The authors reviewed 22 articles derived from experimental or quasi-experimental studies.\nOUTCOME MEASURES: Type 2 diabetes behavioral and metabolic outcomes, breastfeeding, nutrition knowledge, attitudes and behaviors.\nRESULTS: Peer nutrition education has a positive influence on diabetes self-management and breastfeeding outcomes, as well as on general nutrition knowledge and dietary intake behaviors among Latinos.\nCONCLUSIONS AND IMPLICATIONS: There is a need for longitudinal randomized trials testing the impact of peer nutrition education interventions grounded on goal setting and culturally appropriate behavioral change theories. Inclusion of reliable scales and the construct of acculturation are needed to further advance knowledge in this promising field. Articles from indexed journals related to infant feeding decision making published from 1990 to 2011 were reviewed. Fourteen qualitative studies were identified, and all were included in the metasynthesis.\nDATA COLLECTION/ANALYSIS: Findings were extracted from individual studies and organized in tables to place them in juxtaposition with each other. Comparisons were then made across studies and metaphors written to express a new, broader interpretation.\nRESULTS: Two processes emerged as central to maternal infant feeding decision making: making a personal choice and defending the choice. It is important to understand the beliefs and experiences that impact on migrant and refugee women's infant feeding decisions in order to appropriately support women to breastfeed in a new country. The aim of this paper is to report the findings of a meta-ethnographic study that explored migrant and refugee women's experiences and practices related to breastfeeding in a new country.\nMETHODS: CINAHL, MEDLINE, PubMed, SCOPUS and the Cochrane Library with Full Text databases were searched for the period January 2000 to May 2012. Out of 2355 papers retrieved 11 met the inclusion criteria. A meta-ethnographic synthesis was undertaken using the analytic strategies and theme synthesis techniques of reciprocal translation and refutational investigation. Quality appraisal was undertaken using the Critical Appraisal Skills Programme (CASP) tool.\nRESULTS: Eight qualitative studies and three studies reporting both qualitative and quantitative data were included and one overarching theme emerged: 'Breastfeeding in a new country: facing contradictions and conflict'. This theme comprised four sub-themes 'Mother's milk is best'; 'Contradictions and conflict in breastfeeding practices'; 'Producing breast milk requires energy and good health'; and 'The dominant role of female relatives'. Migrant women who valued, but did not have access to, traditional postpartum practices, were more likely to cease breastfeeding. Women reported a clash between their individual beliefs and practices and the dominant practices in the new country, and also a tension with family members either in the country of origin or in the new country.\nCONCLUSION: Migrant women experience tensions in their breastfeeding experience and require support from professionals who can sensitively address their individual needs. J.", "lastName": "Renfrew" } ], "abstractNote": "BACKGROUND: Despite the widely documented health benefits of breastfeeding, initiation rates remain relatively low in many high-income countries, particularly among women in lower income groups.\nOBJECTIVES: To evaluate the effectiveness of interventions which aim to encourage women to breastfeed in terms of changes in the number of women who start to breastfeed.\nSEARCH STRATEGY: We searched the Cochrane Pregnancy and Childbirth Group trials register (30 May 2004), the Cochrane Central Register of Controlled Trials (The Cochrane Library, Issue 1, 2003) and the following databases from inception to October 2002: MEDLINE, CINAHL, ERIC, Applied Social Sciences, PsychLIT, EMBASE, British Nursing Index, BIDS, EPI-centre. We also searched the following in October 2002 for 'grey literature: 'SIGLE, DHSS Data, and Dissertation Abstracts. We handsearched the Journal of Human Lactation, Health Promotion International and Health Education Quarterly from inception to October 1998. We scanned reference lists of all articles obtained.\nSELECTION CRITERIA: Randomised controlled trials, with or without blinding, of any breastfeeding promotion intervention in any population group except women and infants with a specific health problem.\nDATA COLLECTION AND ANALYSIS: One review author independently extracted data and assessed trial quality for checking by a second author. We contacted investigators to obtain missing information.\nMAIN RESULTS: Seven trials involving 1388 women were included. The author used Noblit and Hare's (1988) method to synthesize 15 qualitative breastfeeding studies. Synthesized studies revealed that breastfeeding is an \"engrossing, personal journey,\" which is very physical and requires maternal commitment, adaptation, and support from multiple sources. Breastfeeding was also shown to have significant personal impact on mothers and to require time for resolution on discontinuing. These findings suggest that health care practitioners can provide the most meaningful breastfeeding support by acknowledging each mother's individual breastfeeding capacity, goals, comfort level with her own body, support network, tolerance of breastfeeding difficulties, and willingness to make the life adaptations that breastfeeding requires. The aim of this metasynthesis was to examine women's perceptions and experiences of breastfeeding support, either professional or peer, to illuminate the components of support that they deemed \"supportive.\"\nMETHODS: The metasynthesis included studies of both formal or \"created\" peer and professional support for breastfeeding women but excluded studies of family or informal support. Qualitative studies were included as well as large-scale surveys if they reported the analysis of qualitative data gathered through open-ended responses. Primiparas and multiparas who initiated breastfeeding were included. Studies published in English, in peer-reviewed journals, and undertaken between January 1990 and December 2007 were included. After assessment for relevance and quality, 31 studies were included. Meta-ethnographic methods were used to identify categories and themes.\nRESULTS: The metasynthesis resulted in four categories comprising 20 themes. The synthesis indicated that support for breastfeeding occurred along a continuum from authentic presence at one end, perceived as effective support, to disconnected encounters at the other, perceived as ineffective or even discouraging and counterproductive. A facilitative approach versus a reductionist approach was identified as contrasting styles of support that women experienced as helpful or unhelpful.\nCONCLUSIONS: The findings emphasize the importance of person-centered communication skills and of relationships in supporting a woman to breastfeed. Meta-analysis and metaregression techniques were used to produce pooled relative risks and investigate sources of heterogeneity in the estimates.\nRESULTS: Eleven randomized controlled trials conducted at 13 study sites met the inclusion criteria for systematic review. We noted significant differences in study populations, peer counselor training methods, peer visit schedule, and outcome ascertainment methods. Peer support significantly decreased the risk of discontinuing EBF as compared to control (RR: 0.71; 95% CI: 0.61-0.82; I(2) =92%). The effect of peer support was significantly reduced in settings with >10% community prevalence of formula feeding as compared to settings with <10% prevalence (p=0.048). There was no evidence of effect modification by inclusion of low birth weight infants (p=0.367) and no difference in the effect of peer support on EBF at 4 versus 6 months postpartum (p=0.398).\nCONCLUSIONS: Peer support increases the duration of EBF in LMICs; however, the effect appears to be reduced in formula feeding cultures. Fourteen articles published in English, Portuguese, and Spanish between 1989 and 2009 were selected.\nRESULTS: The synthesis revealed five categories concerning family practices related to the maintenance of breast-feeding: 1) emotional support, which involves welcoming the mother and the baby, valuing and encouraging breast-feeding, and emphasizing the value of breast-feeding; 2) instrumental support, which covers attending prenatal consultations and home visits, participating in baby care, and providing help in everyday tasks beyond the first few weeks postpartum; 3) informational support, which involves stating the wish to be involved in breast-feeding and encouraging the mother, but not forcing her to share experiences; 4) presence support, which involves being close to the mother and taking the time to listen to her; and 5) self-support, which involves maintaining positive expectations about breast-feeding.\nCONCLUSIONS: The results show that practices defined as support contribute to the maintenance of breast-feeding for longer periods.
