In order to highlight opportunities and challenges of the evaluation of studies applying machine learning, we use examples from cardiac electrophysiology, a field characterized by large and often imbalanced amounts of data. We provide recommendations and guidance on evaluating and presenting supervised machine learning studies. We recommend proper cohort selection, keeping training and testing data strictly separate, and comparing results to a reference model without machine learning as basic principles to ensure the quality of studies using machine learning methods. We furthermore recommend specific metrics and plots when reporting on machine learning including on models for multi-channel time series or images. The study data set consisted of 744 body surface potential map (BSPM) recordings. The BSPMs, each of which was made up of 117 leads, were recorded from a mixture of healthy, myocardial infarction, and left ventricular hypertrophy subjects. The BSPMs were interpolated to increase the number of data points in the region of the EASI recording electrodes I, E, and A and the precordial leads. This facilitated 3 experiments. Firstly, recording sites I, E, and A were simultaneously moved ±5 cm vertically, in 0.5 cm increments, from their correct locations. Secondly, recording sites I and A were moved horizontally, again up to ±5 cm, in 0.5 cm increments. Finally, all 6 precordial leads were moved vertically in 0.5 cm increments up to ±5 cm. At each movement step, the resulting 12-lead ECG was compared with the original 12-lead ECG. Root mean square error was determined along with the absolute difference in J-point amplitude. Although the EASI leads were found to be less sensitive to electrode misplacement than the standard precordial leads, it was found that when precordial leads were moved up to ±3 cm vertically, the resulting 12-lead ECG more accurately resembled the original 12-lead ECG than a 12-lead ECG reconstructed from accurately positioned EASI leads. The ACM profile may lead to interoperability between systems of different manufacturers and may result in allowing alarm messages to be communicated in a standardized way. We have implemented an extensible, open source framework (C#, ASP.NET) based on the ACM 2008 standard and interfaced our patient monitoring equipment (Draeger Medical), and infusion pumps (Alaris) using several export interfaces (RSS, ESPA, XMPP, SMS, SNPP) available in the intensive care units of the Erasmus MC. The framework can be extended by plug ins for devices that generate alarms and those that display alarms. Alarms can be archived to a SQL server database. The open source framework is available on http://i-put.sourceforge.net. The framework enables new applications which may improve the accuracy of the alarms. Patient monitoring and therapeutic devices at the bedside generate many audible and visual alarms which interrupt care as they require a clinician to review each alarm. Few of the alarms actually represent life threatening events; most result from (slightly) abnormal values or artifacts. These alarms are often presented on different screens, use different sounds and visual effects. Also, to change specific alarm settings, each device needs to be configured separately. Finally, devices do not exchange alarm information with other devices of the same patient, so each device will generate its own stream of alarms. Thus, a single physiological event may trigger different alarms (time, message and/or level) for each attached device. Using the ACM profile can lead to systems which improve the frequency and presentation, as well as the accuracy of these alarms. METHODS: For all patients with overridden DDI alerts on QTc prolongation during 6 months, data on risk factors for QT prolongation, drug class and ECGs were collected from the medical record. Patients with ventricular pacemakers, patients treated on an outpatient basis, and patients using the low-risk combination of cotrimoxazole and tacrolimus were excluded. The magnitude of the effect on the QTc interval was calculated if ECGs before and after overriding were available. Changes of the QTc interval in these cases were compared with those of a control group using one QTc-prolonging drug. RESULTS: In 33% of all patients with overridden QTc alerts an ECG was recorded within 1 month. ECGs were more often recorded in patients with more risk factors for QTc prolongation and with more QTc overrides. ECGs before and after the QTc override were available in 29% of patients. Thirty-one percent of patients in this group showed clinically relevant QTc prolongation with increased risk of torsades de pointes or ventricular arrhythmias. The average change in QTc interval was +31 ms for cases and -4 ms for controls. CONCLUSIONS: Overriding the high-level DDI alerts on QTc prolongation rarely resulted in the preferred approach to subsequently record an ECG. If ECGs were recorded before and after QTc overrides, clinically relevant QTc prolongation was found in one-third of cases. The study population consisted of 117 lead body surface potential maps (BSPMs) recorded from 559 subjects (approximately one third normal, one third MI and one third LVH). The BSPMs were interpolated to increase the number of recording sites in the vicinity of V2 and V5. For QRS segments the median RMS error across all reconstructed leads was 220.4 microV, 171.4 microV, 277.8 microV when V2 and V5 were simultaneously moved -50mm vertically, 0mm, and +50mm vertically respectively. For STT segments these values were 66.8 microV, 54.3 microV and 76.9 microV respectively. We observed that during the QRS segment the most accurate reconstruction was at -15mm (RMS error: 154.4 microV). During the STT segment the reconstruction error was at its minimum at -20mm (RMS error: 48.5 microV). A similar increase in performance for STT reconstruction was observed at 15mm (RMS error: 48.6 microV). When an abnormality is detected, an alarm is generated. However, most alarms do not signify a life-threatening event. The distribution of alarms by category and over time is not well known, but may provide information on how to improve the accuracy. During a four month period all alarms from the 8-bed ICCU at the Erasmus MC were collected in a database by receiving the pager messages generated by the patient monitoring network. The alarms were subsequently categorized by type, hour and day of week. Over a four month period 34827 alarms were collected during 547 patient admissions with a median (IQR) duration of 6.1(3-23) hours. The most frequent alarm categories were related to mechanical ventilation (42.2%), blood pressure (32.3%), electrocardiogram (9.8%) and heart rate (8.1%). 2750 (7.9%) of the alarms were not limit violation alarms, but technical advisory messages. Main causes of these advisory messages were: ECG artifacts (64.9%), disconnected devices (29.3%) and SpO2 artifacts (1.6%). The mean alarm frequency was 2.2 per patient and 13.4 for the ICCU per hour. The time between alarms varied greatly with a median (IQR) of 1.3 (0.5-4.1) minutes, mainly due to the occurrence of alarm “bursts”: in 50% of the alarms the interval was less than 90 seconds. There was overall decreased alarm frequency during nighttime (0.00 until 6.00) when compared to day: 9.7 vs. 14.6 per hour (P<0.001). A peak in alarm frequency was seen at the time of drawing blood samples from the arterial line and awakening of the patients. Another peak occurred between 8.00 and 10.00 when patients are washed. The alarm frequency during these peaks was 20.3 per hour vs. 12.4 during other hours (P<0.001). There was no difference in frequency of alarms by day of week. Overall alarm frequency on the ICCU was relatively low. However, the distribution over time varied greatly due to the occurrence of alarm “bursts”.
