            "abstractNote": "BACKGROUND\n\nThe learning perspective of panic disorder distinguishes between acute panic and anxious apprehension as distinct emotional states. Following animal models, these clinical entities reflect different stages of defensive reactivity depending upon the imminence of interoceptive or exteroceptive threat cues. The current study tested this model by investigating the dynamics of defensive reactivity in a large group of patients with panic disorder and agoraphobia (PD/AG).\n\n\nMETHODS\n\nThree hundred forty-five PD/AG patients participated in a standardized behavioral avoidance test (being entrapped in a small, dark chamber for 10 minutes). Defense reactivity was assessed measuring avoidance and escape behavior, self-reports of anxiety and panic symptoms, autonomic arousal (heart rate and skin conductance), and potentiation of the startle reflex before and during exposure of the behavioral avoidance test.\n\n\nRESULTS\n\nPanic disorder and agoraphobia patients differed substantially in their defensive reactivity. While 31.6% of the patients showed strong anxious apprehension during this task (as indexed by increased reports of anxiety, elevated physiological arousal, and startle potentiation), 20.9% of the patients escaped from the test chamber. Active escape was initiated at the peak of the autonomic surge accompanied by an inhibition of the startle response as predicted by the animal model. These physiological responses resembled the pattern observed during the 34 reported panic attacks.\n\n\nCONCLUSIONS\n\nWe found evidence that defensive reactivity in PD/AG patients is dynamically organized ranging from anxious apprehension to panic with increasing proximity of interoceptive threat. These data support the learning perspective of panic disorder.",

            "abstractNote": "OBJECTIVE\n\nMajor depressive disorder is associated with increased cardiac mortality. A decrease in vagal modulation related to reduced heart rate variability might contribute to increased mortality among many other factors. We sought to examine the hypothesis that nortriptyline treatment will be associated with a decrease in heart rate variability and coupling between heart rate and respiration compared to treatment with S-citalopram.\n\n\nMETHODS\n\nFifty-two patients suffering from major depression were included. Patients were examined unmedicated in the acute stage and after 5weeks of treatment. Twenty-six were reinvestigated after they received S-citalopram and 26, after nortriptyline. We used non-linear measures of heart rate variability and also a novel measure to examine cardio-respiratory coupling to assess cardiac vagal modulation.\n\n\nRESULTS\n\nThere were significant decreases of non-linear measures of heart rate variability in the nortriptyline group in addition to reduced cardio-respiratory coupling in comparison to the group of patients that received S-citalopram. We observed a significant association between the severity of the disease and vagal withdrawal prior to treatment.\n\n\nCONCLUSIONS\n\nThese findings indicate that S-citalopram influences autonomic modulation on different regulatory levels to a lesser extent than nortriptyline. Our results have implications for treatment of patients with depression as some of them may already have a higher risk for cardiovascular mortality. In addition, it underlines the beneficial use of SSRIs in patients with cardiac diseases.",

            "abstractNote": "Ambulatory monitoring has gained powerful new tools due to recent electronic and computer advances. The capability simultaneously to monitor numerous physiological parameters and behavior enhances the ecological validity of field assessment, but methodological challenges abound that can compromise attempts to understand biobehavioral relations in the real world. A major obstacle is that physiological dysregulation or emotion effects can be masked by variation in physical activity. Using a multi-channel ambulatory recording system, a wide array of self-report, physiological and environmental measurements was collected from 28 participants during quiet sitting, physical exercise and an emotion induction consisting of a short commercial flight. Half of the participants were selected to respond to flying with intense anxiety, the other half, with moderate excitement. Recorded channels included ECG, EDA, calibrated respiration pattern, and skin temperature, from which 17 physiological parameters were calculated. Accelerometry and self-report in an emotion diary served as manipulation checks. Results indicate that many parameters, including heart rate, respiratory sinus arrhythmia, and skin conductance level and its fluctuation rate, were strongly and nonspecifically affected by both anxiety and exercise. However, parameters of respiratory volume were particularly responsive to exercise, while certain parameters of irregularity in breathing were to anxiety. Several respiratory timing parameters were responsive to both exercise and excitement. We conclude that physiological measures provide information helping to distinguish emotional from physical activation. However, additional context awareness is necessary for confident data interpretation in ambulatory recording. This can be achieved by specific channels such as accelerometry, items in an electronic diary, semi-structured protocols, and statistical modeling.",

            "abstractNote": "<AbstractText Label=\"OBJECTIVE\" NlmCategory=\"OBJECTIVE\">Our objective is to summarize the new findings concerning the respiratory subtype (RS) of panic disorder (PD) since its first description.</AbstractText>\n<AbstractText Label=\"METHODS\" NlmCategory=\"METHODS\">Two searches were made in the Institute for Scientific Information Web of Science: with the keywords \"panic disorder\" and \"respiratory symptoms,\" and all articles that cited Briggs and colleagues' 1993 article \"Subtyping of Panic Disorder by Symptom Profile\" (Br J Psychiatry 1993;163:201-9). Altogether, 133 articles were reviewed.</AbstractText>\n<AbstractText Label=\"RESULTS\" NlmCategory=\"RESULTS\">We describe and discuss RS epidemiology, genetics, psychopathology, demographic features, clinical features, correlations with the respiratory system, traumatic suffocation history, provocative tests, and nocturnal panic. Compared to patients with the nonrespiratory subtype (non-RS), the RS patients had higher familial history of PD, lower comorbidity with depression, longer duration of illness, lower neuroticism scores, and higher scores in severity scales, such as the Panic and Agoraphobia Scale, Panic-Agoraphobia Spectrum scale and the Clinical Global Impression scale. Tests to induce panic attacks, such as those with CO(2), hyperventilation, and caffeine, produce panic attacks in a higher proportion of RS patients than non-RS patients. Differences in the subtypes' improvement with the pharmacologic treatment were found. There are also some controversial findings regarding the RS, including the age of onset of PD, and alcohol and tobacco use in RS patients.</AbstractText>\n<AbstractText Label=\"CONCLUSIONS\" NlmCategory=\"CONCLUSIONS\">Some characteristics, such as the increased sensitivity to CO(2) and the higher familial history of PD, clearly distinguish the RS from the non-RS.             "abstractNote": "Respiratory sinus arrhythmia (RSA) is classically described as a vagally mediated increase and decrease in heart rate concurrent with inspiration and expiration, respectively. However, although breathing frequency is known to alter this temporal relationship, the precise nature of this phase dependency and its relationship to blood pressure remains unclear. In 16 subjects we systematically examined the temporal relationships between respiration, RSA, and blood pressure by graphically portraying cardiac interval (R-R) and systolic blood pressure (SBP) variations as a function of the respiratory cycle (pattern analysis), during incremental stepwise paced breathing. The principal findings were 1) the time interval between R-R maximum and expiration onset remained the same ( approximately 2.5-3.0 s) irrespective of breathing frequency (P = 0.10), whereas R-R minimum progressively shifted from expiratory onset into midinspiration with slower breathing (P < 0.0001); 2) there is a clear qualitative distinction between pre- versus postinspiratory cardiac acceleration during slow (0.10 Hz) but not fast (0.20 Hz) breathing; 3) the time interval from inspiration onset to SBP minimum (P = 0.16) and from expiration onset to SBP maximum (P = 0.26) remained unchanged across breathing frequencies; 4) SBP maximum and R-R maximum maintained an unchanged temporal alignment of approximately 1.1 s irrespective of breathing frequency (P = 0.84), whereas the alignment between SBP minimum and R-R minimum was inconstant (P > 0.0001); and 5) beta(1)-adrenergic blockade did not influence the respiration-RSA relationships or distinct RSA patterns observed during slow breathing, suggesting that temporal dependencies associated with alterations in breathing frequency are unrelated to cardiac sympathetic modulation. Collectively, these results illustrate nonlinear respiration-RSA-blood pressure relationships that may yield new insights to the fundamental mechanism of RSA in humans.",

            "abstractNote": "Abstract Researchers are interested in respiratory sinus arrhythmia (RSA) as an index of cardiac vagal activity. Yet, debate exists about how to account for respiratory influences on quantitative indices of RSA. T. Ritz, M. Thons, and B. Dahme (2001) developed a within-individual correction procedure by which the effects of respiration on RSA may be estimated using regression models. We replicated their procedure substituting a spectral high-frequency measure of RSA for a time-domain statistic and a respiratory belt's relative measure of tidal volume for the direct assessment provided by a pneumotachograph. The standardized slopes from the respiratory belt and pneumotachography-derived regression equations (estimated across a 6-min paced breathing protocol) were positively correlated (r=0.93, p<.001); correlations were similar across 2- and 4-min time courses parsed from the 6-min protocol. Our results offer methodological alternatives to the research community.",

            "abstractNote": "Respiratory sinus arrhythmia (RSA) or high-frequency heart rate variability has been widely used as a noninvasive measure of cardiac vagal tone. However, their dependency on both respiration rate and tidal volume is largely ignored. Only a minority of studies published in Psychosomatic Medicine in recent years has implemented precautions for controlling respiration rate in RSA measures, and tidal volume effects were only rarely addressed. We discuss methodologic issues related to respiratory control methods and present data that demonstrate that both respiration rate and tidal volume contribute substantially to the within-individual RSA variance under conditions of variable breathing, with tidal volume contributing up to one third beyond respiration rate. Finally, we outline a respiratory control method for the time-domain index of RSA and review research pertaining to its reliability, validity, and experimental application.",

Our aim was to determine the influence of the type of task, the absolute and relative difficulty of the task, and the perceived stress associated with performance of the task on sympathetic circulatory regulation during cognitive challenge in humans. 2. Sympathetic nerve activity to skeletal muscle (MSNA) determined from peroneal microneurography, heart rate and arterial blood pressure were recorded continuously in twelve subjects during a modified Stroop colour word test (CWT) and mental arithmetic (MA), each performed over six levels of increasing absolute task difficulty. Performance (percentage correct) on each task was assessed and ratings of perceived stress obtained. Responses to CWT and MA were compared at similar levels of performance and perceived stress. 3. MSNA decreased at task onset, remained below baseline levels at low levels of difficulty which were not perceived as stressful, increased above baseline levels at higher levels of difficulty which were perceived as stressful, and increased further during recovery. Thus, the regulation of MSNA was stress dependent. At similar levels of stress perception there were no differences in MSNA between CWT and MA. Although performance declined as task difficulty increased, there was no particular 'threshold' level of performance associated with the stimulation of MSNA. 4. Arterial pressure and heart rate were elevated above baseline levels throughout the mental tasks. Arterial pressure increased over the first 3-4 levels of each task and then plateaued whereas heart rate did not vary across increasing levels of task difficulty. Heart rate and arterial pressure responses to CWT were higher than those to MA. 5. These data demonstrate that during cognitive challenge the stimulation of MSNA is governed primarily by perceived stress which is dependent, in part, on the absolute level of task difficulty. In contrast, neither performance nor the type of cognitive task appear to be important determinants of MSNA. Arterial pressure is influenced by the task and level of difficulty. Heart rate is independent of task difficulty but may be task dependent.",

            "abstractNote": "This study examines similarities and differences in fear potentiation between two protective reflexes: cardiac defense and eyeblink startle. Women reporting intense fear of animals but low fear of blood or intense fear of blood but low fear of animals viewed pictures depicting blood or the feared animal for 6 s in 2 separate trials in counterbalanced order. An intense burst of white noise, able to elicit both a cardiac defense response and a reflexive startle blink, was presented 3.5 s after picture onset. Both cardiac and blink responses were potentiated when highly fearful individuals viewed fearful pictures. However, differences appeared concerning picture order. This pattern of results indicates communalities and differences among protective reflexes that are relevant for understanding the dynamics of emotional reflex modulation.",
