The study reported here provides evidence for benefits of using visual-based tools for food safety training when educating immigrant, Hispanic foodservice workers with no or minimal English language skills. Using visual tools along with hands-on training improved knowledge scores of foodservice workers about thermometer use and cleaning and sanitizing procedures. Extension personnel can use similar educational tools along with accommodation for cultural differences for effective training of immigrant Hispanic workers employed in foodservice and lodging operations, farms, and meat processing plants.",

            "abstractNote": "This article is the second in a series of several by members of the Committee on the Control of Foodborne Illness of the International Association of Food Protection, and it continues the analysis of 816 outbreaks where food workers were implicated in the spread of foodborne disease. In this article, we discuss case morbidity and mortality and the settings where the 816 outbreaks occurred. Some of the outbreaks were very large; 11 involved more than 1,000 persons, 4 with more than 3,000 ill. The larger outbreaks tended to be extended over several days with a continuing source of infections, such as at festivals, resorts, and community events, or the contaminated product had been shipped to a large number of customers, e.g., icing on cakes or exported raspberries. There were five outbreaks with more than 100 persons hospitalized, with rates ranging from 9.9 to 100%. However, overall, the hospitalization rate was low (1.4%), and deaths were rare (0.11% of the 80,682 cases). Many of the deaths were associated with high-risk persons (i.e., those who had underlying diseases, malnutrition, or both, as in a refugee camp, or young children), but a few occurred with apparently healthy adults. An analysis of the settings for the food worker-related events showed that most of the outbreaks came from food service facilities (376 outbreaks [46.1%]), followed by catered events (126 outbreaks [15.4%]), the home (83 outbreaks [10.2%]), schools and day care centers (49 [6.0%]), and health care institutions (43 outbreaks [5.3%]). However, many cases resulted from relatively few outbreaks (< 30 each) associated with community events (9,726), processing plants (8,580), mobile/temporary service (5,367), and camps/ armed forces (5,117). The single most frequently reported setting was restaurants, with 324 outbreaks and 16,938 cases. Improper hygienic practices in homes, on picnics, or at community events accounted for 89 of the 816 outbreaks. There were 18 outbreaks associated with commercial travel in air flights, trains, and cruise ships over several decades, although only the last seems to be a major concern today. Sixteen outbreaks occurred where food, primarily produce, was harvested and shipped from one country to another. Sometimes the presence of an infected worker preparing food was only one of several factors contributing to the outbreak.",

            "abstractNote": "On June 18, 2010, Hamilton County Public Health (HCPH), a local health department in Ohio, began receiving reports of gastrointestinal illness from persons who attended a church festival held during June 11-13 in a suburban community of Hamilton County. HCPH investigated and confirmed the existence of a foodborne outbreak associated with consumption of pulled pork prepared in a private home and sold at the church festival. Sixty-four attendees with gastroenteritis were identified. Salmonella enterica serotype Typhimurium (Salmonella Typhimurium) was found in stool specimens from three patients; no other pathogen was found. Because the outbreak was identified after the church festival had concluded, the environmental investigation was limited to interviews of food handlers. The primary public health interventions consisted of 1) active surveillance for additional cases of salmonellosis associated with the festival, 2) consultation with the festival organizers and food vendors to ensure the pork product was not resold or consumed elsewhere, 3) education of the festival organizers and food vendors about relevant public health regulations and food safety practices, 4) traceback of the implicated product to the retailer in Indiana, and 5) notification of the Indiana State Department of Health. The results of the investigation call attention to the public health implications of unregulated food service at events such as church festivals, which generally are exempt from public health inspection and licensure in Ohio. Food sold in such environments might place populations at risk for foodborne illness.",

            "abstractNote": "The Stavanger Food Festival (the Gladmatfestival) provides the empirical context for a case study on festival safety. Annually the festival gathers 250,000 participants and 500 staff involved as exhibitors and the like in a small area in the harbour area in central Stavanger. Social gatherings like this constitute significant safety challenges, as seen in the recent festival disasters in Phnom Penh (Cambodia) and Love Parade in Duisburg (Germany). A risk analysis of the Gladmat festival showed that drowning, exposure to fire and explosions, excessive alcohol abuse, violence, public disorder, wind damaging the tents, food poisoning and other hygiene issues were central hazards. Some conditions were also potential risk factors for goods and property. In addition, installing and removing the material structures of the festival involved high risks for accidents, human suffering and damaged properties. Over several years, a management plan was developed by external consultants to encounter these risks. While the plan was initiated by the festival, it was developed in concert with the Police being the responsible agent for emergency and rescue. The plan contributed to a learning process in the administration of the hosting municipality, which contributed by launching an online handbook with all relevant application forms needed by festival managers when applying for permission to use public space for festivals. The management plan was communicated thoroughly through the festival organisation and discussed both beforehand and every day at the staff morning meetings. It was followed strictly throughout the entire event. No serious damage was caused to visitors, exhibitors and other staff and property. This may be related to the strict loyalty to the management plan. The safety management planning and its implementation are applicable for other event organisers.",

            "abstractNote": "In August 1988, an estimated 3,175 women who attended a 5-day outdoor music festival in Michigan became ill with gastroenteritis caused by Shigella sonnei. Onset of illness peaked 2 days after the festival ended, and patients were spread throughout the United States by the time the outbreak was recognized. An uncooked tofu salad served on the last day was implicated as the outbreak vehicle (odds ratio = 3.4, p <0.0001). Over 2,000 volunteer food handlers prepared the communal meals served during the festival. This large foodborne outbreak had been heralded by a smaller outbreak of shigellosis among staff shortly before the festival began and by continued transmission of shigellosis from staff to attendees during the festival. S. sonnei isolated from women who became ill before, during, and after the festival had identical antimicrobial susceptibility patterns and plasmid profiles, limited access to soap and running water for handwashing was one of the few sanitary deficits noted at this gathering. This investigation demonstrates the need for surveillance and prompt public health intervention when Shigella infections are recognized in persons attending mass outdoor gatherings, the singular importance of handwashing in reducing secondary transmission of shigellosis, and the potential for explosive outbreaks when communal meals are prepared by large numbers of food handlers.",

            "abstractNote": "Personal interviews and mail surveys were used to examine consumers' perceptions and concerns about the healthfulness and safety of food served at fairs and festivals and other types of food preparation locations. A total of 304 responses were collected. Home-prepared foods were perceived as the most healthful and safest, while foods served at outdoor fairs and festivals and fast food restaurants were considered the most unhealthful and least safe. Food poisoning/spoilage was the major safety concern, while fat or cholesterol was the major health concern at all locations. Insect and dirt or dust contaminations were major concerns when eating at outdoor fairs and festivals.",
