This study is the first to quantify regional brain volume differences in a large sample of youth with diabetes.\nRESEARCH DESIGN AND METHODS—Magnetic resonance images (MRIs) were acquired from youth with diabetes (n = 108) and healthy sibling control subjects (n = 51) aged 7–17 years. History of severe hypoglycemia was assessed by parent interview and included seizure, loss of consciousness, or requiring assistance to treat. A1C values since diagnosis were obtained from medical records; median A1C was weighted by duration of disease. Voxel-based morphometry was used to determine the relationships of prior hypo- and hyperglycemia to regional grey and white matter volumes across the whole brain.\nRESULTS—No significant differences were found between diabetic and healthy control groups in grey or white matter. However, within the diabetic group, a history of severe hypoglycemia was associated with smaller grey matter volume in the left superior temporal region. Greater exposure to hyperglycemia was associated with smaller grey matter volume in the right cuneus and precuneus, smaller white matter volume in a right posterior parietal region, and larger grey matter volume in a right prefrontal region.\nCONCLUSIONS—Qualitatively different relationships were found between hypo- and hyperglycemia and regional brain volumes in youth with type 1 diabetes. Future studies should investigate whether these differences relate to cognitive function and how these regions are affected by further exposure.", "publicationTitle": "Diabetes Care", "publisher": "", It is hypothesized that (1) social anxiety will be negatively associated with adherence and quality of life and (2) that fear of hypoglycemia will mediate this relationship. It is also hypothesized that (3) girls will have higher social anxiety than boys.\nMethods\nAdolescents with type 1 diabetes were recruited during clinic visits at 2 international centers. Participants answered a survey containing questionnaires on social anxiety, behavioral adherence to the diabetes self-care regimen, quality of life, fear of hypoglycemia, and last hemoglobin A1C results.\nResults\nSeventy-six adolescents (33 boys, 43 girls), mean age 15.9 (1.44) years, participated. Social anxiety levels are not statistically different between genders. In boys, social anxiety is associated with worse diet and insulin injection adherence; no associations are found in girls. Social anxiety is positively correlated with poor quality of life in both genders. Fear of hypoglycemia mediates the relationship between social anxiety and insulin adherence in boys.\nConclusions\nFindings suggest that social anxiety, which is common in general populations of adolescents, may interfere with behavioral adherence and quality of life among adolescents with type 1 diabetes. diabetes generally have poor glycaemic control. Managing type 1 diabetes in young people is complex, and is underpinned by relationships with significant others in the social environments they inhabit. This qualitative study explores the social environments of young people with type 1 diabetes and their potential influence on glycaemic control. Twenty young people with type 1 diabetes and their parents (n=27) were interviewed about their experiences in the environments of the home, with friends (social), at school and in the diabetes clinic. It was found that the diabetes clinic was vital to the medical management of type 1 diabetes, and the family provided stable support for most young people with type 1 diabetes. However, there were barriers to self-management in school and social environments.
