La supériorité de la TEP–scanographie au FDG par rapport à la scanographie, pour le bilan d’extension locorégionale et à distance, a été confirmée par une méta-analyse. La TEP–scanographie au fluorodésoxyglucose (FDG) améliore la reproductibilité de la délinéation des volumes cibles, notamment au contact du médiastin ou d’une atélectasie. La réduction du volume cible médiastinal, en limitant l’irradiation aux sites ganglionnaires concentrant le FDG, est communément admise, bien qu’aucune étude randomisée ne l’ait formellement validée. En revanche, la méthode optimale de délinéation reste à définir. L’apport de la TEP–scanographie au FDG pour suivre la réponse tumorale pendant la radiothérapie est en cours d’évaluation, dans la perspective d’une adaptation des modalités d’irradiation selon la radiosensibilité tumorale. D’autres traceurs, notamment le fluoromisonidazole (F-miso) (hypoxie), font l’objet d’études cliniques. L’intégration de l’imagerie multimodalité (scanographie, TEP) sans retarder la mise en traitement impose de résoudre d’importantes difficultés organisationnelles (accès aux équipements), techniques (fusion et traitement des images) et médicales (délinéation pluridisciplinaire des volumes cibles). \n \nThe recent advances in medical imaging have profoundly altered the radiotherapy of non-small cell lung cancers (NSCLC). A meta-analysis has confirmed the superiority of FDG PET-CT over CT for initial staging. FDG PET-CT improves the reproducibility of target volume delineation, especially close to the mediastinum or in the presence of atelectasia. Although not formally validated by a randomized trial, the reduction of the mediastinal target volume, by restricting the irradiation to FDG-avid nodes, is widely accepted. The optimal method of delineation still remains to be defined. The role of FDG PET-CT in monitoring tumor response during radiotherapy is under investigation, potentially opening the way to adapting the treatment modalities to tumor radiation sensitivity. Other tracers, such as F-miso (hypoxia), are also under clinical investigation. The sizes of both the lymph nodes and the metastatic foci within the lymph nodes were measured, and these measurements were compared with those obtained with positron emission tomography scanning. To establish general sizes of metastatic foci within the lymph nodes, 277 metastatic lymph nodes in operative specimens previously resected from another 111 patients with lung cancer were examined as a control.\n\n\nRESULTS\n\nThe sensitivity was significantly higher for positron emission tomography than for computed tomographic scanning (P =.026). The sizes of metastatic foci within lymph nodes that showed false-negative (n = 8) and true-positive (n = 28) with positron emission tomography ranged from 0.5 to 9 mm (3 +/- 1 mm) and from 4 to 18 mm (10 +/- 3 mm), respectively (P <.001). None of the metastatic foci smaller than 4 mm could be detected with positron emission tomography scanning. The review of the 277 previously resected metastatic lymph nodes showed that 89 (32%) had metastatic foci smaller than 4 mm. The sizes of true-positive (n = 28) and false-positive (n = 10) lymph nodes ranged from 6 to 15 mm (10 +/- 2 mm) and from 9 to 16 mm (12 +/- 2 mm), respectively (P <.01). None of the false-positive lymph nodes was smaller than 9 mm.\n\n\nCONCLUSIONS\n\nAlthough positron emission tomography was superior to computed tomography scanning in lymph node staging in lung cancer, positron emission tomography was unable to distinguish metastatic foci smaller than 4 mm, which were not unusual sizes for lymph node metastases in lung cancer. "abstractNote": "PURPOSE\n\nTo assess and compare the usefulness of (11)C-Choline-positron emission tomography (PET) with that of 2-deoxy-2-[(18)F]fluoro-D-glucose (FDG)-PET for the differentiation between benign and malignant in various tumors.\n\n\nMATERIALS AND METHODS\n\nWe examined 38 consecutive patients with various tumors, including seven patients with brain tumors, two with oral cavity tumor, two with esophageal cancer, six with lung cancer, 11 with bone tumor, nine with soft tissue tumors, and one with myeloma. (11)C-Choline-PET and FDG-PET were performed from five minutes and 40 minutes, respectively, after injection of 275-455 MBq tracer. Tracer uptakes were evaluated by the standardized uptake value (SUV) and were analyzed in according to the pathologic data.\n\n\nRESULTS\n\n(11)C-Choline uptake in malignancies (3.9 +/- 1.7, n=24) was significantly higher than that in benign lesions (1.7 +/- 1.2, n=14) (P < 0.0001). On the other hand, the FDG uptake in malignancies was 4.9 +/- 2.0 (n=24) and was also significantly larger than that in benign lesions 1.6 +/- 1.3 (n=14) (P < 0.0001). The (11)C-Choline uptake in all the lesions correlated with FDG uptake (r=0.658, P < 0.02, n=38).
