            "abstractNote": "BACKGROUND:Evidence based medicine (EBM) has become standard approach in medicine. Patients and health authorities increasingly claim active patient roles in decision making. Education to cope with these roles might be useful. We investigated the feasibility, acceptability and possible impact of EBM training courses for patient and consumer representatives.METHODS:We designed a generic one week EBM course based on previous experience with EBM courses for non medical health professionals. A course specific competence test has been developed and validated to measure EBM skills. Formative and summative evaluation of the course comprised: 1) EBM skills; 2) individual learning goals; 3) self reported implementation after six months using semi-structured interviews; 4) group based feedback by content analysis. EBM skills' achievement was compared to results gathered by a group of undergraduate University students of Health Sciences and Education who had attended a comparable EBM seminar.RESULTS:Fourteen EBM courses were conducted including 161 participants without previous EBM training (n=54 self-help group representatives, n=64 professional counsellors, n=36 patient advocates, n=7 others); 71% had a higher education degree; all but five finished the course. Most participants stated personal learning goals explicitly related to practicing EBM such as acquisition of critical appraisal skills (n=130) or research competencies (n=67). They rated the respective relevance of the course on average with 80% (SD 4) on a visual analogue scale ranging from 0 to 100%.\nParticipants passed the competence test with a mean score of 14.7 (SD 3.0, n=123) out of 19.5 points. The comparison group of students achieved a mean score of 14.4 (SD 3.3, n=43). Group based feedback revealed increases of self confidence, empowerment through EBM methodology and statistical literacy, and acquisition of new concepts of patient information and counselling. Implementation of EBM skills was reported by 84 of the 129 (65%) participants available for follow-up interviews. Barriers included lack of further support, limited possibilities to exchange experiences, and feeling discouraged by negative reactions of health professionals. CONCLUSIONS:Training in basic EBM competencies for selected patient and consumer representatives is feasible and accepted and may affect counselling and advocacy activities. Implementation of EBM skills needs support beyond the training course.",

            "abstractNote": "Over the coming years a large number of nuclear power plants (NPP) will be decommissioned as they reach the end of their operational life and the decommissioning proposals will be subjected to Environmental Impact Assessment (EIA). At present, experience of decommissioning is extremely limited and three detailed reviews were conducted of the public participation experiences associated with this activity in three European Union countries (Germany, Spain and the United Kingdom) to try to identify elements of good practice which could be carried forward to future decommissioning projects.\nFor each of the case studies, information was gathered using a combination of review of the Environmental Statement, site visits and interviews with key personnel identified by the study team. The information collected pointed to five [`]success factors', which could be used to summarise the main aspects of good practice to carry forward. These can be categorised as: authority acceptance of the value of participation; integration of all participation activities under the umbrella of EIA; carrying out participation activities before final strategies are adopted; keeping the decision-making process transparent; and providing sufficient information to the public to facilitate participation.\nA further review of public participation experiences in the literature covering 13 contentious projects provides support for these success factors.",

            "abstractNote": "To resolve tensions among competing sources of evidence and public expectations, health-care managers and policy makers are turning more than ever to involve the public in a wide range of decisions. Yet efforts to use research evidence to inform public involvement decisions are hampered by an absence of rigorous public participation evaluation research. In particular, greater rigour in exploring the roles played by different contextual variables--such as characteristics of the issue of interest, the culture of the sponsoring organization and attributes of the decision being made--is needed. Using a comparative quasi-experimental design, we assessed the performance of a generic public participation method implemented in 5 Canadian regionalized health settings between 2001 and 2004. Participant and decision-maker perspectives were assessed and, through direct observation, the roles exerted by contextual variables over the public involvement processes were documented and analysed. Our findings demonstrate that a generic public participation method can be implemented in a variety of contexts and with considerable success. Context exerts fostering and inhibiting influences that contribute to more (or less) successful implementation. Public participation practitioners are encouraged to pay careful attention to the types of issues and decisions for which they are seeking public input. Sufficient organizational resources and commitment to the goals of the public participation process are also required. Attention to these contextual attributes and their influence on the design and outcomes of public participation processes is as important as choosing the \"right\" public participation mechanism.",

            "abstractNote": "Objective: To investigate the extent to which people change their views about priority setting in health care as a result of discussion and deliberation. Design: A random sample of patients from two urban general practices was invited to attend two focus group meetings, a fortnight apart. Setting: North Yorkshire Health Authority. Subjects: 60 randomly chosen patients meeting in 10 groups of five to seven people. Main outcome measures: Differences between people's views at the start of the first meeting and at the end of the second meeting, after they have had an opportunity for discussion and deliberation, measured by questionnaires at the start of the first meeting and the end of the second meeting. Results: Respondents became more reticent about the role that their views should play in determining priorities and more sympathetic to the role that healthcare managers play. About a half of respondents initially wanted to give lower priority to smokers, heavy drinkers, and illegal drug users, but after discussion many no longer wished to discriminate against these people. Conclusion: The public's views about setting priorities in health care are systematically different when they have been given an opportunity to discuss the issues. If the considered opinions of the general public are required, surveys that do not allow respondents time or opportunity for reflection may be of doubtful value.\n\nKey messages Emphasis is being placed on involving the public in decisions about healthcare priorities Different techniques used to obtain the public's views may give different results People's views on setting priorities differ systematically when they have been given opportunity to discuss the issues People become less willing to discriminate against smokers, heavy drinkers, and illegal drug users when they have had an opportunity to reflect on and discuss their views If considered opinions are required, the value of surveys that do not allow time or opportunity for reflection may be in doubt",

            "abstractNote": "Within Canada's Aboriginal population, an ongoing health promotion strategy has been the facilitation of community participation in the development and application of health policy. The Calgary Health Region's Aboriginal Community Health Council has provided a setting for involving the local Aboriginal population in health policy and program development for over a decade. This paper represents the results of a case study to identify the Council's strategies for this work. Data sources included documents, such as meeting minutes and other reports; key informant interviews with past and present Council members and health region representatives; and participant observation of Council functions. Although direct membership in the Council provided a core approach for representing the community, other strategies were actively utilized to involve the public. These included building links and partnerships with community organizations, networking, consultation activities and the identification of special needs groups.",

The report is part of a series available from RAND Europe's Observatory on Health Research Systems. The purpose of the document is to provide non-specialists with an overview of the evolution and development of participatory health research across a range of economically developed countries. The report is divided broadly into two parts. The first part addresses the rationale for involving lay participants in health research in the first place, and explores some of the drivers underpinning recent moves to bolster participatory research methods. It also highlights some of the advantages and disadvantages of participatory approaches. The second part includes a series of country case studies, covering current patient and service-user involvement practices in Australia, Canada, the Netherlands, the United Kingdom, and the United States. The report concludes with a review of some options available to policymakers looking to boost or improve current practices, and an outline of further research needs. The report will be of interest to government officials dealing with health research policy, medical research councils, health research charities, institutions hosting health research projects, researchers, and patients and service users themselves.",

            "abstractNote": "Background\nConceptualisations of the nurse-patient relationship tend to view nursing as embodying an empowering approach to patients, one that places the service user perspective at the centre of decision-making. However, the relationship of nursing to public participation in health service planning and development has been under examined.Aims\nThe aim is to explore the relationship of the nursing profession to public participation as enacted through a UK-based patient and public council, located in an acute hospital. The council was developed by nursing staff and aimed to achieve service user participation in strategic level health care decision-making. The views and experiences of participants and the applicability of the [`]nurse-patient partnership' construct to public participation are considered.Methods\nThe study employed integrative ethnography, involving multiple field methods: non-participant observation of council meetings, i.e. fourteen 3 h meetings (n=42 h); in-depth interviews with councillors (n=17) and with key hospital staff (n=18). A documentary review and mapping of the actions of the council was undertaken.Results\nA nurse-patient partnership was not initially intrinsic to the operation of the council or embedded in the perspectives of the nurse or patient participants. Professional vulnerability and the organisational context constrained the nursing response. Councillors and nursing staff moved to create a shared set of understandings in order to progress change in service organisation and delivery. Nurses' repositioning vis-à-vis the credibility of user experiences and status was central to the effective progression of the council.Conclusions\nPartnership in public participation requires a shift by nurses' towards acceptance of members of the public functioning as informed, critical and powerful agents in health care decision-making. Equipping nurses with the skills to communicate with patient representatives in a position of interactional equality is likely to be a pre-requisite for successful engagement by nursing with public participation.",

            "abstractNote": "This article examines a detailed case study of implementation networks in England using the example of the relocation of the Norfolk and Norwich hospital, which became a flagship PFI project for the Labour government after 1997. The case study illustrates the workings of the new order of multi-layered governance with both local and national networks from different policy areas interacting. However, it also sheds light on the governance debate and illustrates how in the world of new public management, powerful actors, or policy entrepreneurs, with their own agenda, still have the facility, by exercising power and authority, to shape and determine the policy outputs through implementation networks. It is argued that, whereas policy networks are normally portrayed as enriching and promoting pluralist democratic processes, implementation networks in multi-layered government can also undermine democratic accountability. Four aspects here are pertinent: (1) the degree of central government power; (2) local elite domination; (3) the fragmentation of responsibility; and (4) the dynamics of decision making which facilitates the work of policy entrepreneurs. All these factors illustrate the importance of 'the government of governance' in the British state.",

            "abstractNote": "In the UK, policy on the governance of medical performance is characterised by a continuing struggle between state and profession for control of the agenda setting, formation and implementation stages of the policy process. Since 1998 both sides have continued to produce policies in response to highly visible political pressures but have yet to agree on how those policies should engage as they are implemented at the level of the individual practitioner. For the state, clinical governance forms the lynchpin of its drive to increase managerial control over doctors and, for the profession, revalidation is seen as the means for ensuring the quality of medical performance whilst preserving medicine's historic autonomy. This paper analyses the course of this 7-year struggle and shows how in constructing and delivering policy, state and profession draw on quite different and separate sets of institutional structures and values. As a consequence, there is an unresolved competition for dominance and little engagement between the two policy streams.",

            "abstractNote": "How to involve the public in setting health and health care priorities is a constant challenge for health system decisions. Policy maker interest in involving the public in increasingly complex and value-laden priority setting processes has led to the use of deliberative public involvement methods designed to promote discussion and debate among participants with the objective of obtaining more informed and consensual views. These methods have not been evaluated rigorously using controlled designs with pre- and post-test measurements. We examined, using a controlled design, the effects of introducing different opportunities for deliberation into a process for obtaining public input into a community health goals priority setting process. Our findings indicate that deliberation does make a difference to participant views. As more deliberation is introduced, participant views may be more amenable to change. Deliberation also offers the potential for views to become more rather than less entrenched. While we are beginning to understand the difference deliberation makes to participant views, we are still at an early stage in understanding the process through which these differences come about and what difference deliberation makes to broader outcomes such as civic competence, civic engagement and health policy decisions.",

            "abstractNote": "Dans la foulée de la mise à jour du Programme national de santé publique du Québec (PNSP), la Direction générale de la santé publique du ministère de la Santé et des Services Sociaux (MSSS) a rendu publique une Stratégie pour le développement de la recherche en santé publique (2008). Cette stratégie propose des orientations fondées, entre autres, sur les résultats d’un Bilan de la recherche en santé publique au Québec. Une de ces orientations concerne le renforcement des capacités de valorisation des résultats de la recherche en soutien aux acteurs du réseau de la santé et des services sociaux. Parmi les actions prioritaires mentionnées, on retrouve le développement de diverses formes de partenariat pour la production de connaissances, telles des subventions pour des synthèses de connaissances, des projets d’expérimentation ou des études de faisabilité, ainsi que la mise en place de stratégies efficaces de transfert des connaissances.\n\nCes orientations participent à un mouvement plus large en faveur du transfert et de l’utilisation des connaissances. En effet, de nombreuses politiques d’innovation plaident en faveur d’un dialogue plus fructueux entre la science et la société dans le but d’accroître les retombées de la recherche financée par les fonds publics. De façon générale, on remarque également une volonté chez les organismes des secteurs gouvernementaux, universitaires, communautaires et privés de favoriser une plus grande utilisation des connaissances disponibles afin d’apporter des changements dans les pratiques et la prise de décision. Au Québec comme ailleurs, le renforcement des mécanismes de transfert des connaissances devient de plus en plus une priorité.\n\nC’est dans ce contexte qu’en 2008, le MSSS a confié à l’Institut national de santé publique du Québec (INSPQ) la réalisation d’une synthèse de connaissances sur le transfert des connaissances (TC) dans une perspective de soutien à l’action.\n\nLe présent document présente d’abord un survol des connaissances sur le transfert des connaissances dans le domaine de la santé. Ce bilan des connaissances s’appuie sur la littérature pour identifier les principales composantes à considérer pour la mise en oeuvre de pratiques plus structurées de transfert des connaissances. Une synthèse et une conceptualisation dynamique et intégrée du processus de transfert est présentée à la section suivante. En annexe, on retrouve un outil pour animer un processus de transfert des connaissances et un aide-mémoire résumant en un coup d’oeil le contenu de la présente publication.\n\nBien qu’il ait été élaboré à l’intention des acteurs de santé publique, ce document peut s’appliquer à divers contextes. Il s’adresse donc à un vaste public de gestionnaires, décideurs, intervenants, responsables de politiques publiques, oeuvrant dans des sphères d’activités variées, ainsi qu’à des professionnels agissant comme agents de liaison, agents de transfert des connaissances, chercheurs ou courtiers de connaissances dans leur milieu. Pour les acteurs de santé publique, ce document présente toutefois l’avantage d’utiliser des exemples tirés de leur réalité et de leur fournir des repères pour leurs pratiques.",
