Construction plant and equipment harbour significant health and safety hazards. One particular item, the mini-excavator, presents a hazard from its inherent instability and tendency to overturn during use. The purpose of this paper is to investigate turnover incidents to observe prominent factors and contribute to development of best practice guidance for improving health and safety relating to mini-excavator use. Design/methodology/approach - Comprehensive documentary data from eight case study incidents, along with anecdotal data from a further three, were qualitatively analysed in terms of: machine weight, machine activity, ground type/topography, operator competence, type of overturn, damage to property, and injury to person(s). Results were presented to experts in the field for comment and conclusions/recommendations accordingly developed. Findings - Observed prominent casual factors include: inadequate assessment of risk and non-adherence to safe working practice, working on poor ground, and working on inclined surfaces. The use of sealed operator cabs and proprietary seat restraint mechanisms seem to mitigate risk of personal injury to operators during overturn incidents. A need for cyclic training to reiterate good operator and banksman practice is emphasised, while construction managers should also be aware of the risks, and help implement risk controls. Research limitations/implications - Findings will inform construction plant management research generally, and the subject of mechanised workplace transport stability specifically. Practical implications - Best practice protocol will inform health and safety management of mini-excavators at the workplace. However, many problems occur with this approach in practice, including higher risk levels and cost or time overruns. In order to solve such problems, it is proposed that the IKEA model of the manufacturing industry and VP technology are introduced into a prefabricated construction process. The concept of the IKEA model is identified in detail and VP technology is briefly introduced. In conjunction with VP technology, the applications of the IKEA model are presented in detail, i.e. design optimization, production optimization and installation optimization. Furthermore, through a case study of a prefabricated hotel project in Hong Kong, it is shown that the VP-based IKEA model can improve the efficiency and safety of prefabricated construction as well as that results from this form of research are applicable only to the case studied and cannot be generalized further. In the field of construction engineering and management (CEM), the number of papers employing case-study research methods over the past decade has increased substantially. As the method proliferates, the question arises: are CEM case studies being performed with sufficient validity and reliability to meet the burden of proof to generalize from the case-study findings? Meeting the burden of proof is particularly critical in CEM because of the unique, site-based nature of the projects and industry. This paper presents a review of 156 papers employing case studies published in the Journal of Construction Engineering and Management, of which the writers identified 33 theory-building case-study-method papers. This subset of theory-building papers was examined to induce a set of requirements and guidelines derived from these case-study research efforts to aid researchers in meeting the burden of proof. Understanding the innovation process, how innovation can be enhanced and how it can be measured are key steps to managing and enhancing innovation. The factors that affect innovation on a project were identified, as well as how these factors can be used to measure the level of innovation on a project, and the practices and processes that encourage and facilitate innovative changes. Case studies of construction projects in the United States revealed three necessary components of innovation: idea generation, opportunity and diffusion. A variety of practices are used to optimize each component including support and commitment from the owner/client and firm upper management, workforce and project team integration and diversity. Applying the practices identified in the research leads to enhanced innovation through better communication among project team members, integration of the design and construction disciplines, more efficient designs, development of unique ways of completing work and sharing of the lessons learned.
