This study offers quantitative tools for on-line assessment of the quality of this optimization, based on self-monitoring of blood glucose (SMBG). Ninety-six adults with Insulin Dependent Diabetes Mellitus (IDDM), age 35 ± 8 yrs., duration of diabetes 16 ± 10 yrs., HbAlc 8.6 ± 1.8%, 43 of whom had a recent history of severe hypoglycemia (SH), while 53 did not, used Lifescan One Touch II meters for 135 ± 53 SMBG readings over a month. For the following six months the subjects recorded occurrence of SH. The two patient groups, with and without a history of SH, did not differ in age, duration of diabetes, HbAlc, insulin units/day, average BG or BG variability. We suggest a computational procedure based on a symmetrization of the BG measurement scale and on a superimposed BG risk function, that allows for computation of two glycemic control markers: the Low BG Index (LBGI) and the High BG Index (HBGI). The LBGI is associated with SH: the LBGI and the rate of change of the BG risk, classified correctly 77% of the subjects with vs. without a history of SH and accounted for 46% of the variance of future SH. The HBGI, in combination with age, duration of diabetes and daily insulin dose, accounted for 57% of the variance of patients' glycosylated hemoglobin. The proposed method also suggests the use of linear type response surface functions in conjunction with Rackwitz-Fiessler algorithm rather than quadratic functions, for which advanced Monte Carlo algorithms are commonly used to evaluate the reliability estimates. The proof-of-concept Leuven studies assessed causality, and revealed that targeting strict normoglycemia (80-110 mg/dL) with insulin improved outcome compared with tolerating hyperglycemia to the renal threshold (215 mg/dL). A large multicenter trial (NICE-SUGAR [Normoglycaemia in Intensive Care Evaluation and Survival Using Glucose Algorithm Regulation]) found an intermediate blood glucose target (140-180 mg/dL) safer than targeting normoglycemia. Differences in design and in execution of glycemic control at the bedside may have contributed to these results. In NICE-SUGAR (Normoglycaemia in Intensive Care Evaluation and Survival Using Glucose Algorithm Regulation), the blood-glucose target range in the control group was lower, there were problems to reach and maintain normoglycemia in the intervention group, and inaccurate handheld blood glucose meters and variable blood sampling sites were allowed. Inaccurate tools led to insulin-dosing errors with consequently (undetected) hypoglycemia and unacceptable blood glucose variability. Also, the studies were done superimposed upon different nutritional strategies. Specific criteria related to the entry and display of data elements were developed to evaluate the functionality of each PHR. Information abstracted from an actual case was used to create a series of representative PHRs. Output generated for review was evaluated to assess the accuracy and completeness of clinical information related to the diagnosis and treatment of specific disorders.\nResults The PHRs selected for review employed data entry methods that limited the range and content of patient-entered information related to medical history, medications, laboratory tests, diagnostic studies, and immunizations. Representative PHRs created with information abstracted from an actual case displayed varying amounts of information at basic and comprehensive levels of representation.\nConclusions Currently available PHRs demonstrate limited functionality. Although large integrated health systems have promoted use through costly advertising campaigns, other implementation methods are needed for small to medium-sized practices where most patients receive their care. METHODS We conducted a mixed methods assessment of a proactive implementation strategy for a patient portal (an interactive preventive health record [IPHR]) offered by 8 primary care practices. The practices implemented a series of learning collaboratives with practice champions and redesigned workflow to integrate portal use into care. Practice implementation strategies, portal use, and factors influencing use were assessed prospectively. RESULTS A proactive and customized implementation strategy designed by practices resulted in 25.6% of patients using the IPHR, with the rate increasing 1.0% per month over 31 months. Fully 23.5% of IPHR users signed up within 1 day of their office visit. Older patients and patients with comorbidities were more likely to use the IPHR, but blacks and Hispanics were less likely. Older age diminished as a factor after adjusting for comorbidities. Implementation by practice varied considerably (from 22.1% to 27.9%, P <.001) based on clinician characteristics and workflow innovations adopted by practices to enhance uptake.
