In the current study of college women (N = 345, 79% Caucasian; mean age = 19.69 years, SD = 2.55), we examined whether institutional failure to prevent sexual assault or respond supportively when it occurs may similarly exacerbate posttraumatic symptomatology-what we call \"institutional betrayal.\" Almost half (47%) of the women reported at least one coercive sexual experience and another 21% reported no coercion, but at least one unwanted sexual experience (total reporting unwanted sexual experiences, N = 233). Institutional betrayal (e.g., creating an environment where these experiences seemed more likely, making it difficult to report these experiences) was reported across different unwanted sexual experiences (47% and 45% of women reporting coercion and no coercion, respectively). Those women who reported institutional betrayal surrounding their unwanted sexual experience reported increased levels of anxiety (R(2) = .10), trauma-specific sexual symptoms (R(2) = .17), dissociation (R(2) = .11), and problematic sexual functioning (R(2) = .12). Data are from a study of college life at a large midwestern university involving nine months of ethnographic observation of a women's floor in a \"party dorm,\" in-depth interviews with 42 of the floor residents, and 16 group interviews with other students. We show that sexual assault is a predictable outcome of a synergistic intersection of processes operating at individual, organizational, and interactional levels. Some processes are explicitly gendered, while others appear to be gender neutral. We discuss student homogeneity, expectations that partiers drink heavily and trust their party-mates, and residential arrangements. We explain how these factors intersect with more obviously gendered processes such as gender differences in sexual agendas, fraternity control of parties, and expectations that women be nice and defer to men. We show that partying produces fun as well as sexual assault, generating student resistance to criticizing the party scene or men's behavior in it. girls in an urban slum in Nairobi, Kenya.\nMETHODS: Population-based survey of 522 high school girls in the Korogocho-Kariobangi locations in Nairobi, Kenya, at baseline and 10 months later. Subjects were assigned by school attended to either a \"No Means No Worldwide\" self-defense course (eight schools; N = 402) or to a life-skills class (two schools; N = 120). Both the intervention and the life-skills classes were taught in the schools by trained instructors. Participants were administered the same survey at baseline and follow-up.\nRESULTS: A total of 522 girls (mean age, 16.7 ± 1.5 years; range, 14-21 years) completed surveys at baseline, and 489 at 10-month follow-up. At baseline, 24.5% reported sexual assault in the prior year, with the majority (90%) reporting assault by someone known to them (boyfriend, 52%; relative, 17%; neighbor, 15%; teacher or pastor, 6%). In the self-defense intervention group, the incidence of sexual assault decreased from 24.6% at baseline to 9.2% at follow-up (p < .001), in contrast to the control group, in which the incidence remained unchanged (24.2% at baseline and 23.1% at follow-up; p = .10). Over half the girls in the intervention group reported having used the self-defense skills to avert sexual assault in the year after the training. Rates of disclosure increased in the intervention group, but not in controls.\nCONCLUSIONS: A standardized 6-week self-defense program is effective in reducing the incidence of sexual assault in health problems worldwide. In parts of sub-Saharan Africa, sexual assault has reached epidemic proportions. This study evaluated the efficacy of an empowerment and self-defense intervention for adolescent girls to decrease the incidence of sexual assault and harassment in Nairobi’s large informal settlements.\nMETHODS: A prospective cohort of 1978 adolescents from 4 neighborhoods near Nairobi were taught empowerment, deescalation, and self-defense skills in six 2-hour sessions. The standard-of-care (SOC) group (n = 428) received a life skills class. Self-reported, anonymous survey data were collected at baseline and 10.5 months after intervention.\nRESULTS: Annual sexual assault rates decreased from 17.9/100 person-years at baseline to 11.1 at follow-up (rate ratio = 1.61; 95% confidence interval [CI], 1.26–1.86; P < .001); there was no significant change in the SOC group (14.3 to 14.0, rate ratio = 1.02; 95% CI, 0.67–1.57, P = .92). Sexual assault disclosure in the intervention group increased from 56% to 75% (P = .006), compared with a constant incidence of disclosure (53%) in the SOC group. The majority (52.3%) of adolescents in the intervention group reported using skills learned to stop an assault.\nCONCLUSIONS: This intervention decreased sexual assault rates among adolescent girls in Kenya. The intervention was also associated with an increase in the disclosure of assaults, thereby enabling survivors to seek care and support and possibly leading to the identification and prosecution of perpetrators. Program group women significantly increased their protective behaviors over the 6-month follow-up period compared to the waiting-list control group. Although past research has identified fraternities and athletic teams as high-risk groups for sexual assault, the findings have been inconclusive. Based on student perceptions, we separated fraternities and athletic teams into high-risk and low-risk groups. A survey of 182 male students, including those who were members of three fraternities and five athletic teams as well as nonmembers, indicated that the high-risk groups scored significantly higher than the low-risk groups on measures of sexual aggression, hostility toward women, and male peer support endorsing sexual aggression. These findings showed that some fraternity or athletic team members are more likely to commit sexual assault than males in the general student population, but this is not true for all such groups.
