One of these traditional preparations with potentially beneficial properties is aqueous extracts of Sutherlandia frutescens, a shrub indigenous to the Western Cape region of South Africa. The aims of this study were to evaluate in vitro efficacy of these preparations on the MCF-7 breast adenocarcinoma and MCF-12A non-tumorigenic cell lines in terms of cell proliferation, cell morphology and possible induction of cell death.\nMaterials and methods\nCrystal violet staining was used to evaluate cell proliferation, light-and fluorescence microscopy were used to investigate both intracellular and extracellular morphological features of apoptosis and autophagy (e.g. membrane blebbing, condensed chromatin and intracellular lysosomes), while flow cytometry quantified cell cycle changes and induction of apoptosis through analysis of the flip-flop translocation of phosphatidylserine.\nResults\nCrystal violet staining showed a time- and dose specific response to aqueous Sutherlandia frutescens extracts, revealing exposure to 1 mg/ml aqueous extract for 48 h to be ideal for comparing the differential effects of Sutherlandia frutescens in the MCF-7 and MCF-12A cell lines. Microscopy showed distinct morphological changes with hallmarks of apoptosis being observed in both cell lines. The pharmaceutical industry has corrupted the practice of medicine through its influence over what drugs are developed, how they are tested, and how medical knowledge is created. Since 1906, heavy commercial influence has compromised congressional legislation to protect the public from unsafe drugs. The authorization of user fees in 1992 has turned drug companies into the FDA's prime clients, deepening the regulatory and cultural capture of the agency. Industry has demanded shorter average review times and, with less time to thoroughly review evidence, increased hospitalizations and deaths have resulted. Meeting the needs of the drug companies has taken priority over meeting the needs of patients. Unless this corruption of regulatory intent is reversed, the situation will continue to deteriorate. MDR gene products: P-gp, MRP and BCRP and metabolizing enzyme (cytochrome P450) together constitute a highly effective barrier for many orally absorbed drugs. Therapeutic agents and active herbal constituents are substrates, inhibitors and inducers of both P-gp and CYP3A4. Inhibition of P-gp by pure herbal constituents such as hypericin, kaempferol and quercetin or extract of St. John's wort resulted in higher absorption of ritonavir and erythromycin in vitro. Similarly, inhibition of CYP by hypericin, kaempferol and quercetin also caused a remarkable reduction in cortisol metabolism in vitro. Conversely, an enhanced expression of both CYP3A4 and MDR-1 mRNA in Caco-2 cells was observed after chronic exposure of hyperforin, kaempferol, quercetin and hypericin, kaempferol, quercetin, silibinin respectively. Chronic exposure (96 h) of African herbal (Sutherlandia frutescens) extract (300 μg/mL) resulted in elevation of both P-gp and CYP3A4 expressions in LS180V cells. Cellular accumulation of [3H]Digoxin, a model P-gp substrate was significantly reduced indicating higher efflux. These in vitro results were further confirmed in rats. Administration of Sutherlandia-extract for 4 days significantly affected pharmacokinetics of nevirapine – a CYP3A4 (CYP3A2) substrate. Plasma concentration of nevirapine was diminished by 51% (Cmax) and 45% (AUC0-inf) compared to controls. Similar pharmacokinetics of fexofenadine, a P-gp substrate was also observed. Plasma concentration of fexofenadine was declined by 49.7% (Cmax) and 34.5% (AUC0-inf) compared to controls. Hepatic CYP3A4 and P-gp mRNA levels were enhanced by 3-fold and 1.75-fold respectively. Intestinal CYP3A4 (2-fold) and P-gp expressions (3-fold) was also elevated. Whole plant parts have been used and traditional healers claim that it can treat cancer. In this study it is shown that a crude aqueous Sutherlandia frutescens whole plant extract induced cytotoxicity in neoplastic cells (cervical carcinoma) and CHO (Chinese Hamster Ovary cells) cell lines. Morphological observation and monitoring with other biological assays involving chromatin condensation as well as phosphotidyl serine externalisation point to apoptotic responses. Further biochemical assays showed similar DNA fragmentation patterns induced by Sutherlandia frutescens extracts compared to other inducers of apoptosis such as staurosporine and ceramide. Furthermore, Sutherlandia frutescens extracts induced apoptosis was confirmed by flow cytometric analysis.
