Methods Prospective data from N=225 consecutively admitted first time valve replacement patients was assessed before and one year after surgery. Patients were asked about their illness perceptions (Illness Perception Questionnaire-Revised) and mood state (Hospital Anxiety and Depression Scale). Health status was defined by quality of life (Short-Form 36) and New York Heart Association (NYHA) class. Cluster analyses were conducted to identify illness perception profiles over time. Predictors of health status after surgery were analyzed with multivariate methods. Results Patients were grouped according to the stability and nature (positive, negative) of their illness perception profile over one year. One year after surgery patients holding a negative illness perception profile showed a lower physical quality of life and were diagnosed in a higher New York Heart Association class than patients changing to positive and patients with stable positive illness perceptions (P<.001). Over and above biological determinants, post-surgery physical quality of life and {NYHA} class were both predicted by pre-surgery illness perception profiles (P<.05). Conclusion Patients going for heart valve replacement surgery can be easily categorized into illness perception profiles that predict health status one year after surgery. Methods All patients admitted to the intensive care unit and expected to be ventilated for at least 72 hours were included and underwent weekly electromyograms and {HRV} analyses for three weeks. {HRV} was assessed by time domain analysis of 24 h recording electrocardiograms, and alterations in {HRV} were assessed as the square root of the mean squared differences of successive {RR} intervals (RMSSD) ≤ 15. Results We evaluated 26 patients, 12 men and 14 women, median age 64 years. During follow-up, 12 patients died and 9 developed CIP. {CIP} was not associated with age, sex, simplified acute physiology score {II} and treatment agents. Altered {RMSSD} tended to be associated with onset of {CIP} (P = 0.06). Altered {RMSSD} occurred earlier or at the same time as electromyogram abnormalities in all {CIP} patients, but the difference was not significant. Conclusion Altered HRV, may be associated with the onset of {CIP} in {ICU} patients. Although not statistically significant (P = 0.06), altered {RMSSD} may be a surrogate marker of {CIP} in {ICU} patients undergoing mechanical ventilation. Methods Key databases were systematically searched (CINAHL, Medline, PsycINFO, Scopus and Web of Science) for studies matching the inclusion criteria between November 2011 and February 2012. References of included studies were examined and key authors contacted. Studies were subject to a quality control check. Results 21 studies met the inclusion criteria. A synthesis of the results found that illness perceptions were correlated to and predicted QoL and mood across {CHD} diagnoses. Specific illness perceptions (control, coherence and timeline) were found to be important for patients that had experienced an unexpected medical event, such as myocardial infarction. Conclusion The results of this study provide support that illness perceptions are related to outcomes across {CHD} populations and disease progression, however the results do not selectively support one particular model. Recommendations are consistent with cardiac rehabilitation guidelines. Results Symptoms of depression and anxiety were positively associated with anger-in (P < .001) and alexithymia (P < .01), and were negatively correlated with emotional expression (P = .05). Furthermore, illness knowledge moderated the relationship between anger-in and depressive symptoms (P = .01), such that high anger-in individuals with greater illness knowledge displayed greater depressive symptoms. Knowledge also moderated the relationship between emotional expression and anxiety (P = .02), indicating that low emotional expression was associated with greater anxiety among those with less illness knowledge. Depressive and anxiety symptoms, anger-in, alexithymia, and emotional expression were not correlated with physical QoL. El enfoque del tratamiento es diferente si el dolor es agudo o crónico e implica conocer el mecanismo fisiopatológico responsable del mismo. El tratamiento farmacológico se ha de iniciar con la dosis mínima efectiva y titular lentamente, evitando la polifarmacia. Se enfatiza el control de los efectos adversos, especialmente la sedación y el estreñimiento que predisponen el desarrollo de la encefalopatía hepática. El paracetamol es el primer eslabón terapéutico siendo seguro a dosis 2-3 g/día. Los antiinflamatorios no esteroideos están contraindicados ya que pueden inducir insuficiencia renal aguda y/o sangrado gastrointestinal. El tramadol es una opción segura en el tratamiento del dolor moderado a severo. Los opioides con mayor seguridad terapéutica son el fentanilo, la hidromorfona y la metadona como segunda opción. El tratamiento tópico puede disminuir el consumo de fármacos por vía oral. En el tratamiento del dolor neuropático la gabapentina es la primera opción terapéutica, mientras que los antidepresivos tricíclicos pueden estar indicados en cierto grupo de pacientes. Las técnicas intervencionistas son una herramienta valiosa a utilizar en el dolor moderado a severo ya que permiten disminuir el tratamiento farmacológico y con ello los efectos adversos. Las intervenciones psicológicas, la terapia física y la rehabilitación deben ser consideradas como parte de la terapia multimodal en el abordaje del dolor crónico. Abstract Pain management in patients with liver cirrhosis is a real challenge and is often inadequate due to a lack of therapeutic efficacy or the high incidence of adverse effects. The focus of treatment differs depending on whether the pain is acute or chronic and involves understanding the causative pathophysiological mechanism. Analgesics should be started with the minimum effective dose and should be titrated slowly with avoidance of polypharmacy. Adverse effects must be monitored, especially sedation and constipation, which predispose the patient to the development of hepatic encephalopathy. The first-line drug is paracetamol, which is safe at doses of 2-3 g/day. Non-steroidal anti-inflammatory agents are contraindicated because they can cause acute renal failure and/or gastrointestinal bleeding. Tramadol is a safe option for moderate-severe pain. The opioids with the best safety profile are fentanyl and hydromorphone, with methadone as an alternative. Topical treatment can reduce oral drug consumption. In neuropathic pain the first-line therapeutic option is gabapentin. The use of antidepressants such as amitriptyline can be considered in some patients. Interventional techniques are a valuable tool in moderate to severe pain, since they allow a reduction in drug therapy and consequently its adverse effects. Psychological treatment, physical therapy and rehabilitation should be considered as part of multimodality therapy in the management of chronic pain.", "publicationTitle": "Gastroenterología y The challenge for clinicians is to know how best to manage the symptoms of weight loss and anorexia for optimal patient outcomes. Different treatments have been used in this syndrome. La agenesia vaginal puede ser causa de retención de sangre y secreciones, con la consiguiente formación de un hematómetra, que produciría una gran masa pélvica y dolor intenso; y puede ser necesario realizar su drenaje y utilizar un catéter de Foley, para evitar posibles reestenosis con recidiva del hematómetra. Describimos el caso de una adolescente con agenesia de los 2/3 superiores de la vagina, sin otras malformaciones asociadas, a la cual fue necesario realizarle drenaje de un hematómetra por los intensos dolores que le causaba.\n\nRokitansky syndrome is characterized by primary amenorrhea in female phenotypic patients and the absence of vagina, as well as other associated malformations of the genital, urinary, skeletal or cardiac system, with functioning ovaries. Vaginal agenesis may result in blood and secretion retention, with the subsequent formation of a hematometra producing a large pelvic mass and acute pain. The mass requires drainage with a Foley catheter to prevent possible restenosis and hematometra recurrence. La toma de decisiones al final de la vida es compleja y se ve dificultada por la confusión en el empleo incorrecto de la palabra eutanasia. Se definen eutanasia, suicidio asistido, limitación de esfuerzo terapéutico, rechazo de tratamiento y sedación paliativa con intención de clarificar conceptualmente estas situaciones diferentes y poder hacer una aproximación más acertada a las decisiones conflictivas.\n\nBioethics helps to resolve ethical problems raised by clinical practice. Knowledge of this discipline is required by personnel in all specialities. End-of-life decision-making is complex and is further hampered by confusion surrounding the incorrect use of the term euthanasia. Las más frecuentes son la infección urinaria, la neumopatía, la bacteriemia/septicemia y la infección de la zona quirúrgica. Estas infecciones nosocomiales se ven favorecidas por la presencia de dispositivos o procedimientos invasivos. Las bacterias representan las tres cuartas partes de los microorganismos aislados en estas infecciones nosocomiales. Las más habituales son Escherichia coli, Staphylococcus aureus y Pseudomonas aeruginosa. Algunas de estas bacterias plantean el problema grave de la multirresistencia a los antibióticos (por ejemplo, S. aureus resistente a la meticilina). La prevención de las infecciones nosocomiales se basa en un mejor respeto de las precauciones de higiene, la aplicación de protocolos de antibioticoterapia y la instauración de programas de vigilancia.
