"abstractNote": "Several neural interfaces have been developed to control neuroprostheses and hybrid bionic systems. Among them, intraneural electrodes are very promising because they represent an interesting trade-off between the needs for high selectivity and for reduced invasiveness. However, in most of the cases, no particular attention has been devoted so far to the design of these systems starting from the mechanical properties of the system to be interfaced. The aim of this paper was to study and characterize in a quantitative way the piercing of peripheral nervous tissue in order to gather useful information to design intraneural interfaces able to reduce (as much as possible) the damages provoked by this task. In particular, attention has been paid to determine the values of force and pressure to carry out the piercing task in different velocity conditions. From the experimental data it was possible to characterize indirectly the tissue sinking under the needle tip. For each experimental velocity (ranging from 1 to 2000 mm/min) a threshold, under which the tissue cannot be pierced, has been calculated. The force magnitude required for piercing was shown to be in the range 0.3-25 mN for the different velocities. Moreover, differences between piercing carried out at very low velocity (multi-piercing) and at low velocity (mono-piercing) have been characterized and correlated with the physical characteristics of the nervous tissue. Experimental data have been integrated with a theoretical analysis of the neural interfaces piercing structures. The problem of buckling, representing for these structures the main cause of tissue piercing impossibility, has been analyzed. The nonlinear theoretical model allows to compare different needle geometries and materials with regard to piercing possibility at different velocities. Moreover, an optimization of piercing elements geometry with regard to amount of used material and space has been provided.",

The INI encapsulated with 6 µm Parylene-C was immersed in phosphate buffered saline (PBS) for a period of over 276 days (with the monitoring of the functional device still ongoing). The full functionality (wireless radio-frequency power, command and signal transmission) and the ability of the electrodes to record artificial neural signals even after 276 days of PBS soaking with little change (within 14%) in signal/noise amplitude constitute a major milestone in long term stability and allow us to study and evaluate the encapsulation reliability, functional stability and its potential usefulness for a wireless neural interface for future chronic implants.",

"abstractNote": "The performance of cathode-anode configurations in a cuff electrode to stimulate a single fascicle in a nerve trunk has been investigated theoretically. A three-dimensional volume conductor model of a nerve trunk with four fascicles in a cuff electrode and a model of myelinated nerve fiber stimulation were used to calculate the recruitment of 15 m fibers in each fascicle. The effect of a monopole, a transverse bipole (anode opposite the cathode), and a narrow transverse tripole (guarded cathode) in selectively stimulating 15 m fibers in each fascicle has been quantified and presented as recruitment curves. It is predicted that selective fascicle stimulation is advanced most by stimulation with a bipole in a plane perpendicular to the axis of the nerve trunk. Monopoles and conventional longitudinal tripoles perform less well, as does a longitudinal tripole with an additional \"steering\" anode. Apart from transverse bipolar stimulation an additional anode may be used to maximally fit the area of excitation to the topography of the fascicle to be recruited. As compared to monopolar and longitudinal tripolar stimulation, the slope of the recruitment curves in transverse bipolar stimulation is reduced considerably, thus allowing improved fine tuning of nerve (and thus force) recruitment. Another advantage of this method is a minimal number of cable connections to the cuff electrode. The cost of the improved selectivity is an increased stimulation current.",

"abstractNote": "Using a self-sizing spiral-cuff electrode placed on the sciatic nerve of the cat, the joint torque evoked with stimulation applied to contacts in a monopolar configuration was judged to be the same as the torque evoked by stimulation applied to contacts in a tripolar configuration. Experiments were carried out in six acute cat preparations. In each experiment, a 12-contact electrode was placed on the sciatic nerve and used to effect both the monopolar and tripolar electrode configurations. The ankle torque produced by electrically evoked isometric muscle contraction was measured in three dimensions: plantar flexion, internal rotation, and inversion. Based on the recorded ankle torque, qualitative and quantitative comparisons were performed to determine if any significant difference existed in the pattern or order in which motor nerve fibers were recruited. No significant difference was found at a 98% confidence interval in either the recruitment properties or the repeatability of the monopolar and tripolar configurations. Further, isolated activation of single fascicles within the sciatic nerve was observed. Once nerve fibers in a fascicle were activated, recruitment of that fascicle was modulated over the full range before \"spill-over\" excitation occurred in neighboring fascicles. These results indicate that a four contact, monopolar nerve-cuff electrode is a viable substitute for a 12 contact, tripolar nerve-cuff electrode. The results of this study are also consistent with the hypothesis that multicontact self-sizing spiral-cuff electrodes can be used in motor prostheses to provide selective control of many muscles. These findings should also apply to other neuroprostheses employing-cuff electrodes on nerve trunks.",

"abstractNote": "Written by renowned authorities in the field, Deep Brain Stimulation in Neurological and Psychiatric Disorders discusses today’s most current and cutting-edge applications of DBS. Initially used to treat Parkinson’s disease and essential tremor, DBS now has new clinical indications, new anatomic targets, and new technologies that enable an expanded role for DBS in the treatment of other movement disorders such as dystonia, and for other neurologic disorders such as epilepsy, Tourette syndrome and cluster headache. Early experience has also been reported for psychiatric syndromes, such as obsessive-compulsive disorder and depression. This important book begins with reviews of the functional anatomy and physiology of motor and nonmotor aspects of the basal ganglia and their connections which underlie the application of DBS to neurological and psychiatric disorders. This is followed by proposed mechanisms of action of DBS based on functional neuroimaging and neurophysiologic studies in animals and man. Discussions of patient selection, preoperative assessment, operative complications and brain targeting are followed by chapters concerning the demands of microelectrode mapping as well as new and emerging brain imaging approaches for target localization inside the operating room. Postoperative management and outcomes are reviewed in a series of chapters concerning immediate and delayed complications. Particularly important chapters on programming, DBS safety with regard to MRI and other electronic devices, neuropsychological sequelae, and quality of life are also included. Finally, new areas of DBS application are addressed by experts with experience in Tourette syndrome, epilepsy, obsessive-compulsive disorder, depression, and cluster headache. Comprehensive and state-of-the-art, Deep Brain Stimulation in Neurological and Psychiatric Disorders provides a balanced and level-headed approach to the use of DBS with emphasis on proper and objective assessment of outcomes, particularly in regard to the new and emerging applications -- all with the main goal of providing patients with some measure of relief from their difficult conditions.",

"abstractNote": "Acute experiments were performed on adult cats to study selective activation of medial gastrocnemius, soleus, tibialis anterior, and extensor digitorum longus with a cuff electrode. A spiral nerve cuff containing twelve \"dot\" electrodes was implanted around the sciatic nerve and evoked muscle twitch forces were recorded in six experiments. Spatially isolated \"dot\" electrodes in four geometries: monopolar, longitudinal tripolar, tripolar with four common anodes, and two parallel tripoles, were combined with transverse field steering current(s) from an anode(s) located 180 degrees around from the cathode(s) to activate different regions of the nerve trunk. To quantify the degree of selectivity, a selectivity index was defined as the ratio of the force in one muscle to the force in all four muscles in response to a particular stimulus. The selectivity index was used to construct recruitment curves for a muscle with the optimal degree of selectivity. Physiological responses were correlated with the anatomical structure of the sciatic nerve by identifying the nerve fascicles innervating the four muscles, and by determining the relative positions of the electrodes and the nerve fascicles. The results indicated that the use of transverse field steering current improved selectivity. We also found that tripoles with individual dot anodes were more selective than tripoles with four common dot anodes. Stimulation with two parallel tripoles was effective in activating selectively fascicles that could not be activated selectively with only a single tripole. The multipolar cuff proved an effective method to control selectively and progressively the force in muscles innervated by fascicles that were well defined at the level of the cuff.",

"extra": "Functional Electrical Stimulation (FES) endeavors to restore the lost functions of the nervous system by means of electrical stimulation. It is a multidisciplinary field on the interface between the neuroscience, engineering, rehabilitation and the clinic. In this review, the principal notions behind FES are outlined together with the relevant moments of its history. The specific research problems encountered in the development of FES applications are extensively covered. From an engineering perspective, special attention is dedicated to the electrodes for selective stimulation. The major clinical applications are overviewed: pacing of the heart, respiratory pacing of the diaphragm, cochlear neuroprostheses for restoration of hearing, the rehabilitation and restoration of locomotion and hand functions, and the restoration of bladder and bowel functions. An analysis of the current FES applications is performed from the perspective of the evidence-based clinical approach. Currently, FES for restoration of the lost locomotor functions is a rapidly developing area that is on the verge of broadening its acceptance in the clinic. Evidence for this is the increasing number of various neuroprosthetic devices, some of them already beyond the prototype stage, that have been developed recently. The present FES treatments combined with conventional occupational and physical therapy still remain the most promising approach in rehabilitation of the spinal cord injury patients and the stroke patients. FES in combination with neuromodulation, which is however still in a rather empirical state of operation, becomes a viable option in the treatment of various urological disorders. The analysis indicates that the pacing of the heart and cochlear neuroprostheses remain the most developed applications of FES. The presented overview of the FES applications shows that a number of fundamental scientific problems have to be solved before a comparable degree of effectiveness and penetration in the common medical practice is achieved for the locomotion neuroprostheses and the urological FES appliances. Finally, a summary is given of the major clinical and fundamental directions of research needed for further improvement of FES. Biomedical Reviews 2003; 14: 23-50.",

"abstractNote": "The peripheral distribution of the fibre content of individual ulnar nerve fascicles supplying skin and muscles of the hand in human volunteers was indirectly mapped by tracing the fields of projected sensation evoked by intraneural electrical microstimulation (INMS) and by tracing receptive fields delineated through intraneural recording of afferent impulse activity elicited by natural stimulation of end organs. Moderate intensity suprathreshold INMS, delivered in cutaneous fascicles, induced nonpainful sensations projecting to stereotyped and coherent areas of skin, the fascicular projected fields (FPFs). Fascicular receptive fields (FRFs) were mapped during microneurographic recording by determining the area of skin which, when activated by light tactile stimuli, elicited afferent neural discharges recorded intraneurally. It was found that at a given electrode position in a skin nerve fascicle, moderate intensity INMS induced nonpainful sensations projecting to a cutaneous field (FPF) that coincides with the FRF, while high intensity INMS induced painful sensations projecting within the cutaneous field of nonpainful sensations. Pain induced by INMS in muscle nerve fascicles was projected to the muscles innervated by that fascicle and, in most instances, to areas beyond the muscular receptive field. The study demonstrates that individual ulnar nerve fascicles, at wrist levels, subserve well-defined cutaneous territories in the hand, and that the area of skin covered by the sum of all ulnar fascicular receptive or projected fields matches the maximal possible cutaneous distribution of the ulnar nerve. Insights of practical relevance regarding clinical expression of fascicular nerve injuries are also brought up by this study.",
