric muscle tension patterns, and a stress management program, including guided visualization and breath training. Head-trauma-related symptoms are quite common and varied. Eighty to ninety percent of individuals recover neurologic function within 10 days of injury. Headache is the most frequently reported symptom of sport-related concussion, and one of the most common symptoms requiring pharmacological management, with up to 86 % of athletes reporting posttraumatic headaches. It is estimated that 15 % of individuals will develop chronic posttraumatic headache as a result of mild head injury. The most common recognizable forms of posttraumatic headache in concussion are tension-type headache, cervicogenic headache, migraine, and combined tension-type headache and migraine. The posttraumatic headache is thought to impair other functions such as energy, sleep, attention, higher-level cognitive function, and emotion. Headaches can also result in decreased activity tolerance and are frequently exacerbated during exercise. Physical activity tolerance testing is a component of return-to-play rehabilitation programs. Once the headache type has been determined, treatment is often consistent with the respective disorder. Experts urge a cautious approach, an intense preventive and acute regimen of medicines treating immediate post-injury headache and other symptoms of concussion may mask unresolved symptoms and lead to the premature return of an athlete to the playing field or return a soldier to battle prematurely. It was hypothesized that those with posttraumatic migraine would take longer to recover on all clinical measures.\nStudy Design: Cohort study (prognosis); Level of evidence, 2.\nMethods: A total of 296 student athletes who sustained sport-related concussions (migraine, 52; headache, 176; no headache, 68) were evaluated on measures of balance performance, cognition, and symptoms. Patients were categorized into posttraumatic migraine, posttraumatic headache, and no headache groups based on symptoms reported 1 day after injury. Serial testing was conducted with the Balance Error Scoring System, the Standardized Assessment of Concussion, and a graded symptom checklist to evaluate balance performance, cognition, and symptoms, respectively, at baseline, at the time of injury, after the event, and at days 1, 2, 3, 5, 7, and 90.\nResults: Student athletes with posttraumatic migraine experienced greater symptom severity scores than did those with posttraumatic headache and no headache at the time of injury, after the event, and through day 7 (P < .001 for all). By day 7, symptom severity scores reported by patients with posttraumatic migraine still demonstrated deficits relative to baseline (Δ = 5.6 ± 1.2) compared with patients with headache (Δ = −0.8 ± 0.7) and those with no headache (Δ = −1.4 ± 1.1). Patients with headache experienced greater symptom severity scores than patients with no headache at the time of injury (P < .001), after the event (P < .001), and at day 1 (P < .001), day 2 (P = .005), and day 3 (P = .038). Much smaller differences were observed between the headache and no headache groups beyond day 3 (P > .05). There were no group differences on balance performance (P = .439) and cognitive testing (P = .200) over time. Female patients were 2.13 times more likely than males to report posttraumatic migraine characteristics after concussion.\nConclusion: Posttraumatic migraine characteristics are related to prolonged symptom recovery after sport-related concussion.
