"abstractNote": "Geometric morphometric methods rely on the accurate identification and quantification of landmarks on biological specimens. As in any empirical analysis, the assessment of inter- and intra-observer error is desirable. A review of methods currently being employed to assess measurement error in geometric morphometrics was conducted and three general approaches to the problem were identified. One such approach employs Generalized Procrustes Analysis to superimpose repeatedly digitized landmark configurations, thereby establishing whether repeat measures fall within an acceptable range of variation. The potential problem of this error assessment method (the \"Pinocchio effect\") is demonstrated and its effect on error studies discussed. An alternative approach involves employing Euclidean distances between the configuration centroid and repeat measures of a landmark to assess the relative repeatability of individual landmarks. This method is also potentially problematic as the inherent geometric properties of the specimen can result in misleading estimates of measurement error. A third approach involved the repeated digitization of landmarks with the specimen held in a constant orientation to assess individual landmark precision. This latter approach is an ideal method for assessing individual landmark precision, but is restrictive in that it does not allow for the incorporation of instrumentally defined or Type III landmarks. Hence, a revised method for assessing landmark error is proposed and described with the aid of worked empirical examples.",

The mean age of the patients upon commencement of expansion was 7 years and 6 months (standard deviation [SD], 12 months). Measurement of palatal volume was conducted via 3D acquisition of plaster models using laser scanning before treatment (T1), upon device removal (T2), and 2.6 years afterward (T3).\n\n\nRESULTS\n\nThe volume of the palate increased in a statistically significant fashion from T1 to T2 and from T1 to T3, and it decreased in a nonsignificant fashion from T2 to T3.\n\n\nCONCLUSIONS\n\nPalatal volume significantly increases with rapid maxillary expansion (RME) treatment with insignificant relapse. The use of virtual 3D models with the aid of Apposite software permits evaluation of the morphologic and volumetric changes induced by orthodontic treatment.",

"abstractNote": "A retrospective study of dental and maxillary skeletal changes occurring during a period of orthodontic treatment was made from pretreatment and posttreatment dental casts. Sixty maxillary expansion cases were examined. Thirty cases had maxillary expansion accomplished with a fixed rapid palatal expander and 30 were expanded with a quadhelix appliance. All cases were finished with full fixed edgewise appliances. Multiple linear regression analyses were completed for both groups with upper molar width change as the criterion and age, tipping of the upper molars, palatal width change and maxillary tipping as the predictors. All predictors were included in the analysis for the quadhelix group with a significant R2 value of 0.55. For the rapid expansion group, a significant R2 value of 0.33 was achieved with the inclusion of palatal width change and age only. The other variables did not meet the level of significance for entry into the model. Although both groups demonstrated similar amounts of maxillary dental expansion, the rapid expansion group demonstrated greater average skeletal expansion. In addition, there was a significant relationship between skeletal and dental expansion for the rapid expansion group, but not the quadhelix group. Palatal depth increased more on average in the rapid expansion group suggesting that there was greater dental eruption in that group. Expansion across the mandibular molars was greater on average in the quadhelix group. There was no difference in the degree of upper molar rotation or final upper and lower arch forms between the two groups.",

"abstractNote": "INTRODUCTION\n\nThe purpose of this retrospective study was to estimate the area change of the palate after rapid maxillary expansion (RME) in the early mixed dentition stage by using a 3-dimensional (3D) helical computed tomography (CT) scanning technique. In addition, linear changes in the maxillary arch were evaluated.\n\n\nMETHODS\n\nThe treated sample consisted of 43 children (mean age, 9 years 1 month) treated with a bonded RME appliance. The untreated control group consisted of 7 children (mean age, 9 years 3 months). Pretreatment and posttreatment dental casts were evaluated by using 3D helical CT scanning procedures. The Student t test was used to compare the linear, area, and angular differences between the treatment times.\n\n\nRESULTS\n\nRME produced clinically significant increases in interdental widths across the canines, the deciduous first molars, and the permanent first molars in the maxillary arch. Significant increases in cross-sectional area were observed across the permanent first molars (15.3 mm(2)). There was marked variability in the buccal tipping of the permanent first molars.\n\n\nCONCLUSIONS\n\nThree-dimensional helical CT scanning is an accurate and cost-effective method of assessing dental cast morphologic changes. It can also provide fast and accurate data acquisition and subsequent analysis.",

"abstractNote": "The purpose of this study was to evaluate treatment outcomes and long-term stability in patients treated by surgically assisted rapid maxillary expansion (SARME) and to compare the results with a matched, untreated control group. The sample comprised consecutive study models from 31 subjects (17 males and 14 females) with a mean pre-treatment age of 25.9 years [standard deviation (SD) 9.6]. The mean follow-up time was 6.4 years (SD 3.3). The transverse distances between the maxillary canines and maxillary first molars were measured with digital sliding callipers before treatment (T0), after treatment (T1), and at follow-up (T2). The data were analysed with a Mann-Whitney U, Spearman's rho, and Wilcoxon signed-rank tests. At T1, all posterior crossbites were corrected and the expansions were statistically significant. At T2, despite some reduction in the transverse measurements, the posterior crossbites remained corrected. There were no statistically significant differences between the treatment and control groups at T2 regarding transverse measurements, except for the distance between the mesio-buccal cusp tips of the maxillary first molars. In the treatment group, there was no significant difference in terms of reductions in the transverse dimensions over the short- or long-term, no significant correlations between age or gender and the decrease in transverse dimensions or between the degree of anterior and posterior expansion. There were no significant correlations between the degree of expansion and subsequent post-treatment decrease. The results indicate that SARME normalizes the transverse discrepancies and is stable a mean of 6 years post-treatment. The decreases in the transverse dimensions are most pronounced during the first 3 years post-treatment.",

Data were extracted by 2 observers.\n\n\nRESULTS\n\nTen studies fulfilled the inclusion criteria, of which 9 were prospective and 1 was a retrospective case series.\n\n\nCONCLUSION\n\nNo RCT's or CCT's were published on bone-borne surgically assisted rapid maxillary expansion (SARME). For expected advantages compared to tooth-borne SARME, only weak evidence was found for less buccal tipping of the teeth used as anchor teeth in tooth-borne expansion. The heterogeneity of the retrieved publications and the wide variety of outcome variables posed serious restrictions on the review of the literature in a quantitative systematic manner. There is a need for well designed clinical trials research on the effects of tooth-borne and bone-borne SARME.",

METHODS: A search was performed in electronic databases. Human clinical trials with patients submitted to surgically assisted rapid maxillary expansion with a follow-up of at least 1 year after expansion were selected. A methodological quality scoring process was used. A meta-analysis was performed to compare measurements of skeletal and dental structures. RESULTS: Three hundred sixty-five titles and abstracts were read. Ultimately 10 studies met the inclusion criteria. The 3 articles ranked as presenting low methodological quality were excluded. Three measurements could be compared and 3 time periods were used to assess changes. CONCLUSIONS: There is moderate evidence to conclude that maxillary alveolar width and intercanine and intermolar width have a long-term significant increase as a result of surgically assisted rapid maxillary expansion. A significant relapse is expected in the intercanine width after expansion.",

"abstractNote": "OBJECTIVES\n\nTo evaluate changes in the palatal vault after rapid maxillary expansion (RME) with bonded splint appliances.\n\n\nSETTING AND SAMPLE POPULATION\n\nThe sample comprised 24 children (12 boys and 12 girls) with mixed dentition (mean age 8.3 years; range 6.4-10.4 years).\n\n\nMATERIALS AND METHODS\n\nFollowing expansion, the splint appliance was used as a retainer for 6 months and then removed. Study casts were taken before RME (T0) and when the appliance was removed (T1). Then, 3D laser scans were taken to build complete 3D jaw models. Frontal cross sections were constructed at 53-63, 55-65 and 16-26, exported as coordinates, and finite element calculated to quantify their area, width and height. Maxillary length was also determined.\n\n\nRESULTS\n\nPaired t-tests indicated statistically significant increases in the average palatal width (T1-T0=6.53-6.79 mm) and cross-sectional area (T1-T0=20.39-21.39 mm2) after RME (p<0.001). However, small but statistically significant reductions were observed in palatal height (T1-T0=-0.49 mm, only at 55-65; p<0.001) and length (T1-T0=-0.54 mm; p<0.01). Linear regression analysis showed statistically significant (p<0.001) direct correlations between the widths and respective cross-sectional areas. Age did not influence any measurement. The reliability of the measurements was examined with an intraclass correlation coefficient (ICC). We found an ICC>0.99 (p<0.001) for all tested parameters.\n\n\nCONCLUSIONS\n\nRapid maxillary expansion distinctly increased mean palatal widths and cross-sectional areas. However, palatal height (55-65) and maxillary length decreased to a small extent.",

"abstractNote": "INTRODUCTION\n\nRapid palatal expansion (RPE) is routinely used to correct transverse deficiencies in the maxilla, but its effects on the dentoalveolus are uncertain. The purpose of this study was to compare measurements made on cone-beam computerized tomography scans between patients with RPE treatment and controls to determine transverse dimension increases and the amounts of alveolar and dental tipping.\n\n\nMETHODS\n\nTwenty-five patients with posterior crossbite who required RPE treatment and 25 sex- and age-matched controls (no crossbite) were orthodontically treated and received cone-beam computerized tomography scans at the beginning and middle of treatment. Transverse widths and several angulations were measured, and matched paired t tests used.\n\n\nRESULTS\n\nRPE treatment produced a significant increase in all measured transverse dimensions. Analysis of posttreatment angulation changes in the RPE group showed that the alveolus substantially tipped buccally by nearly 5.6° (measured from a horizontal reference, the base of the hard palate). The angulations of the dentition, however, remained constant before and after treatment (<1° of change) in both groups.\n\n\nCONCLUSIONS\n\nThese data showed no statistically significant amount of relative dental tipping after RPE treatment but significant alveolar tipping compared with the controls.",

"abstractNote": "Different devices are available to aid surgically-assisted maxillary expansion. In this study we have evaluated the changes to the anchoring teeth, the hard palate, and the lower nasal passage made by tooth-borne distraction devices. Thirty-one patients (mean (SD) age 28 (2) years) with deficiencies in the transverse width of the maxilla were examined by computed tomography and cone beam scans before and after operation. The data were analysed with the help of Wilcoxon's signed rank test and Spearman's r correlation. The mean (SD) distraction width was 6.5 (2.3) mm. All anchorage teeth were tilted (p<0.01). The axes changed by a mean (SD) of 4.8 (0.9)° in the first premolar and 3.1 (0.8)° in the first molar. The nasal isthmus increased by a mean (SD) of 2.5 (0.3) mm. The hard palate adjacent to the anchoring teeth increased anteriorly by a mean (SD) of 2.8 (0.4) mm and posteriorly by 2.7 (0.4) mm. The hard palate was lowered by 1.2 (0.8) mm. There was a significant correlation in the distraction width, with changes in the intercoronal and interapical distances of the anchoring premolars (p<0.05) and with the interapical distance of the anchoring molars (p<0.01). There was also a correlation between the distraction width and the overall gain in width of the lower nasal passage (p<0.05). The results suggested that surgically-assisted maxillary expansion with tooth-borne devices has significant effects on the anchoring teeth, the nasal floor, and the hard palate. Both tilting of the teeth and an evenly distributed movement of the segments were seen.",

"abstractNote": "INTRODUCTION\n\nThe purpose of this retrospective study was to evaluate the dentoskeletal changes after slow maxillary expansion in adults.\n\n\nMETHODS\n\nThree kinds of maxillary expanders (Haas [Dyna Flex, Laboratory, St Louis, Mo], MAX-2000 [Dyna Flex], and DMAX-2000 [Dyna Flex]) were used randomly in 3 study groups of 65 adults treated with nonsurgical slow maxillary expansion, followed by full straight-wire appliance therapy. A control group of 22 adults, meeting the same inclusion criteria, were treated only with full straight-wire appliance therapy without expansion. Posteroanterior and lateral cephalometric records were taken before the start of treatment (T1) and after fixed orthodontic treatment (T2). The data were analyzed statistically by using paired t tests and Kruskal-Wallis H tests.\n\n\nRESULTS\n\nFrom T1 to T2, there was no significant change in nasal width, maxillary width, and mandibular width in the 3 groups. The sella-nasion-mandibular plane angle increased 0.97 degrees (P <0.05) in the DMAX-2000 group. The mandibular incisor-mandibular place angle in the Haas, MAX-2000, DMAX-2000, and control groups had mean increases of 4.90 degrees, 4.42 degrees, 4.55 degrees, and 5.20 degrees (P <0.05 for each), respectively. The maxillary incisor-nasion-A-point angle increased 3.04 degrees (P = 0.05), and the distal root of the maxillary first molar-nasion perpendicular decreased 1.48 mm (P <0.05) in the control group. The distal facet of the maxillary first molar-nasion perpendicular decreased 3.57 mm (P <0.05), and the distal root of the maxillary first molar-nasion perpendicular decreased 2.50 mm (P <0.05) in the MAX-2000 group.\n\n\nCONCLUSIONS\n\nAfter expansion treatment, there were no evident transverse or significant sagittal and vertical skeletal maxillary changes in the 3 groups.",

"abstractNote": "The aim of this study was to assess, by a digital photogrammetric technique, the relative dimensional changes before and after rapid maxillary expansion (RME). The transverse diameters and volumetric variations of the palate were measured by photogrammetry on study casts taken at three different phases of therapy: at the beginning of treatment (T1), on removal of the rapid expander, after expansion and retention for three months (T2), and six months after appliance removal (T3). The sample consisted of 30 children, (age range 7-8 years), all with a crossbite; 15 were angle Class I, six Class II and nine Class III. They were treated with an acrylic splint expander with two turns per day until the maxillary molar palatal cusps were in contact with the mandibular molar buccal cusps. The RME device was used as a passive retainer for three months, after which it was removed. During the following six months, no retention was used and no orthodontic treatment was undertaken. The findings demonstrated a significant relapse (P < 0.001) in the dental transverse diameter in all patients six months after appliance removal, although the palatal volume remained stable.",

"abstractNote": "Previous studies of the morphologic changes of the maxilla after palatal expansion have used 2-dimensional methodologies. In the present study, we used a 3-dimensional surface laser scanning technique and computerized cast analysis, in addition to analysis of anteroposterior cephalograms, to assess the morphologic changes of the palate by 2 kinds of expanders: tissue borne (Haas; n = 9) and tooth borne (Hyrax; n = 10). Cast analysis demonstrated that, although all patients started treatment with similar malocclusion, treatment outcomes were different depending on the appliance used. Both appliances generated maxillary expansion (ie, improved mean surface area, mean intermolar linear distance, and mean perimeter) (P <.05). However, the appliances performed differently to achieve the final expansion. Haas appliances demonstrated a greater orthopedic movement (ie, improvement of the mean interpalatal distance) (P <.05), and Hyrax appliances demonstrated dentoalveolar expansion by increasing the mean palatal angulation of the alveolus (P <.05). Anteroposterior cephalometric analysis showed that both appliances increased mean maxillary width and mean intermolar distance significantly (P <.05). On the other hand, differences in nasal cavity width and upper incisal apex distance were not statistically significant (P >.05). This new 3-dimensional methodology proved useful for comparing treatment outcomes by evaluating the morphologic changes induced by palatal expansion and generated a better visualization of these outcomes.",

"abstractNote": "This study investigated, using cone beam computed tomography (CBCT), the spatial mandibular positioning after rapid maxillary expansion (RME) in Class II Division 1 malocclusion. This prospective study evaluated 17 children (mean initial age 10.36 years old) presenting Class II, Division 1 malocclusion and skeletal maxillary constriction that underwent to RME Haas' protocol. CBCT was performed before treatment (T1), immediately after the stabilization of expander screw (T2) and after the retention period of 6 months (T3). The scans were managed in Dolphin Imaging® 11.0 software, where landmarks (right and left condylion, right and left gonion, and menton) were positioned and measured in relation to sagittal, coronal and axial plane to verify, respectively, transverse, anteroposterior and vertical displacement of the mandible. Paired Student's t-test was used to identify significant differences (p<0.05) between T1 and T2, T2 and T3, and T1 and T3. After RME, right and left gonion moved downward (1.11 mm and 0.89 mm) and menton displaced downward (1.90 mm) and backward (1.50 mm). During the retention period, only anteroposterior displacement was significant, with the right and left gonion (0.97 mm and 1.26 mm) and the menton (2.29 mm) moving forward. Three-dimensional assessment of the mandible in Class II Division 1 patients subjected to RME showed a transitory backward and downward mandibular positioning, without any lateral displacement. The 6-month retention period allowed the mandible shifting significantly forward, exhibiting a more anterior position compared with the initial condition, even remaining in a more downward direction.",

"abstractNote": "Facial recognition is central to the diagnosis of many syndromes, and craniofacial patterns may reflect common etiologies. In the pleiotropic Bardet-Biedl syndrome (BBS), a primary ciliopathy with intraflagellar transport dysfunction, patients have a characteristic facial \"gestalt\" that dysmorphologists have found difficult to characterize. Here, we use dense surface modeling (DSM) to reveal that BBS patients and mouse mutants have mid-facial defects involving homologous neural crest-derived structures shared by zebrafish morphants. These defects of the craniofacial (CF) skeleton arise from aberrant cranial neural crest cell (NCC) migration. These effects are not confined to the craniofacial region, but vagal-derived NCCs fail to populate the enteric nervous system, culminating in disordered gut motility. Furthermore, morphants display hallmarks of disrupted Sonic Hedgehog (Shh) signaling from which NCCs take positional cues. We propose a model whereby Bbs proteins modulate NCC migration, contributing to craniofacial morphogenesis and development of the enteric nervous system. These migration defects also explain the association of Hirschsprung's disease (HD) with BBS. Moreover, this is a previously undescribed method of using characterization of facial dysmorphology as a basis for investigating the pathomechanism of CF development in dysmorphic syndromes.",

"abstractNote": "Many genetic syndromes involve a facial gestalt that suggests a preliminary diagnosis to an experienced clinical geneticist even before a clinical examination and genotyping are undertaken. Previously, using visualization and pattern recognition, we showed that dense surface models (DSMs) of full face shape characterize facial dysmorphology in Noonan and in 22q11 deletion syndromes. In this much larger study of 696 individuals, we extend the use of DSMs of the full face to establish accurate discrimination between controls and individuals with Williams, Smith-Magenis, 22q11 deletion, or Noonan syndromes and between individuals with different syndromes in these groups. However, the full power of the DSM approach is demonstrated by the comparable discriminating abilities of localized facial features, such as periorbital, perinasal, and perioral patches, and the correlation of DSM-based predictions and molecular findings. This study demonstrates the potential of face shape models to assist clinical training through visualization, to support clinical diagnosis of affected individuals through pattern recognition, and to enable the objective comparison of individuals sharing other phenotypic or genotypic properties.",

"abstractNote": "Sexual dimorphism is responsible for a substantial part of human facial variability, the study of which is essential for many scientific fields ranging from evolution to special biomedical topics. Our aim was to analyse the relationship between size variability and shape facial variability of sexual traits in the young adult Central European population and to construct average surface models of adult males and females. The method of geometric morphometrics allowed not only the identification of dimorphic traits, but also the evaluation of static allometry and the visualisation of sexual facial differences. Facial variability in the studied sample was characterised by a strong relationship between facial size and shape of sexual dimorphic traits. Large size of face was associated with facial elongation and vice versa. Regarding shape sexual dimorphic traits, a wide, vaulted and high forehead in combination with a narrow and gracile lower face were typical for females. Variability in shape dimorphic traits was smaller in females compared to males. For female classification, shape sexual dimorphic traits are more important, while for males the stronger association is with face size. Males generally had a closer inter-orbital distance and a deeper position of the eyes in relation to the facial plane, a larger and wider straight nose and nostrils, and more massive lower face. Using pseudo-colour maps to provide a detailed schematic representation of the geometrical differences between the sexes, we attempted to clarify the reasons underlying the development of such differences.",

"abstractNote": "OBJECTIVE\n\nTo evaluate three-dimensional (3D) facial morphology in patients surgically corrected for unilateral cleft lip and palate (UCLP) following pre-surgical nasoalveolar molding (NAM).\n\n\nDESIGN\n\nProspective, longitudinal study. Digital stereophotogrammetry was used to capture 3D facial images, and x, y, and z coordinates of five landmarks were digitized to compute mean morphologies. The sample comprised 15 patients with left UCLP and 10 matched control subjects. Facial form differences at age 37 weeks, using principal components analysis and finite-element scaling analysis (FESA) were assessed.\n\n\nRESULTS\n\nUsing the first two principal components, which accounted for 63% of the total shape-change, UCLP and control groups showed similar distributions in the modal space (p > 0.05). For the UCLP group, the mean 3D facial form was smaller and less protrusive when superimposed on the non-cleft mean. Using FESA, reductions in facial volume were found in the UCLP group, involving the columella (29%), labial tubercle (51%), lower lip (29%) and lateral aspects of the face (19%). The UCLP group also showed increases in size above the tip of the nose (25%) and laterally to the columella directly below the nares (29%).\n\n\nCONCLUSIONS\n\nFollowing surgical repair of UCLP in patients previously treated with NAM, 3D facial morphology was virtually indistinguishable from the non-cleft mean. Clinically, the apparent improvement in the facial soft tissues may mask dysmorphic skeletal growth, and further studies are required to characterize the underlying bony changes associated with the soft tissue changes reported here.",

"abstractNote": "Dense surface models can be used to analyze 3D facial morphology by establishing a correspondence of thousands of points across each 3D face image. The models provide dramatic visualizations of 3D face-shape variation with potential for training physicians to recognize the key components of particular syndromes. We demonstrate their use to visualize and recognize shape differences in a collection of 3D face images that includes 280 controls (2 weeks to 56 years of age), 90 individuals with Noonan syndrome (NS) (7 months to 56 years), and 60 individuals with velo-cardio-facial syndrome (VCFS; 3 to 17 years of age). Ten-fold cross-validation testing of discrimination between the three groups was carried out on unseen test examples using five pattern recognition algorithms (nearest mean, C5.0 decision trees, neural networks, logistic regression, and support vector machines). For discriminating between individuals with NS and controls, the best average sensitivity and specificity levels were 92 and 93% for children, 83 and 94% for adults, and 88 and 94% for the children and adults combined. For individuals with VCFS and controls, the best results were 83 and 92%. In a comparison of individuals with NS and individuals with VCFS, a correct identification rate of 95% was achieved for both syndromes. "abstractNote": "Bilateral complete cleft lip and palate (BCLP) is the most severe of the common orofacial clefts and is associated with the greatest deformity during development. The aim of this study was to use geometric morphometrics to evaluate palatal shape and size variability in patients with BCLP in comparison to nonclefted Czech boys. The variability of palatal size and shape in BCLP patients was greater when compared with the nonclefted population. Though palate variability in BCLP was wide, nonclefted palatal shapes were generally different (a high, wide and vaulted palate) and fell almost outside the range of BCLP variability. The palatal size and shape of BCLP patients (range from 12.1 to 16.5 years) was not correlated with age. A comparison of the mean shapes of the clefted and nonclefted groups showed that the BCLP palate is flatter and narrower. The most notable size difference was found in the area between the maxilla and premaxilla. This phenomenon is associated with the persisting separation of the premaxilla from the rest of the palate. The shape of the palatal configuration of the premaxilla and adjacent area was concave in the nonclefted group and convex in BCLP patients.",

"abstractNote": "OBJECTIVE\n\nFourier transform profilometry was used for the three-dimensional measurement of maxillary dental casts to analyze the size and shape of the palate. The objective of this study was to test the accuracy of the measuring system and determine the precision and reliability of the measurement\n\n\nMETHODS\n\nImages of dental casts were analyzed using newly developed measuring software. Based on five landmarks located on the alveolar ridge, the measuring software constructed 10 transversal sections of the palate. In each section profile, the width, area, and 23 height variables were assessed.\n\n\nSUBJECTS\n\nMaxillary dental stone casts of 25 healthy girls, 14.1 to 15.3 years of age, were studied.\n\n\nRESULTS\n\nThe technical error of measurement exceeded 5% of the size of the measurement only in variables with means less than approximately 3 mm. In fact, such small absolute dimensions were exhibited only by the palate height in anterior profile 2 and the palate height at the margins of other profiles. Reliability of the measurements was found to be very high for the width and area of the profiles. For height measurements, the coefficient of reliability was slightly lower at the profile margins than near the midline. Nevertheless, only three height variables showed a coefficient of reliability lower than 0.90. The coefficients of reliability of other height measurements of profiles 3 through 10 were only sporadically lower than 0.97.\n\n\nCONCLUSION\n\nWith regard to the accuracy of the measuring system as well as the precision and reliability of the measurement, this method proved to be a suitable tool for studying palatal morphology.",
