            "abstractNote": "Background: The aim of this study was to determine the patterns and determinants of service use in severely mentally ill persons drawn from the National Institute of Mental Health Epidemiological Catchment Area (ECA) program, a community-based epidemiologic survey. This information provides a baseline against which to track ongoing changes in the US mental health service system. Methods: Severe mental illness (SMI) was defined according to US Senate Appropriations Committee guidelines. Comparisons were made with persons who had a mental disorder that did not meet these criteria (non-SMI). Sociodemographic factors, and 1-year volume and intensity of mental or addictive services use were determined. Differences between those who used services and those who did not were examined using logistic regression. Results: Persons with SMI differed from persons with non-SMI in most sociodemographic characteristics. A higher proportion of persons with SMI used ambulatory services, but the mean number of visits per person did not differ from the non-SMI population. Persons with SMI comprised the bulk of hospital inpatients admitted during a 1-year period. Several significant sociodemographic determinants of service use were found, with different patterns for general medical and specialty service use, pointing out potential barriers to care. Conclusions: As health care reform measures continue to be debated, attention to the service needs of the severely mentally ill is of crucial importance. Pre-managed care (pre-1993) baseline service use benchmarks will be essential to assess the impact of managed care on access to care, particularly for the severely mentally ill. Periodic collection of epidemiologic data on prevalence and service use would thus greatly facilitate service planning and addressing barriers to receiving mental health services in this population.",

            "abstractNote": "BACKGROUND: Anatomic injury severity scores can be grouped into two classes; consensus-derived and data-derived. The former, including the Injury Severity Score (ISS), the New Injury Severity Score (NISS), and the Anatomic Profile Score (APS), are based on the severity score of the Abbreviated Injury Scale (AIS), assigned by clinical experts. The latter, including the International Classification of Disease Injury Severity Score (ICISS) and the Trauma Registry Abbreviated Injury Scale Score (TRAIS) are based on survival probabilities calculated in large trauma databases. We aimed to compare the predictive accuracy of consensus-derived and data-derived severity scores when considered alone and in combination with age and physiologic status.\nMETHODS: Analyses were based on 25,111 patients from the trauma registries of the four Level I trauma centers in the province of Quebec, Canada, abstracted between April 1998 and March 2005. The predictive validity of each severity score was evaluated in logistic regression models predicting hospital mortality using measures of discrimination (Area Under the Receiver Operating Characteristics curve [AUC]) and calibration (Hosmer-Lemeshow statistic [HL]).\nRESULTS: Data-derived scores had consistently better predictive accuracy than consensus-derived scores in univariate models (p < 0.0001) but very little difference between scores was observed in models including information on age and physiologic status. The difference in AUC between the least accurate severity score (ISS) and the most accurate severity score (TRAIS) was 15% in anatomic-only models but fell to 2% in models including age and physiologic status.\nCONCLUSIONS: Data-derived scores provide more accurate mortality prediction than consensus-derived scores do when only anatomic injury severity is considered but offer little advantage if age and physiologic status are taken into account. This may be because of the fact that data-derived scores are not an independent measure of anatomic injury severity.",

            "abstractNote": "Context  \n Data from facility-level surveys indicate that US inpatient psychiatric admissions rose in 2004, from their trough in 1998 to 2000, mainly in acute care settings. Patient-level factors, including age, admission type, diagnoses, length of stay, and payment source, are vital to understanding hospitalization trends.Objective  \n To evaluate trends in acute care hospitalizations for primary psychiatric diagnoses between 1996 and 2007 in relation to patient-level variables.Design, Setting, and Participants  \n The yearly National Hospital Discharge Survey furnished demographic, clinical, and payment data on a probability sample of discharges from short-stay facilities (mean [SD], 448.33 [19.66]), along with weights for extrapolation to population estimates. Discharges with a primary psychiatric diagnosis (mean [SD], 19 535 [2615]) were identified among children (aged 5-13 years), adolescents (aged 14-19 years), adults (aged 20-64 years), and elderly individuals (≥65 years).Main Outcome Measures  \n Annual rates of discharges and total days of inpatient care associated with primary psychiatric diagnoses for each age group.Results  \n Psychiatric discharges increased for children from 155.54 per 100 000 children in 1996 to 283.04 per 100 000 in 2007 (P = .003); for adolescents, from 683.60 to 969.03 per 100 000 (P = . 001); and for adults, from 921.35 to 995.51 per 100 000 (P = .003) but declined for elderly individuals from 977.63 to 807.55 per 100 000 (P < .001). Total inpatient days increased for children (1845 days per 100 000 in 1996 to 4370 days in 2007; P = .02) and for adolescents (5882 days per 100 000 in 1996 to 8247 days in 2007; P < .001) but decreased for elderly patients (10 348 days per 100 000 in 1996 to 6517 days; P < .001). The proportion of inpatient days paid by private sources declined among children (36%-21%), adolescents (52%-22%), and adults (35%-23%; all P < .001).Conclusions  \n Inpatient discharges in short-stay facilities with a primary psychiatric diagnosis rose between 1996 and 2007, most dramatically for youth, but decreased among elderly individuals. Private funding bore a declining share of costs.",

            "abstractNote": "Context  \n    Little is known about lifetime prevalence or age of onset of DSM-IV disorders.Objective  \n    To estimate lifetime prevalence and age-of-onset distributions of DSM-IV disorders in the recently completed National Comorbidity Survey Replication.Design and Setting  \n    Nationally representative face-to-face household survey conducted between February 2001 and April 2003 using the fully structured World Health Organization World Mental Health Survey version of the Composite International Diagnostic Interview.Participants  \n    Nine thousand two hundred eighty-two English-speaking respondents aged 18 years and older.Main Outcome Measures  \n    Lifetime DSM-IV anxiety, mood, impulse-control, and substance use disorders.Results  \n    Lifetime prevalence estimates are as follows: anxiety disorders, 28.8%; mood disorders, 20.8%; impulse-control disorders, 24.8%; substance use disorders, 14.6%; any disorder, 46.4%. Median age of onset is much earlier for anxiety (11 years) and impulse-control (11 years) disorders than for substance use (20 years) and mood (30 years) disorders. Half of all lifetime cases start by age 14 years and three fourths by age 24 years. Later onsets are mostly of comorbid conditions, with estimated lifetime risk of any disorder at age 75 years (50.8%) only slightly higher than observed lifetime prevalence (46.4%). Lifetime prevalence estimates are higher in recent cohorts than in earlier cohorts and have fairly stable intercohort differences across the life course that vary in substantively plausible ways among sociodemographic subgroups.Conclusions  \n    About half of Americans will meet the criteria for a DSM-IV disorder sometime in their life, with first onset usually in childhood or adolescence. Interventions aimed at prevention or early treatment need to focus on youth.",

            "abstractNote": "INTRODUCTION: Patients with orthopaedic trauma experience substantial psychological and physical morbidities. The purpose of this study was to assess depressive symptoms in patients with a distal radius fracture, and to determine whether early use of the wrist after volar plating reduces depressive symptoms as compared with cast immobilisation during the early recovery period.\nMATERIALS AND METHODS: Twenty-six patients with a distal radius fracture, who underwent volar plating and were allowed immediate use of the wrist, and 24 patients treated by cast immobilisation for 6 weeks were prospectively compared with respect to depressive symptoms at week 0, and at 2, 6, 12 and 24 weeks after injury, using the Center for Epidemiologic Studies Depression Scale (CES-D). Physical morbidity was assessed using the Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaire and a pain Numerical Rating Scale (NRS). Multivariate analysis was performed to identify factors that independently predicted CES-D scores at each time point.\nRESULTS: No differences in the CES-D scores were found between the volar plating and the cast immobilisation groups, although volar plating group had marginally better CES-D scores at 24 weeks. Multivariate analysis indicated that CES-D scores at each time were independently associated with pain NRS scores at 0 and 24 weeks, and DASH scores at 6 weeks.\nCONCLUSION: Early use of the wrist after volar plating was not found to reduce depressive symptoms as compared with cast immobilisation in the early treatment period following a distal radius fracture. Pain was found to be an important predictor of depression, suggesting that caution is needed to address pain during the early rehabilitation period.",

            "abstractNote": "A large body of literature advocates exercise as a successful intervention for increasing positive affect while also reducing negative affect and anxiety. Questions concerning the mechanisms driving these effects remain unanswered, particularly considering theorized attentional adaptations that may be elicited by acute exercise bouts. We investigated pre- and post-exercise attentional bias to examine possible attentional explanations that may account for these reported changes in affect. On separate visits to the laboratory, 30 high trait anxious participants completed 30 min of exercise on a cycle ergometer at 70% of their heart rate reserve, or completed a 30-min quiet rest protocol. During each intervention, pre-test and post-test modified dot-probe assessments of attentional bias were completed, as were a series of self-report anxiety and affect questionnaires. Attentional bias scores and reaction times were calculated. Post-exercise dot probe performance did not vary significantly as a function of the affective valence of presented stimuli. As hypothesized, however, positive affect and reaction time improved significantly following exercise compared with the pre- and post-rest conditions and the pre-exercise condition, suggesting that exercise facilitates a broadening of attentional scope. Implications of these findings and future directions are discussed within the context of traditional and contemporary theories of dispositional affect and state-specific emotional responses.",

            "abstractNote": "Few studies have explored psychosocial factors affecting depression, anxiety and quality of life (QOL) among adults with external fixation devices (EFDs). This cross-sectional study investigated whether maladaptive and adaptive coping and locus of control predicted these outcomes. Forty-seven participants, recruited from online support groups, completed an 86-item online questionnaire. Maladaptive coping e.g. denial, self-blame, substance-abuse and behavioural disengagement significantly predicted high anxiety and depression and lower psychological and social QOL. Adaptive coping e.g. active-coping and acceptance predicted higher psychological QOL. Results emphasise the importance of supporting EFD patients in developing adaptive coping strategies during, but also after, fixation.",
