This review examines the current evidence regarding the impact of this conversion on healthcare resource utilization and costs. Simulated decision models represent the majority of the research conducted in this area and appear to suggest that the availability of OTC loratadine reduces patient cost sharing, health-plan costs, and motor-vehicle accidents, and also improves work-related productivity. Other empirical research examining the policies adopted by health plans in response to the availability of OTC loratadine suggests that restricting access to prescription SGAs by placing these products in the third tier, thereby encouraging patients to purchase OTC loratadine, results in the greatest reduction in pharmacy-related costs for SGAs. However, early evaluations of such policies using health-plan data for the year 2003 show mixed results. Initial conclusions suggest that OTC loratadine has the potential to function as a safer treatment alternative to the sedating first-generation antihistamines in the treatment of allergic rhinitis. What is unclear is the impact of OTC loratadine on overall healthcare costs, as patients may choose the convenience of an OTC alternative to manage their allergic rhinitis in lieu of physician-supervised care. Further research is needed to examine if patients are unable to correctly diagnose their allergic rhinitis symptoms, which could potentially mask other chronic conditions and result in increased The Self Evaluation of Lovastatin to Enhance Cholesterol Treatment (SELECT) Study was designed to assess consumers' ability to self-select for treatment with lovastatin in an unsupervised setting. Subjects examined proposed OTC lovastatin cartons with labels that detailed an algorithm for self-selection based on age, lipid profile, and cardiovascular risk factors. Subjects viewed a carton with either a low-density lipoprotein cholesterol–based self-selection algorithm or one based on total cholesterol. Labels also contained warnings against use based on health conditions that might increase the risk of adverse events. Subjects were asked if the drug was appropriate for their use (self-assessment) and whether they would like to purchase the drug (purchase decision). A total of 1,326 consumers provided self-assessment decisions. After viewing the low-density lipoprotein cholesterol–based label, 82%, 36%, and 82% of those who self-assessed that the drug was appropriate for their use were correct with respect to the age, lipid, and risk-factor criteria, respectively. Corresponding numbers for the total cholesterol algorithm were 85%, 50% and 75%. Almost 90% of women aged <55 years who evaluated the drug indicated the drug was not right for them, and women in this age group made up only 9% of the total group of subjects who believed the drug was appropriate for their use. The label was also effective in discouraging use by women who were or may become pregnant, consumers with liver disease, and those with potential drug interactions. Doctors, nurses, and pharmacists need to discuss how they will respond to self medication practices, and ways of rewarding pharmacists for advising patients need to be found. Data from the 2005 National Survey on Drug Use and Health demonstrated that 6.4 million (2.6%) people aged 12 or older had used prescription drugs for nonmedical reasons during the past month. Of these, 4.7 million used pain relievers, 1.8 million used tranquilizers, and 1.1 million used stimulants. The nonmedical use of prescription drugs in the past month among young adults aged 18 to 25 increased from 5.4% in 2002 to 6.3% in 2005, primarily because of an increase in the abusive use of pain relievers.\nPhysicians need to watch for prescription and OTC medication abuse. We assessed physician attitudes towards a transition from prescription only to over-the-counter availability (rx-OTC) for oral contraceptive pills by disseminating an electronic survey directed primarily to residents training in Obstetrics and Gynecology (OBGYN) and Family Practice in the US. An overwhelming majority of 638 respondents (71 %) were against an rx-OTC switch for combined oral contraceptives and among this subset of respondents the primary concern was safety (92.3 %). Overall, respondents were evenly divided on the issue of an rx-OTC switch for progestin-only-pills but of those who opposed, 73.2 % cited safety as their primary concern. For progestin-only-pills female respondents were more likely to support OTC availability. Most OBGYN and Family Practice residents opposed to OTC availability for oral contraceptives cite safety as their primary concern.
